While assisting in carrying a wire cable, both employees slipped and fell due to mud conditions. Employee's wrist was cut by the cable by employee falling. Went to Princeton Community Hospital for attention. Treated and released after application of adhesive in lieu of stitches.
Plant operator was cleaning out around overloaded screen unit. While doing the clean out a rock fell on their finger. Plant supt. transported to Bluefield Regional Medical, treated and released. No stitches or sutures.
Employee was handing a box of 1 1/2 inch x 10 inch bolts out the driver side window of a pickup when the box slipped out of his left hand, he tried to grab hold with his right hand and felt a pull in groin area.
While descending steps from operators room at High Cal plant, employee stopped five steps from stairs end to pull a 1 1/2 inch water hose used for cleaning and felt a pain in his right bicep. Sent to hospital for evaluation.
Maintenance work on plant individual was being handed a tool (3 ton come-a-long) by another employee. Finger was caught between the tool and plant frame causing a cut that required 3 stitches.
Injury not reported until 06/11/2013 at 11:15AM Highway Truck driver possible sprain to stomach area while opening hood on truck. Supervisor was present at the time, no indication given as to supervisor by the driver of any injury at that time. Injury reported as noted.
Employee was removing front idler and cannon on Cat D8R dozer. There was a 12"-15" rock wedged between roller frame and main frame. Employee was reaching over push-arm and roller frame to removed rock. Employee felt pain in lower - left area of his back.
Highway delivery truck driver-Removing stone buildup on tailgate apron-Schotched the tailgate to keep the tailgate open in order to clean the area-Wheel choked slipped allowing tailgate to close under its own weight striking the drivers hand.

EE helping maintenance man hook chain up to lifting hook to break loose wedged rock in feeder a loose rock about size of football rolled down about 5' and struck him on right leg. The leg does have a cut about 11/2" long. Supervisor sent man to emergency room.

SHOVELING PACKED MATERIAL FROM UNDER TUNNEL CONV. BELT USING A LONG-HANDLED ROUND-POINT SHOVEL. MATERIAL PACKED VERY TIGHTLY AND EE HAD TO USE THRUSTING MOTION & GREAT EFFORT TO DISLODGE, IN ORDER TO SHOVEL & CLEAN. DURING THRUSTING MOTION, THE PACKED MATERIAL BROKE LOOSE & CAUSED EE'S RIGHT WRIST AREA TO STRIKE THE CONVEYOR STRUCTURE. MINE SUPT. FIRST INFORMED OF ACC. 5/21.
