Worker was getting rocks out of tail pulley using an air hose. The tip of the air wand got clipped by metal gear which forcefully hit the wand and the top of worker's hand into the equipment above.

IW was shoveling from knees when they noted that their knee hurt, no abnormal motion or action was involved. There were no known materials or objects that caused the injury.

Injured Worker (IW) jumped from the low end of a conveyor, landing on uneven ground and rocks. Per IW, the injury was ""self-inflicted."" The conveyor was approximately five feet high at the location IW jumped from. The injury resulted in a left talus fracture.

The Injured Worker (IW) was stepping out of the cone screen box and slipped on the lower end of the ""to waste"" conveyor frame. IW landed on feet but felt a pop in right knee and could not bear weight on it.

A rock came off of the feeder area and hit the person behind the ear right under EE's hard hat. Person reported to foreman at the time but did not request to be seen by a doctor

Miner was bending a tab on a lid of a grease drum. The metal tab was sharper than the miner anticipated. Pulling and curling the tab between forefinger and thumb the miner cut thumb

The Miner was removing a V belt guard on a JCI screening plant. Employee's foot slipped from where employee was standing, when employee's foot slipped employee fell forward into the side of the screen box resulting in a small cut to employee's lower lip area and a broken tooth.

Walking back to sample trailer stepped on a large rock and twisted ankle.

Employee was getting down from working on equipment, slipped off ladder and landed on ground.

While working in appropriate maintenance area, employee was struck in the lower back by a rock unexpectedly rolling out of crusher jaw.

Tearing down crusher - getting ready to move to new location - moving wood blocks from bunker and stacking them - while moving a block lost control and dropped it on right foot

Removing power disconnect on crusher - broke free and caught finger

Setting up bunker attaching legs EE was trying to bolt in to secure. Bunker was lower smashing his thumb.

Pinched finger between a bolt and gear box.

Working to put cone back in crusher - smashed thumb.

Helping to loosen hydraulic fitting - smashed finger when hose came down.

Cleaning under jaw crusher. When loader dumped load, rock fell over side - striking shoulder and head - causing bruising.

Slipped while climbing down ladder of catwagon.

CHANGING BELTS AND CLEANING AROUND END PULLY. EE PUT ARM BETWEEN BELT WHEN CONVEYOR WAS STARTED.

WORKING ON CRUSHER UNIT, FELL BACKWARDS LANDING ON WRIST. BROKE WRIST.

EMPLOYEE WALKING INTO TOOL SHED WHEN 2 ROCKS HIT HIM ON SHOULDER.
