A truck and pup driver was preforming is pre-trip inspection on the truck to start the shift. When lifting the hood of the truck, the employee felt discomfort in the right elbow. Employee was treated with first aid onsite. When visiting the doctor on 10/21/24 the employee was placed on work restrictions resulting in a reportable injury.

Showing all 33The injury has been diagnosed as an incisional hernia. The Hernia had been originally repaired in March of 2020. The repair to the hernia began to pull apart. The employee could not pin point the time or the cause of the start of the pain. The employee was doing EE's Normal work routine.
Employee was lifting an auger bit on the rack and dropped it. It pinched/crushed the tip of left ring finger.
Employee was cleaning a material pipe when the water pressure caused material to come out of the pipe and strike EE in the chin. Resulted in a laceration to the chin.
Employee was washing the pad under and around the plant. EE stood up and hit head on the frame of the plant. The angle iron support caused a laceration to the right eye/forehead.
Employee was changing a shaker screen. When tossing the old screen down to the ground, he caught his arm on a sharp point of the guard where they change the screen. Caused a laceration to his left bicep.
Employee was lifting a hydraulic pump and felt a pop in his back. Caused back pain and numbness and tingling into his legs.
Employee was changing a filter on the diesel engine on the dredge. When pulling the filter out of the housing, the handle on the filter broke, cutting the employee's right middle finger.
Stepped off the lowboy trailer and when his feet met the ground he had "discomfort" in his lower back.
Employee was climbing down a ladder and a piece of iron fell from the top of the ladder striking him in the left foot.
Helping change screen in the plant. Picked up roll of screen and felt pop & pain in left shoulder. On restricted work activity but can still operate dredge. Taking anti-inflammatory meds.
Lifting cable from spool and strained abdomen while pulling on the wire cable. Possible hernia. Hernia surgery is scheduled for 5/11/12, first day of lost time.
EE stepped down onto the rubber dredge hose which was 2 feet lower than steel platform, when a 500 lb steel sheave & block slipped off a steel platform & struck his left ankle with a glancing blow, causing a deep abrasion & fracture. The abrasion required 3 layers of stitches.
When changing screens on the rotary, the employee's right index finger tip was pinched between the hoisting chain and the rotary. The employee's finger nail was removed and sutured.
The employee was pushing the plastic fusing machine by hand with a lot of force and felt immediate pain in back of right lower leg, in the calf area. Received medical attention, RX for pain and muscle relaxant and several days of restricted work activity.
Employee was moving the dredge anchor cable that was against the workboat railing, when his left thumb was pinched between the railing cable, cut need 8 stitches to close wound.
When doing lower level dregde floor repairs, they moved a 8" H-beam forward, it came off the rails it was resting on, as the steel beam fell it caught his hand between the beam and a hydraulic pump.
A metal guard fell on his right hand while removing it from a gear box, causing a minor fracture to his right hand. A cast was placed on his right hand--no lost time.
The employee was helping move a heat exchanger from the work boat to the dredge to be installed, when he felt a strain in his abdomen. Went to the doctor on 9-14-06 & will need hernia surgery. Not lost time yet.
The ee was helping to install grizzley screens that replaced the old one. The screen was jammed & stuck when it came loose it dropped on the ee's left arm causing a laceration that required 8 stitches. He also had a tetnus shot.
THE EMPLOYEE WAS PERFORMING MAINTENANCE ON THE TRUCK SCALE PEDESTAL, WHEN IT SLIPPED OUT OF HIS HAND PINCHING & LACERATING HIS LEFT RING FINGER & LEFT LITTLE FINGER. EE REQUIRED STITCHES.
AFTER LOADING THE LOADER ONTO THE LOW-BOY TRAILER, THE EE SLIPPED ON THE TIRE WHEN COVERING THE EXHAUST & REMOVING THE ANTENNA. HE BRACED HIS FALL FROM ABOUT 5 FT WITH HIS RIGHT ARM STRAINING HIS RIGHT SHOULDER
The employee was moving a container of welding rod to the back of the truck, the container slipped, cutting his rt. index finger, that required sutures. No restrictions.
THE EMPLOYEE WAS ASSISTING WITH THE INSTALLATION OF A REPLACEMENT TAIL PULLEY ON A CONVEYOR. HE REACHED DOWN TO GRAB THE SHAFT TO POSITION IT WHEN IT SLID OVER AND PINCHED HIS LEFT HAND.
THE EMPLOYEE WAS HELPING WITH REMOVING SCREENS FROM THE ROTARY WHEN THE SCREEN SLIPPED & THE EDGE OF THE SCREEN WENT UNDER HIS GLOVE, CUTTING THE BACK OF HIS LEFT HAND, REQUIRING 6 STITCHES.
EE WAS REMOVING A DOGHOUSE FROM A ROTARY SCREEN. IT CAME LOOSE AND HIT HIM IN THE RIBS, BREAKING ONE RIB ON HIS RIGHT SIDE.
EMPLOYEE WAS WORKING ON DREDGE, REPLACING A PART ON THE PUMP AND CHANGING WORN PIPES. HE WAS LIFTING PIPES AND TIGHTENED BOLTS ON PIPELINE AND STRAINED HIS BACK.
EMPLOYEE WAS REPAIRING WENCH ON DREDGE WHILE STANDING ON A WET HOSE. HE SLIPPED AND TRIED TO CATCH HIMSELF WITH HIS RIGHT ARM. THE EMPLOYEE RUPTURED HIS RIGHT BICEP MUSCLE REQUIRING SURGERY.
THE EE WAS HELPING TO DISMANTLE THE DREDGE WHEN DOING SO HE MOVED A FLANGED PIPE THAT SLID AND TIPPED OUT OF THE SADDLE PINCHING AND SEVERING THE END OF THE EE'S RIGHT THUMB BETWEEN THE PIPE FRAME AND CORNER OF THE DREDGE CABIN. THE EE HAD SURGERY ON 9-6-02 TO PIN BONE ON THE END OF THE THUMB.
THE EMPLOYEE WAS HELPING SET AN EMPTY FUEL TANK ON A LOWBOY TRAILER WITH A PETTIBONE CRANE. TANK WAS BEING SET BETWEEN TWO TUBES ON THE LOWBOY TRAILER. THE WIND SHIFTED THE TANK CAUSING A TUBE THAT THE EMPLOYEE WAS HOLDING TO GUIDE THE TANK INTO PLACE TO FALL. THIS STRUCK THE EMPLOYEES FINGERS CAUSING THE CONTUSIONS & LACERATIONS NEEDING STITCHES. NO LOST TIME, SOME RESTRICTED WORK
THE EMPLOYEE WAS CHANGING SHAKER SCREENS. WHILE DOING SO THE SCREEN SLIPPED, PINCHING & CRUSHING HIS RIGHT MIDDLE FINGER BETWEEN THE SCREEN & SCREEN FRAME. FRACTURE & TORN TENDON OF THE RIGHT MIDDLE FINGER.
THE EMPLOYEE WAS ASSISTING WITH REPLACEMENT OF STEEL PADS IN THE DE-WATERING SCREW, WHEN APAD SLIPPED OUT OF HIS RIGHT HAND, STRIKING THE TIP OF HIS LEFT RING FINGER, CAUSING A FRACTURE OF BON E TIP AND ABRASION. 30-35 LB FLAT STEEL PAD FOR THE DE-WATERING SCREW.
THE EE WAS LIFTING A STEEL INSPECTION PLATE LID OFF THE SCALE DECK TO PUMP WATER OUT FROM BELOW THE SCALE. HE TRIEDF TO LIFT THE LID & IT APEARED TO BE STUCK, SO HE JERKED HARDER TO OPEN THE L ID. WHEN DOING SO, HE FELT LIKE A RIPPING PAIN IN HIS LWOER BACK. HE IS NOW ON RESTRICTED WORK ACTIVITY, SO AS NOT TO AGGRAVATE HIS BACK CONDITION.
