Employee used other means to access plant other than ladder. Still under investigation. Employee states EE twisted EE's lower right hand back. Employee did not report until the following day. No witnesses.

Showing all 10IW and the supervisor were changing V-belts on the cyclone pump. The equipment was locked out and tagged out while working on it. IW looked away and caught the left index finger in/between the v-belt and pulley wheel causing a laceration.
Miner was bending a tab on a lid of a grease drum. The metal tab was sharper than the miner anticipated. Pulling and curling the tab between forefinger and thumb the miner cut thumb
Tearing down crusher - getting ready to move to new location - moving wood blocks from bunker and stacking them - while moving a block lost control and dropped it on right foot

Working to put cone back in crusher - smashed thumb.
Helping to loosen hydraulic fitting - smashed finger when hose came down.
