When the employee got injured, he was sanding a shaft to a tail pulley of a conveyor belt. The employee had the shaft proped up about two inches off the floor to sand the shaft. The shaft slipped off the prop and crushed the employee's left middle finger. Employee should have been using a different means to prop the shaft up when sanding.

Employee was lifting crusher parts. This job is done by more than one person.

WAS CLEANING DIRT FROM UNDER MOVING TAIL-PULLEY WHEN BELT SNAGGED THE HANDLE OF SCRAPER AND MOVING HANDLE STRUCK HIM ACROSS HIS FACE AND HEAD.

EE WAS TRYING TO UNJAM PORTABLE CRUSHER, STRAINED HIS BACK (HAS A HISTORY OF BACK PROBLEMS AT FORMER EMPLOYER). DID NOT SEEK TREATMENT UNTIL 3/23 - REPORTED PROBLEM HAS WORSENED ON 3-24-2000.
