Removing anvil on rope from screen deck when strained shoulder muscle.
AB plant fixing the Universal crusher. Turning the blow bar by hand. The only way to move it with the power locked out is to put your weight into. While EE was turning it, EE's glove got caught on the blow bar. This caused EE's left hand to be caught between the blow bar and the wall of the universal crusher causing contusion.
Employee had just completed a walk around inspection of employee's CAT775 Rock Truck 26-1009, as employee was climbing back in felt sharp pain in groin area
As loader was getting out of cab the loader crushing plant. EE struck knee on door latch, EE's left knee received a hematoma.
Removing flashing from talley pulley section of the A/C side conveyor belt. Employee did not completely remove retainer & then jerked on the used flashing straining right shoulder tendon
Working on loader and employee felt a slight strain in back at the time. The next day the employee reported that his back was feeling sore. Employee was placed on modified duty starting 6-25-15.
doing a welding repair under an excavator in an awkward position
Employee set lunch box down to go load a skid steer loader on an equipment transport trailer. As the employee set his lunch box on the ground he heard two (2) popping sounds and felt extreme pain in right shoulder.
Employee was replacing screen cloth sections on the second deck of the Telesmith screen at the AB Plant when he felt pain in his right shoulder. The employee was taken to a local health care facility and returned to work on restricted duty.
3 ee where installing conveyor belting over the top of the modules on the JCI screen to protect them from the elements. 2 ee put a piece of pipe through a roll of belting to use as a handle. When picking up the belting and starting to walk around a corner ee felt a strain in his left knee.
Employee was assisting with the installation of a new rubber skirt on the oversize chute at the sand plant. When he reached for and pulled on the bottom right corner of the rubber piece he felt something pop in his right arm.
Employee was splitting a sample (rock). Employee was taking sample pan out of splitter that was full of material and strained his back.
The EE was removing the dust cover over the threaded port hole on a grease barrel and cut his right finger on the metal pull tab. The laceration to the tip of his pinky finger required two stitches.
The EE accessed the front end loader Co. #71-485 via the ladder, located on the left hand side, to check fluids. While descending from the right side of the cat 988F loader at 05:45 am, the operators visibility was limited and the EE misjudged the distance from the last rung to the ground, hyperextending his left knee.
Employee was cleaning and rearranging screen storage room when a yellow jacket bee was encountered. He tried to swat the bee and accidentally hit a wooden shelf with his left hand and jammed his long finger. Injury was diagnosed as left long finger avulsion fracture on 5/7/08 and employee was placed on modified duty.
He was working on brake assembly for a motor grader. Assembly had been removed from grader for rebuild. While on the bench, he tested the piston seals & retainers blew out & struck him on lip & under chin causing laceration.
The ee was trying to make an adjustment to the table. The handle used to move the table saddle swung around striking the ee (r) tip of index finger-(r) hand. causing the nail to partially come loose.
EE was installing a new cutting edge on the motor grader. He and a co-worker had the cutting edge suspended with a bar on each end, the edge slipped off EE's bar while changing hand positions and landed on his foot. The mow board was blocked.
Employee was mixing cement mortor to use for the new installation of the outbound truck seale for the napa quarry. a square end margin trowel was being used as a mixing tool. While mixing the mortor with his left hand the employee felt pain in his left wrist that radiated up to his elbow. the injury was diagnosed a left wrist strain on 7/14/06 and ee was placed a modified duty
EE was completing repairs to a Lippmann swing jaw shaft assembly. Task at hand was to plug the threaded puller holes in the shaft bearing housing. When rotating housing to locate the holes his left 5th fingertip was caught between the housing & counterweight clamp bolts. Injury was diagnosed as a left 5th finger crush injury on 5/31/06 & he was placed on modified duty.
Picking up cages and stacking onto truck trailer. Bent over to pick up another cage and felt pain in back.
EE was placing a section of drill steel on his drill while performing his normal job duty. While placing the steel into position he felt a sudden pain and burning in his right groin area. Injry was diagnosed as right groin strain on 2/23/06 and EE was placed on modified duty.
While employee was shoveling under the screen deck at the scalper he felt something tear near his left ribs.
EMPLOYEE JARRED HIS NECK WHEN HE LIFTED UP WHILE PUTTING SHOES INSIDE CIRCLE OF GRADER, RESULTING IN A CERVICAL STRAIN.
Installing air tank under equipment, he went to get under equipment and hit his hard hat on a bar jamming his neck.
EE WAS PUTTING FILTERS INTO BOX ON LUBE TRUCK THAT HAS A HINGED LID. THE LID FELL ONTO EE'S LEFT THUMB - CONTUSION.
EMPLOYEE WAS PAINTING INSIDE OF BLENDING SIDE OF LAB ON & OFF ALL WEEK FROM 1/18/05 TO 1/21/05. ON FRIDAY AT 1:30PM HE SAID HE BECAME DIZZY & DID NOT FEEL WELL.
MOVING BOX, FELT PAIN IN SHOULDER
While working on truck suspension employee began to feel pain in right knee and a burning sensation. Employee rested that night. The following day the pain was worse.
Ee was cleaning plant area with a water hose. He was pulling hose to move it out of the walkway when hose came apart at a splice and struck ee in the mouth, chipping a front tooth.
HOOKING UP BRAKE LINES TO THE BRAKE VALVE LAYING ON PLATFORM WITH SHOULDER & ARMS DOWN INTO MACHINE PULLING ON BRAKE LINE, FELT PAIN IN LOWER BACK.
WORKING ON A CONVEYOR UNDER JAW CRUSHER, TWISTED AND STOOD UP.
PLACING PIPE ALONG BERM, TURNED WRONG WITH PIPE IN HAND AND STRAINED BACK.
PULLING BULK POWDER 2" DIAMETER HOSE FROM HOLE TO HOLE OVER THE SHOT AREA. PAIN WAS FELT IN LOWER BACK.
WORKER WAS ON BLENDING PLANT OPERATION. HE WAS OPENING A GATE AND ENDED UP WITH A SMASHED AND CUT FINGER.
BLUE ROCK JAW PLUGGED SHUT HYDRAULIC FEED OFF, LOOKED DOWN OVERSIDE OF RAILING, WENT DOWN TO NEXT LEVEL OPENED DOOR, ROCK FELL AND HIT HAND.
LOADER OPERATOR. HE THINKS GETTING OFF THE MACHINE HE PULLED A MUSCLE IN LOWER BACK RIGHT SIDE.
LAYING ON SIDE INSPECTING BRAKES ON THE REAR OF THE LOADER, STRAINED NECK WHILE HOLDING HEAD UP.
HE WAS WAITING TO INSERT ROPE WEDGE THAT HOLDS TRIP LINE ON. WHEN HE WENT TO INSERT WEDGE THE WIRE ROPE SLIPPED OUT AND HIT HIM IN THE FACE.
STRAINED BACK WHILE SHOVELING 1" MATERIAL IN SHORT GRAVEL TUNNEL
LOADING OXYGEN CYLINDER ONTO SERVICE TRUCK, CYLINDER SLIPPED OFF TRUCK AND LANDED ON LEFT FOOT.
PICKING MUD & STICKS OFF CONVEYOR. PICKED UP ONE PIECE OF MUD & THREW IT ON THE TRASH BELT. FELT SOMETHING PINCH IN THE BASE OF NECK BETWEEN SHOULDER BLADE AREA, FINGERS STARTED TINGELING.
WHILE EE WAS SHOVELING ROCK OFF BELT THAT HAD STALLED HE EXPERIENCED PAIN IN HIS NECK AND SHOULDER. NOTE: EE WAS NOT PUT ON MODIFIED DUTY UNTIL 1-28-03.
TWO EMPLOYEES WERE IN THE PROCESS OF HANGING A SIGN, WHICH WAS SUSPENDED ON THE FORK OF A FORKLIFT, AT THE SCALES. WHILE ONE OF THE EMPLOYEES WAS POSITIONING THE SIGN, IT SLIPPED OFF THE FORKL IFT STRIKING THE EE ON TH LEFT ARM FRACTURING HIS ELBOW IN THREE PLACES.
EE PERFORMS CONCRETE WORK AND CARPENTRY WORK. DURING PERFORMING THESE ACTIVITIES HE NOTICED THAT HIS NECK AND SHOULDERS WILL START TO HURT.
WENT TO CLOSE DOOE RON EXCAVATOR WHILE HOLDING ON TO RAIL SLAMMED DOOR WITH R HAND CATCHING L HAND, RING FINGER IN DOOR.
MOVING BOXES UP AND DOWN STAIRS NOTICED RIGHT KNEE STARTED TO HURT.
CLEANING UP AND PERFORMING YARD WORK. MOVING HEAVY MATERIAL. DID NOT NOTICE PAIN ON FRIDAY WOKE UP SATURDAY WITH A SHIFT BACK.
