Employee was working on the Jaw Feeder and was removing the Jaw Grizzly Bars. EE was helping to remove the old Grizzly Bars. One of the bars hung up while EE was trying to move it. When the bar broke free, it rolled over and crushed left hand-index, middle, and ring fingers. This resulted in lacerations to all three fingers.
Employee was lifting 1/2"" metal shim into the jaw and felt a pain in their mid-back. EE then went ahead and put the shim into place and back pain continued. EE then notified the supervisor of the back injury.
Employee was holding bolt head to enable tightening of lock nut. When the impact wrench was activated to tighten the nut, it caused the bolt to spin and crush employee's right middle finger between bolt head and crusher housing.
IE was holding the t-handle punch while co-worker was hitting it with a sledge hammer. Co-worker missed and the hammer hit the side of the crusher causing the hammer to ricochet hitting IE's hand.
Employee was using a pry bar to dislodge a rock in the tail pulley of a conveyor. After the rock was dislodged and employee was sliding out the pry bar, their hand struck a rock and resulted in laceration of their finger.
EE was cutting a tac weld with grinder that had a cutting wheel. The grinder kicked back and cut EE's finger. EE was wearing gloves
IE was helping with changing v-belts. The belts were put back on but 1 belt was twisted, so another EE was using a pipe wrench on the shaft and it slipped off causing the belt to roll back pinching IE's thumb.
He was holding the ladder by a rope out of the way so the rock breaker could get in the jaw to hammer down the side plates. While he was holding the ladder by the rope and guiding the operator the rock breaker hit the ladder and smashing the tip of his middle finger. He was behind a guard rail
After exiting the excavator he started walking he slipped so to catch himself he put his hand out and a rock cut his hand.
He was adjusting v-belts on the main feeder with a wrench when the wrench came loose it smashed his finger between the I-beam and wrench.
He had been standing all day running the plant when he turned wrong & hurt his back. He went to the Dr. on 6/10/13 the Dr. said it was a strain and employee went back to work. He was still having some trouble 7/11/13 they scheduled an MRI. He seen surgeon on 7/22/13 & decided he didn't want surgery. He then went back on 8/8/13 & he decided to have the surgery on 8/28/13.
He was grinding inside the main frame liners in the simmon crusher when a piece of metal got in his right eye. He was wearing all of his PPE, safety glasses, and face shield. He also washed his eye out several times thinking that he had gotten. On Friday March 1st is when he when to the doctor.
He was cutting a piece of flashing when the blade came out of the flashing cutting his thumb.
The weld broke loose on piece of metal and the porta-power came up and hit him in the nose.
Was lifting gear on the side of feeder, putting on cover of the gear on the other side, and standing on unlevel ground. Accident was on 7/29/09 he didn't ask to go to the doctor till 8/13/09.
Changing roller bracket got his hand caught in the bracket when it fell off the conveyor. The accident was on 4-9-08 but he didn't go to the doctor till 4-14-08
Bobcat was doing clean up at the hopper when haul truck backing up to dump didn't see the bobcat & bumped the back of bobcat with tire. Employee's neck was hurting.
A piece of metal popped loose and hit what he was swinging (sq. tubing) and hit his leg cutting his leg open.
MOUNTING A SAFETY COVER OVER BREAKER & IT GROUNDED OUT & BLEW UP.
SANDING TRUCK AND PIECE OF METAL GOT IN EYE.

PUTTING BELT BACK ON & BREAKING OFF BOLTS - SHAKING BOLT BREAKER CAUSE BOLT WOULDN'T COME OUT. BOLT CAME OUT & PIECE HIT HIM IN LEFT EYE.
THE HYDRAULIC HOSE CRIMPING MACHINE MALFUNCIONED, CAUSING IT TO BREAK & A PIECE HIT HIM IN THE HEAD.
DIAGNOSED WITH ACUTE SILICOSIS.
WELDING PLATES IN END DUMP BED, CHIPPING WELDS AND PIECE FLEW UNDER HOOD AND GOT IN EYE.
REPAIRING JAW OF CRUSHER, PUTTING TOGGLE BACK INJAW THE SEAT OF THE TOGGLE PLATE FELL OUT AND FELL ON HAND CRUSHING IT.
GRINDING METAL AND PIECE OF METAL FLEW IN EYE.
