Employee was using a utility knife with right hand to strip wire before making terminations. The knife slipped and lacerated the top of left hand.

The employee drove the empty truck down a ramp leading to the pit floor and upon almost reaching the bottom of the inclined portion of the haul road, the employee lost control of the truck and the truck veered to the right and traveled on the top of a catch berm built on the right side of the roadway adjacent to the highwall.

Contract Welder ee was walking in the area adjacent to the primary jaw crusher and tripped on concrete from ""clean out"" of previous concrete pour.

Struck by flying object

Over the road CDL truck driver irritated his eye while cleaning his truck at the scale house. Employee was using a cleaning chemical called Big Orange to clean his truck and while rinsing the big orange off, it splashed into his right eye.

Struck against stationary object

Employee stepped on a rubber conveyor belt lying on the ground which had a rock underneath it. As a result, employee sprained his left ankle.

Employee is an over the road delivery driver and slipped while walking across scale to get into his truck. The left knee had torn ligaments and required surgery.

Employee was burning bolts off machinery with a torch. A washer went down his right boot and burned his ankle. About 2 weeks later the employee went to the hospital for treatment. EMPLOYER WAS UNAWARE OF INJURY UNTIL INVOICE WAS RECEIVED AND EMPLOYEE WAS QUESTIONED ON 6-20-11.

Fall from ladders

Going up step ladder with cutting torch. Was on the 6th step of a 12 ft step ladder when ladder twisted & went to the side and fell down to the ground. I fell down and hit the concrete floor and hit on my right shoulder & bruised the muscle in my right arm. Had checked ladder stability each time used.

Employee was operating Euclid (Off Road) dump truck (#H-57) and claims noise from left front tire ""Blow-out"" injured his hearing in both ears. Was wearing hearing protection at the time of the incident.

Was using torch to burn head off bolt on 6x20 scrubber when hot metal residue flew back into his ear canal. ee failed to report injury until 1/15/07 at which time he was sent to medical provider.

ACCORDING TO PARAMEDICS ON SCENE, HE PROBABLY EXPERIENCED A MASSIVE CORONARY AND WAS DOWN AN ESTIMATED 15 TO 30 MINUTES BEFORE BEING FOUND. CPR WAS STARTED AND PARAMEDICS WERE DISPATCHED TO TH E SCENE. HE COULD NOT BE REVIVED.

EMPLOYEE BACKED HAUL TRUCK TO THE TOP OF THE 3A STOCK PILE. WHEN THE TRUCK STOPPED AT TOP OF PILE, THE REAR OF THE PILE COLLAPSED CAUSING THE TRUCK TO TUMBLE BACKWARDS DOWN THE PILE. THE TRUCK ROLLED SIDEWAYS AND CAME TO REST ON ROOF.
