Employee was servicing D10N dozer, stepping down from the dozer they twisted wrong and felt a pop in their hip.

Struck against a moving object

Driver was backed up to the dump location and started to raise the trailer. As the bed was being raised, the driver noticed the bed starting to sway. EE hit the down lever. As the bed was coming down, it started to lean to the right and then continued to the ground on its passenger side. Tractor and trailer were damaged. Load not evenly distributed was leading factor.

Employee was welding then began to use a chipping hammer. A hot chip flew up and dropped behind EE's safety glasses hitting EE's right eye.

Started the loader to let it warm up first thing in the morning, and when employee was climbing down the ladder, foot slipped and employee tried to catch self. Employee held onto the rail with right arm, and while doing this felt a popping sensation in right shoulder.

Employee was lying under the D7 preparing to change the oil. IE was taking off belly pan plate when IE's coworker, who was above IE, was taking off the steel plate to get to the oil filter when EE dropped the plate and it went through the tracks and hit employee on the head.

The employee was inside the shop turning the head on a starter. He completed the task and climbed up on the tailgate of Truck #77 and was bending down to lift the starter into the bed of his truck so that he may take it back to the pit and complete the maintenance task. While doing so his left foot slipped and his leg moved forward. He heard a pop in his left leg.

Employee was using a utility knife to cut a tie off of a package. Cut tie and swung the knife upward and lacerated his left forearm.

Cleaning roller off a got hand stuck between roller and belt. Cut left hand and belt rash to left thumb

Fall to the walkway or working surface

Employee was getting down from a piece of equipment uses a small ladder when his boot buckle got caught on the step. He slipped on ice with his other foot, causing him to fall on his back. He hit his back and head on the ground. Accident report filled out on 2/2/09, did not seek medical attention until 2/20/09.

Exiting tractor trailer and left heel of boot caught on the step on the fuel tank causing employee to land hard on my right foot. Felt pain in lower back.

Was removing track pin from loading, twisted and felt a pain in middle of back. Note: EE went to a chriopractor set up thru. safety director. We found this out at a later date.

EE HAS A PRE-EXISTING BACK INJURY. HE TOLD MY FOREMAN HE HAD RECEIVED EPIDURAL FOR HIS BACK THE WEEK BEFORE AND IT WAS BOTHERING HIM. HE WENT BACK TO HIS DR. ON 4-16-02. HIS DR. HAS HIM SCHEDULED FOR ANOTHER EPIDURAL ON 5-13-02 AND OFF WORK TILL AT LEAST 5-20-02. EE GAVE US NO INDICATION OR REASON FOR HIS BACK BEING AGGRIVATED.

WAS TIGHTENING A JAM NUT WITH A WRENCH AND SLIPPED OFF NUT HITTING LEFT HAND. LEFT RING FINGER HURT SO WE TOOK HIM TO GET A X-RAY AND FOUND A BROKEN BONE ON LEFT RING FINGER BETWEEN TIP OF FIN GER AND FIRST JOINT. HE RETURNED TO WORK WITH A COVER OVER TIP OF FINGER.

EE WAS LIFTING CLUTCH DRIVER AND FELT DISCOMFORT IN ABDOMEN. HE REPORTED THIS TO US ON 3/19/01. WE MADE HIM A DOCTOR'S APPOINTMENT. DOCTOR SAID COULD BE A STRAIN OR LIGHT RUPTURE. EE CONTINUED TO WORK UNTIL 11/13/01. I WAS ON VACATION AND WHEN I RETURNED, EE HAD BEEN TO THE DOCTOR'S AND WENT INTO THE HOSPITAL TO HAVE SURGERY.

EE WAS WALKING DOWN TO A SUMP CHECKING HIS CAUSTIC LINES. HE SLIPPED ON SNOW AND ICE AND FELL. HE GRABBED HOLD OF A STAKE AS HE FELL WHICH WAS HOLDING THE END OF THE CAUSTIC LINE. THE CAUSTIC( SODIUM HYDROXIDE) SLPASHED HIM IN THE FACE AND IN THE LEFT EYE. WE FLUSHED HIS FACE AND EYE AND TOOK HIM TO GET CHECKED OUT AT HOSPITAL.

GETTING OUT OF TRUCK AFTER DINNER AND TWISTED RIGHT ANKLE. WENT TO THE DR ON 9/29/00. CAME TO WORK ON MON 10/2/00 WITH A SLIP FROM DR FOR LIGHT DUTY. WE DID NOT HAVE ANY LIGHT DUTY WORK, SO HI S DR PUT HIM OFF FOR APPROX 2 WEEKS. THIS IS THE SAME ANKLE EE HAD AN OPERATION ON BEFORE. I WAS ON VACATION FROM 9/29/00 TO 10/8/00 RETURNING TO WORK ON 10/9 & FOUND OUT HE WAS OFF.

LIFTING A TRACK ROLLER WITH TRUCK CRANE. THE ROLLER SLIPPED DOWN THE CHAIN AND PINCHED FINGER AGAINST ANOTHER ROLLER ON THE TRUCK BED. WASN'T TOO BAD BUT THE DR PUT IN A FEW STITCHES. NOTHING BROKE.
