Employee was working on the Jaw Feeder and was removing the Jaw Grizzly Bars. EE was helping to remove the old Grizzly Bars. One of the bars hung up while EE was trying to move it. When the bar broke free, it rolled over and crushed left hand-index, middle, and ring fingers. This resulted in lacerations to all three fingers.

Employee was trying to free up a seized release pin on the skid steer mast so the bucket could be removed. The release pin broke free resulting in the skid steer bucket falling and striking top of employee's foot directly behind safety toe of work boot.
Employee was dragging roller bearing out of travel way when inner bearing and race turned and pinched left hand middle and ring finger. Employee received two stitches.
Positioning Jaw Crusher in, it sprang back and penned arm against a beam and fractured wrist.
Employee was lifting 1/2" metal shim into the jaw and felt a pain in their mid-back. EE then went ahead and put the shim into place and back pain continued. EE then notified the supervisor of the back injury.
Employee was shoveling material off the C-4 pad and caught a snag on the surface of the pad with the shovel while scooping. Thus, causing EE to twist a little and strain lower back.
Employee picked up a piece of flat metal off back of welding truck. When employee began to carry metal to the work area, they felt an intense pain in their abdominal region. After medical review, employee was diagnosed with an abdominal strain.
Employee was replacing a roller on conveyor. They lost grip of roller and it fell and struck their right hand.
Prior to tool use, employee was changing defective fitting on a railcar jack stand after pre-shift inspection. Metal sliver cut employees thumb requiring stitches by medical prof. Jack stands owned by contractor. Injury did not involve mine equipment or system. Tool training & inspection covered in Part 46, why fitting was being changed prior to use. All PPE worn.
EE was changing a bearing. The bearing housing slipped and smashed EE's left ring finger. EE was a temp and EE's assignment was ended on 6/8/2022 because EE didn't follow instructions on going to the medical facility to have the injury looked at.
EE was setting down a bucket with metal in and turned the wrong way and sprained back
Employee was putting in feeder bottoms and twisted then back started hurting. Employee didn't want to go to the doctor, so they waited till today 9/3/2020 for us to take them to the doctor.
EE was carrying a motor and when EE went to set it down it slipped. It caused the shaft to hit right behind the steel toe.
EE was moving the beam for the HP500 to set on. When EE reached for the beam EE's arm popped.

Was helping to pick up ne plates from the gradall straight across to the metal belt. When employee was putting them on the belt it slipped out of hand and fell on their other hand cutting the palm of their hand right under the fingers.
EE was pulling on a rubber screen.
EE was working on a screw that was stuck. EE was turning it backward to get it unstuck and that's when the v-belt smashed left index finger
EE was removing a guard on the counter weight of C4X when the guard pinched right hand middle finger.
Load of angle iron was being dropped in a new location when the load slipped and fell. One piece of angle iron bounced and hit EE in the lower leg. EE had a laceration with stitches.
Threw a piece of rip over car and it landed on my left index finger instead of falling off the side of rail car. Cleaning out rail cars
He was holding the ladder by a rope out of the way so the rock breaker could get in the jaw to hammer down the side plates. While he was holding the ladder by the rope and guiding the operator the rock breaker hit the ladder and smashing the tip of his middle finger. He was behind a guard rail
