Contact with hot objects or substances

Employee was welding in the box chute off #1 screen when a piece of hot slag fell on EE's pant leg and caught pants on fire causing a burn to lower leg.

Fall to the walkway or working surface

Employee was walking down a slope behind our electrical building when ee stepped on wet leaves and mud, ee slipped and fell landing on left elbow shoulder area causing chronic irregularity of the clavicle from old injury ee had before EE came to work for us.

Over-exertion in pulling or pushing objects

Employee was performing maintenance on #2 screen. Using a pry bar on counter weight when bar slipped. Felt the right shoulder pop. Employee is the superintendent for the welding team. At the time of the accident they had no lost time or restrictions. Later MRI was done and found a torn rotator cuff. Surgery on November 28, 2018.

Employee suffering from symptoms of diabetic trauma, contacted EMS for transport to Bluefield Regional Medical Center. Reported later that employee has infection in employee's foot from a prior amputation of toe.

Struck by falling object

Plant operator was cleaning out around overloaded screen unit. While doing the clean out a rock fell on their finger. Plant supt. transported to Bluefield Regional Medical, treated and released. No stitches or sutures.

Struck against a moving object

Employee was operating end loader on quarry pit, hydraulic hose ruptured and bucket dropped to pit floor. Operator states was ""jarred'. Operator requested hospital check up and went to Bluefield Regional Medical Center. On workers compensation at this time.

Individual is scale clerk, operating truck scales, observed by highway truck drivers of being incoherent and unresponsive. Emergency Response contacted and individual transported to Bluefield Regional Hospital, ER started treatment for Stroke. Later same day he was air lifted to Roanoke, found individual had stroke, heart attack, high blood pressure, diabetic and a hernia.

WALKED DOWN GABION BELT CATWALK TO UNSTOP GABION CHUTE. PICKED UP 8-LB. SLEDGE HAMMER WITH RIGHT HAND. ATTEMPTED TO STEP FORWARD AND DOWN TO METAL BOX LOCATED AT BOTTOM OF GABION CHUTE AND ONTO THE GABION BOX LEDGE WHICH IS APPROX. 12 INCHES IN WIDTH. STEPPED DOWN APPROX. 8-10 INCHES ONTO THE BOX LEDGE - LEFT FOOT CURLED AND TWISTED ANKLE.

SHOVELING PACKED MATERIAL FROM UNDER TUNNEL CONV. BELT USING A LONG-HANDLED ROUND-POINT SHOVEL. MATERIAL PACKED VERY TIGHTLY AND EE HAD TO USE THRUSTING MOTION & GREAT EFFORT TO DISLODGE, IN ORDER TO SHOVEL & CLEAN. DURING THRUSTING MOTION, THE PACKED MATERIAL BROKE LOOSE & CAUSED EE'S RIGHT WRIST AREA TO STRIKE THE CONVEYOR STRUCTURE. MINE SUPT. FIRST INFORMED OF ACC. 5/21.

ATTEMPTING TO REMOVE 3"" PIPE COUPLING FROM FRONT OF WATER TANK MOUNTED ON PETERBILT TRI-AXLE WATER DISTRIBUTION TRUCK. STANDING BETWEEN TRUCK CAB & MOUNTED TANK WITH LEFT FOOT ON TRUCK FRAME & RIGHT FOOT PRESSING ON THE HANDLE OF A 36"" PIPE WRENCH, JAWS SLIPPED CAUSING LOSS OF BALANCE & FALLING, BUTTOCKS/ PELVIS HITTING TRUCK FRAME FIRST THEN LANDING IN SITTING POSITION ON GROUND.
