Recent fatalities in Nebraska
The driver was operating a articulated truck hauling dirt from the back of the lake. Driver lost control of the vehicle for an undetermined reason and drove the vehicle over the berm and into the lake. Operator was removed from the truck and transported to local hospital then later moved to Kearney then transferred to Omaha Hospital where he died on 3/19/15.
Forklift turned over - The miner was operating a forklift, traveling on a decline toward the mine entrance, when the forklift went out of control. The forklift struck a concrete support for the belt conveyor and overturned, killing him.
Two front end loaders, one on each side, were being used to position an 80 foot section of 18 inch diameter pipe in the waters edge. When the loader on the right was about 25-30 feet from the lake, the bank started to cave in. Within 3 minutes the loader was submerged in about 20 feet of water.
The victim was dumping sand and gravel into the plant grizzly with a skid loader when he over traveled the stop block. The loader rolled into the grizzly and overturned. The victim was thrown out of the loader and fatally injured.
ONE EMPLOYEE INSIDE CAB OF TRUCK, ONE EMPLOYEE UNDER TRUCK TRYING TO TROUBLE-SHOOT PROBLEM WITH THE PTO CABLE. BED RAISED INADVERTENTLY AND VICTIM PLACED HIMSELF BETWEEN THE TRUCK BED AND FRA ME TO WATCH EMPLOYEE UNDER THE TRUCK BED. BED INADVERTENTLY LOWERED ON THE VICTIM. HE WAS TAKEN TO THE LOCAL HOSPITAL AND PASSED AWAY SEVERAL HOURS LATER.
WAS NOT WEARING LIFT JACKET WHEN CLEANING OUT DEBRIS FROM CUTTERHEAD, FELL INTO WATER AND DROWNED.
