EE was backing up on stockpile to dump loaed 775D rock truck on oneway stockpile. Rock & truck backed over end of stockpile. Truck flipped over backwards and landed upside down about 30' below. No damage to cab, employee suffered fatal blunt force trauma.
During a routine maintenance procedure on an air conditioning unit, site mechanic was injured when EE's left leg became entrapped by the equipment's bucket. Employee sustained a left ankle and foot fracture.
Initially treated as a first-aid injury on 4/9/24 operator attempted to walk down a steep undeveloped slope, when EE lost EE's footing falling to the ground. On 4/11/24 employee requested medical evaluation where EE was diagnosed with an abrasion to the left forearm and contusion of unspecified front wall of thorax.
On 01/19/2023 employee experienced a medical emergency while operating a loader. Employee was transported via ambulance to the hospital and was subsequently taken off work.
On Monday, November 22, 2021 rock truck model 775G caught fire due to a hydraulic line failure. The truck burned beyond repair.
Employee left work at lunch time, stated that wheel loader had made back hurt.
Employee was using a cordless drill. EE was drilling and when EE got to the end of the hole it hung up and twisted EE's wrist causing a sprained right wrist.
Employee was washing out hopper of street sweeper. EE was wearing safety glasses but, EE had muddy water splash into right eye. EE had to go to the emergency room where they flushed eye out and provided a antibiotic eye drop.
Employee loaded parts into a truck. EE was putting on gloves and dropped one on the floor. EE bent over picked up the glove and felt pain in their back. EE told supervisor, finished shift. The next morning called in and said was not able to report to work. We sent them to the doctor on 5-15-2019 and the doctor has placed them on restricted duty.
Employee had just completed a walk around inspection of employee's CAT775 Rock Truck 26-1009, as employee was climbing back in felt sharp pain in groin area
Employee was pulling on tire chain. Employee was using a tee bar with a hook on the end to pull the chain in place. The employer was pulling hard and the hook slipped of the chain causing the employee to fall on back
Employee reported 9/29/16 that hands were going numb when working with tools. Employee saw Doctor and was given physical therapy. Employee was never placed on modified duty or lost time until surgery on 4/10/17
Removing flashing from talley pulley section of the A/C side conveyor belt. Employee did not completely remove retainer & then jerked on the used flashing straining right shoulder tendon
While swinging the 103 hoist crane around with a load on the hoist, EE was pulling from a rope and co-worker was assisting to swing the crane by pulling from the hoist chain. The rope broke. EE fell backwards onto the Nordberg crusher. Adjustment ran. EE fractured left side ribs.
Parking area had material added to it in the a.m. raising the truck level so that final step to ground was about 3'.
The lab Tech was walking into the lab and tripped on rubber floor mat. We repositioned the mat away from the entrance to reduce the trip hazard. EE fell onto concrete floor straining or spraining left wrist. He was placed on modified duty.
EE was replacing the rear spray bar. On water truck 21-1003, after cutting old spray bar off. He was grinding to prepare for new spray bar. He was wearing safety glasses and face shield when foreign object got in right eye.
Employee was splitting a sample (rock). Employee was taking sample pan out of splitter that was full of material and strained his back.
EE was walking on discharge chute. As he was climbing out his foot slipped causing lower back pain when he landed. The screen discharge chutes were on the ground while maintenance was being performed.
While employee was operating his loader he turned to grab his lunch box and felt a pain in his lower back.
EE was inspecting plant for anything in need of repair. He was stepping from 1 screen deck to another when his right foot landed on a rock causing his ankle to twist inward.
EE was installing a new cutting edge on the motor grader. He and a co-worker had the cutting edge suspended with a bar on each end, the edge slipped off EE's bar while changing hand positions and landed on his foot. The mow board was blocked.
Employee was moving a full wheelbarrow when the handle broke and employee's body twisted as wheelbarrow tipped over causing a back strain.
While employee was shoveling under the screen deck at the scalper he felt something tear near his left ribs.
While driving a stake into a wattle, employee's hand & stake moved causing sledge hammer to hit his left hand.
WHILE EMPLOYEE WAS EXITING THE MACHINE HIS LEFT FOOT SLIPPED OFF THE STEP. HIS LEFT LEG WENT BETWEEN THE DRILL & MACHINE. HIS MOMENTUM CAUSED HIS BODY WEIGHT TO SHIFT TO THE LEFT CAUSING PAIN IN HIS KNEE.
While working on truck suspension employee began to feel pain in right knee and a burning sensation. Employee rested that night. The following day the pain was worse.
EMPLOYEE WAS USING A HAND HELD GRINDER ON A SPLICE WHEN THE GRINDER KICKED OUT CAUSING LACERATION ON LEFT INDEX FINGER. EMPLOYEE HAD REMOVED GUARD FROM THE GRINDER PRIOR TO STARTING THE JOB.
EE WAS TRYING TO STRAIGHTEN A RADIATOR HOOD OFF OF A 988F. WHILE HE WAS HITTING A PIECE OFF THE HOOD WITH A HAMMER, IT CAME LOOSE STRIKING HIM ON THE RIGHT INDEX FINGER.
WHILE CLIMBING BACK ONTO THE DOZER EE GRABBED AT A HANDLE AND MISSED. HE TRIED TO JUMP TO AVOID LANDING ON HIS BACK AND LANDED ON HIS LEFT SIDE INSTEAD.
OVER A PERIOD OF TIME WHILE EMPLOYEE HAS BEEN OPERATING A 992C LOADER REPETITIVE MOTION HAS CAUSED PAIN IN HIS LEFT SHOULDER.
WHILE SERVICING A EXCAVATOR, EMPLOYEE REACHED FOR THE RADIATOR CAP TO CHECK COOLANT, WAS STUNG BY A BEE ON THE PALM OF HIS RIGHT HAND.
WHILE EE WAS CHIPPING THE FEEDER PAN A LARGE ROCK FELL FROM THE EDGE AND STRUCK HIM IN THE BACK.
WHILE EE WAS SHOVELING ROCK OFF BELT THAT HAD STALLED HE EXPERIENCED PAIN IN HIS NECK AND SHOULDER. NOTE: EE WAS NOT PUT ON MODIFIED DUTY UNTIL 1-28-03.
WHILE DRIVING A WATER TRUCK EMPLOYEE NOTICED LOWER BACK PAIN. AFTER HIS FIFTH LOAD HE NOTIFIED THE PLANT MANAGER THAT THE PAIN HAD WORSENED AFTER EACH LOAD. EMPLOYEE DID NOT NOTIFY PLANT MANAG ER UNTIL 10:30 AM.
TWO EMPLOYEES WERE IN THE PROCESS OF HANGING A SIGN, WHICH WAS SUSPENDED ON THE FORK OF A FORKLIFT, AT THE SCALES. WHILE ONE OF THE EMPLOYEES WAS POSITIONING THE SIGN, IT SLIPPED OFF THE FORKL IFT STRIKING THE EE ON TH LEFT ARM FRACTURING HIS ELBOW IN THREE PLACES.
WHILE OPERATING THE 988G LOADER EE AGGRAVATED AN EXISTING CONDITION OF CARPAL TUNNEL.
EMPLOYEE WAS CLEANING CHUTE WHEN HE LOST HIS BALANCE AND STARTED TO FALL, WHILE TRYING TO JUMP CLEAR OF CONVEYER BELT HE STRUCK HIS RIGHT FOOT ON THE RAIL CAUSING FOOT INJURY.
WHILE DRILLING ON THE DRILL PRESS A PIECE OF SHAVING WAS GATTING BIGGER AND BIGGER IN AN ATTEMPT TO PUSH IT OUT OF THE WAY IT CURLED AROUND MY FINGER RESULTING IN A SEVERE CUT.
WHILE CLEANING THE FRONT OF THE LUBE TRUCK, WAS STUNG BY A BEE ON THE INSIDE OF HIS RIGHT ARM.HE IS HIGHLY ALLERGIC AND TOOK HIMSELF TO CLINIC.
REPORTED TO WORK AT 0600 PERFORMING HIS PERMANENT MODIFIED WORK. HE WENT IN TO SEE THE DR. AND RECEIVED RESULTS FROM THE MRI. THE DR. TOLD HIM TO REMAIN OFF WORK UNTIL FURTHER NOTICE. (SCHEDUL E SURGERY).
EMPLOYEE WAS GRINDING ON A PIECE OF STEEL, WITH SAFETY GLASSES IN PLACE WHEN A PARTICLE SOMEHOW FOUND ITS WAY INTO HIS LEFT EYE. THE EMPLYEE WENT TO THE DOCTOR AND HAD THE PARTICLE REMOVED.
EMPLOYEE WAS SHOVELING UNDER FEED BELT AT CLASS II PERM PLANT WHEN HE EXPERIENCED PAIN IN LOWER BACK. WE SENT HIM TO THE DOCTOR.
EE REPORTED THAT HE HAS HAD HIP PAIN SINCE EARLY1998. HE WENT TO DR SEVERAL TIMES & ON 7/25/00 WAS TOLD INJURY WAS JOB RELATED. PRIOR TO 7/25/0-0 HE RAN A CAT992C LOADER & WHEN HE TOLD US ABOU T THE PAIN HE WAS REASSIGNED TO RUN THE PRIMARY CRUSHER. ON 11/17/00 HE WAS PUT ON MODIFIED WORKDUTY BUT THE CO WASN'T NOTIFIED UNTIL 2/16/01. HIS WORK HAD ALREADY BEEN MODIFIED TO HELP ALLEV
WHILE SERVICING ROCK TRUCK, EE SLIPPED AND FELL BETWEEN THE LEFT FRONT TIRE AND THE MUD FLAP FRAME, INJURING HIS LEFT KNEE DURING FALL.
EE WALKED UP STAIRS TO WATER TANK AND STEPPED ON SOME LOOSE ROCKS WHICH CAUSED HIM TO TWIST HIS KNEE AND FALL.
EMPLOYEE WAS HELPING DRILL CREW CHANGE DRILL STEEL. WHEN STEPPING DOWN FROM DRILL RIG, HE TWISTED HIS KNEE. HE STATED HE WAS WEARING HIS BIFOCALS LOOKING THROUGH THE LOWER PART AND THE GROUND WAS ACTUALLY FARTHER AWAY THAN HE REALIZED. CAUSED INJURY TO HIS LEFT KNEE.
