Employee was hit by an anvil from a VSI crusher. The anvils were being removed and thrown out of the machine by another employee. The employee that was struck walked into the area where the anvils were being tossed to the ground. EE was struck in the lower back and buttocks. EE had scrapes and bruising.

Showing all 28Dragging cords feeling pop in lower back. Tried first aid when injury occurred. First visit to see a Dr. was on 8/7/23 with no additional treatment. 8/14/23 for a recheck appointment receiving additional treatment making injury reportable.
Employee was placing angle iron onto a conveyor when EE caught finger between the metal. EE received a laceration to the tip of the finger that required two stitches.
Employee was lifting the cover on the hydraulic tank and a finger got caught fracturing the pinky finger.
The employee was moving 480 volt power cords during plant set-up and strained left bicep. 1st aid treatment at that time. On 9/10/2017 pain was getting worse, ee was taken back to the doctor where ee received lifting restrictions and set-up with physical therapy appointments.
Employee was changing screens on the Wash Plant. Lumbar strain injury.
Employee was pushing the door release on the excavator and the door release knob broke and a sharp piece cut hand.
Employee was placing a chain around a rock that was stuck in a Jaw crusher. The rock shifted and pinched his hand between the rock and the sidewall of the crusher. He fractured a bone on his left hand.
Employee was lifting power cords overhead and felt a "pop" in his neck and shoulder. He was taken to our doctor who referred him to a chiropractor. The chiropractor adjusted his neck and shoulder.
Employee was dragging the fuel hose to the loader to fuel it up. When dragging the hose back to the tank he felt a pop in his hip.
While servicing the cone crusher, operator's left finger tip was crushed when keeper ring fell while being removed. Medical treatment included xray, antibiotic and pain med, and dressing of wound. Distal finger bone is broken and nail was removed.
Miner received steam burns to his right arm while attempting to service a pump. The pump had lost its prime and had run dry for a period of time building up heat and steam. Miner attempted to open the filler plug and did not notice pump body was hot, and when he unscrewed the plug the steam escaped burning his arm.
Employee had been cutting a pipe with a band-saw so it would fit in the dumpster. A piece of pipe was on the rack where it had been cut off and employee said when he walked by the pipe it rolled and fell off poking his arm on the way down.
4000 cone was packed tight with crused rock. EE entered from underneath cone to free material, when material came down on top of EE. He was smothered and confined for one hour til he was released.
Tightening head pulley scraper on #2 sand conveyor with bungee cord. Scraper was worn & popped under head pulley. The scraper tensioning arm spun around and struck top of right hand.
Tearing down crusher wrapping cords, taking out the plywood for the ramp, deblocking, move conveyor and picking up. Did have safety glasses on.
Changing screen on wash plant. Dropped socket & went underneath wash plant to remove drain cover. Cover broke, employee was on stomach, pushed back on arms and got right elbow stuck between machine and ground. Pulled arm back out with force.
Climbing out of loader stepped off last step of ladder and twisted knee.
Taking chain binder off backhoe bucket from trailer, pipe slipped, struck him on chin and left cheek. Laceration to cheek requiring 15 stitches.
SHOVELING ROCK SPILLAGE FROM CATWALK ON WASH PLANT - BACK "POPPED" AND CAUSED PAIN.
Employee was in the shop changing a cone and got a foregn body in his right eye.
EMPLOYEE WAS CLEANING OUT JAW THAT WAS FULL OF ROCK. WHILE BENDING FORWARD AND SHOVELING OUT JAW, ROCK GAVE WAY FROM THE OVERHEAD FEEDER AND FELL ONTO HIS LOWER BACK.
Ee was changing cone on crusher. A metal burr was protruding from the crusher bowl.
EE WAS SHOVELING DOWN CONVEYER LINE. EXPERIENCED PAIN IN RT SIDE OF LOWER BACK.

EMPLOYEE WAS INSTALLING SCREEN & STRAINED UPPER BACK & NECK.
FIXING FASTENERS ON CONVEYOR. PIECE OF FASTENER STRUCK EE IN RT. EYE.
WHILE PUSHING A PIECE OF STEEL TO AN UPRIGHT POSITION E.E. SLIPPED ON A BOARD CAUSING A JARRING MOTION TO THE BACK.
