Employee was lying under the D7 preparing to change the oil. IE was taking off belly pan plate when IE's coworker, who was above IE, was taking off the steel plate to get to the oil filter when EE dropped the plate and it went through the tracks and hit employee on the head.

Was removing track pin from loading, twisted and felt a pain in middle of back. Note: EE went to a chriopractor set up thru. safety director. We found this out at a later date.

GRABBED HAND RAIL TO PULL HIMSELF UP ON MACHINE TO MAKE REPAIRS AND FELT FOOT SHIFT AND HAD SLIGHT PAIN IN BACK. WORKED UNTIL 11/3/03 THEN WENT TO DR. 11/4/03. DR. HAS HIM OFF UNTIL HE GETS RESULTS OF CAT SCAN.

EE WAS LIFTING CLUTCH DRIVER AND FELT DISCOMFORT IN ABDOMEN. HE REPORTED THIS TO US ON 3/19/01. WE MADE HIM A DOCTOR'S APPOINTMENT. DOCTOR SAID COULD BE A STRAIN OR LIGHT RUPTURE. EE CONTINUED TO WORK UNTIL 11/13/01. I WAS ON VACATION AND WHEN I RETURNED, EE HAD BEEN TO THE DOCTOR'S AND WENT INTO THE HOSPITAL TO HAVE SURGERY.
