Employee slipped while walking down steps. Employee fractured right ankle.

The employee was removeing a piece of steel from the magnet area in the plant and injured a hand while doing so.

A piece of metal flew off of a generator and hit employee in right leg

Employee had a seizure and injured leg as a result of the seizure.

EE was walking and tripped, EE fell on EE's face and broke EE's nose and rt. cheek bone.

Employee was crawling through railing and banged left knee.

Employee was using a utility knife with right hand to strip wire before making terminations. The knife slipped and lacerated the top of left hand.

The employee drove the empty truck down a ramp leading to the pit floor and upon almost reaching the bottom of the inclined portion of the haul road, the employee lost control of the truck and the truck veered to the right and traveled on the top of a catch berm built on the right side of the roadway adjacent to the highwall.

The employee was exiting a front end loader. As employee was getting down from the ladder left foot stepped on a rock and employee sprung ankle.

Employee suffered a heart attack before starting work.

Employee tripped on steel stacked in shop area. NOTE: Employee did not seek treatment (to which restrictions were placed) until 05/11/2016. This report was filed within the 10 working day requirement.

Contract Welder ee was walking in the area adjacent to the primary jaw crusher and tripped on concrete from ""clean out"" of previous concrete pour.

Over the road CDL truck driver irritated his eye while cleaning his truck at the scale house. Employee was using a cleaning chemical called Big Orange to clean his truck and while rinsing the big orange off, it splashed into his right eye.

Employee stepped on a rubber conveyor belt lying on the ground which had a rock underneath it. As a result, employee sprained his left ankle.

Employee claims to have injured back while lifting rollers. Employee has had past back problems.

Employee is an over the road delivery driver and slipped while walking across scale to get into his truck. The left knee had torn ligaments and required surgery.

Employee was removing blocking cribs and strained lower back. Employee reported injury on 11/05/2013.

Employee was burning bolts off machinery with a torch. A washer went down his right boot and burned his ankle. About 2 weeks later the employee went to the hospital for treatment. EMPLOYER WAS UNAWARE OF INJURY UNTIL INVOICE WAS RECEIVED AND EMPLOYEE WAS QUESTIONED ON 6-20-11.

Back strain while trying to lift a blower onto a frame.

Going up step ladder with cutting torch. Was on the 6th step of a 12 ft step ladder when ladder twisted & went to the side and fell down to the ground. I fell down and hit the concrete floor and hit on my right shoulder & bruised the muscle in my right arm. Had checked ladder stability each time used.

Employee was lifting oxygen bottles, and shoveling clay/soil. Strained / sprained lower lumbar portion of back.

Employee slipped on fuel tank and fell to ground. The tank was wet, with fuel residue and the tank was round.

Employee was operating Euclid (Off Road) dump truck (#H-57) and claims noise from left front tire ""Blow-out"" injured his hearing in both ears. Was wearing hearing protection at the time of the incident.

Was using torch to burn head off bolt on 6x20 scrubber when hot metal residue flew back into his ear canal. ee failed to report injury until 1/15/07 at which time he was sent to medical provider.

Strained elbow using repetitive motion in pulling cord to release dump mechanism on truck bed.

Lifted and carried bucket of grease up steps; did not use lifting device supplied for this purpose. Per company call on 5-24-05, this was not a hernia, it was an inflammation of the prostate. coder 12.

ACCORDING TO PARAMEDICS ON SCENE, HE PROBABLY EXPERIENCED A MASSIVE CORONARY AND WAS DOWN AN ESTIMATED 15 TO 30 MINUTES BEFORE BEING FOUND. CPR WAS STARTED AND PARAMEDICS WERE DISPATCHED TO TH E SCENE. HE COULD NOT BE REVIVED.

DISMOUNTING FROM TRUCK CAB & FOOT SLIPPED ON BOTTOM STEP CAUSING HIM TO TWIST HIS BODY.

LOST HAND GRIP ON HANDHOLD BAR WHILE ENTERING WATER TRUCK AND FELL.

LIFTING A JACKSHAFT INTO BUCKET OF A BACKHOE & TWISTED BODY CAUSING BACK STRAIN.

WELDER USED SLEDGE HAMMER TO POUND PIPE WRENCH HANDLE HAMMER BOUNCED OFF WRENCH HANDLE AND STRUCK THIS EE ON HEAD.

EMPLOYEE BACKED HAUL TRUCK TO THE TOP OF THE 3A STOCK PILE. WHEN THE TRUCK STOPPED AT TOP OF PILE, THE REAR OF THE PILE COLLAPSED CAUSING THE TRUCK TO TUMBLE BACKWARDS DOWN THE PILE. THE TRUCK ROLLED SIDEWAYS AND CAME TO REST ON ROOF.

CLIMBING OUT OF HOPPER BIN AND TWISTED KNEE.

EMPLOYEE WAS PULLING A BELT ON A NEW CONVEYER WHEN THE RIGGING FAILED. THE EMPLOYEE WAS STRUCK IN THE LEG WITH A PIECE OF FLAT STOCK AT THE END OF 1/4"" CABLE PULLING THE BELT.

WAS WALKING ON STOPPED CONVEYER BELT AND SLIPPED ON SNOW ON BELT- HIT SHIN ON BELT CAUSING ABRASION, FAILED TO REPORT IT AND GET MEDICAL TREATMENT UNTIL INFECTION BEGAN, WAS SENT TO MEDICAL PR OVIDER TODAY.

WAS WALKING ACROSS FLAT GROUND AREA; STEPPED ON SIDE OF FOOT, TWISTING ANKLE.

WAS CLEANING DIRT FROM UNDER MOVING TAIL-PULLEY WHEN BELT SNAGGED THE HANDLE OF SCRAPER AND MOVING HANDLE STRUCK HIM ACROSS HIS FACE AND HEAD.

EE WAS TRYING TO UNJAM PORTABLE CRUSHER, STRAINED HIS BACK (HAS A HISTORY OF BACK PROBLEMS AT FORMER EMPLOYER). DID NOT SEEK TREATMENT UNTIL 3/23 - REPORTED PROBLEM HAS WORSENED ON 3-24-2000.
