EE was backing up on stockpile to dump loaed 775D rock truck on oneway stockpile. Rock & truck backed over end of stockpile. Truck flipped over backwards and landed upside down about 30' below. No damage to cab, employee suffered fatal blunt force trauma.

During a routine maintenance procedure on an air conditioning unit, site mechanic was injured when EE's left leg became entrapped by the equipment's bucket. Employee sustained a left ankle and foot fracture.
Initially treated as a first-aid injury on 4/9/24 operator attempted to walk down a steep undeveloped slope, when EE lost EE's footing falling to the ground. On 4/11/24 employee requested medical evaluation where EE was diagnosed with an abrasion to the left forearm and contusion of unspecified front wall of thorax.

Employee was using a cordless drill. EE was drilling and when EE got to the end of the hole it hung up and twisted EE's wrist causing a sprained right wrist.

Employee was washing out hopper of street sweeper. EE was wearing safety glasses but, EE had muddy water splash into right eye. EE had to go to the emergency room where they flushed eye out and provided a antibiotic eye drop.
Employee loaded parts into a truck. EE was putting on gloves and dropped one on the floor. EE bent over picked up the glove and felt pain in their back. EE told supervisor, finished shift. The next morning called in and said was not able to report to work. We sent them to the doctor on 5-15-2019 and the doctor has placed them on restricted duty.

Employee was pulling on tire chain. Employee was using a tee bar with a hook on the end to pull the chain in place. The employer was pulling hard and the hook slipped of the chain causing the employee to fall on back

While swinging the 103 hoist crane around with a load on the hoist, EE was pulling from a rope and co-worker was assisting to swing the crane by pulling from the hoist chain. The rope broke. EE fell backwards onto the Nordberg crusher. Adjustment ran. EE fractured left side ribs.

The lab Tech was walking into the lab and tripped on rubber floor mat. We repositioned the mat away from the entrance to reduce the trip hazard. EE fell onto concrete floor straining or spraining left wrist. He was placed on modified duty.

EE was replacing the rear spray bar. On water truck 21-1003, after cutting old spray bar off. He was grinding to prepare for new spray bar. He was wearing safety glasses and face shield when foreign object got in right eye.
Employee was splitting a sample (rock). Employee was taking sample pan out of splitter that was full of material and strained his back.

EE was inspecting plant for anything in need of repair. He was stepping from 1 screen deck to another when his right foot landed on a rock causing his ankle to twist inward.
EE was installing a new cutting edge on the motor grader. He and a co-worker had the cutting edge suspended with a bar on each end, the edge slipped off EE's bar while changing hand positions and landed on his foot. The mow board was blocked.

TWO EMPLOYEES WERE IN THE PROCESS OF HANGING A SIGN, WHICH WAS SUSPENDED ON THE FORK OF A FORKLIFT, AT THE SCALES. WHILE ONE OF THE EMPLOYEES WAS POSITIONING THE SIGN, IT SLIPPED OFF THE FORKL IFT STRIKING THE EE ON TH LEFT ARM FRACTURING HIS ELBOW IN THREE PLACES.
WHILE OPERATING THE 988G LOADER EE AGGRAVATED AN EXISTING CONDITION OF CARPAL TUNNEL.
EMPLOYEE WAS CLEANING CHUTE WHEN HE LOST HIS BALANCE AND STARTED TO FALL, WHILE TRYING TO JUMP CLEAR OF CONVEYER BELT HE STRUCK HIS RIGHT FOOT ON THE RAIL CAUSING FOOT INJURY.
WHILE DRILLING ON THE DRILL PRESS A PIECE OF SHAVING WAS GATTING BIGGER AND BIGGER IN AN ATTEMPT TO PUSH IT OUT OF THE WAY IT CURLED AROUND MY FINGER RESULTING IN A SEVERE CUT.

PRIOR TO 7/25/0-0 HE RAN A CAT992C LOADER & WHEN HE TOLD US ABOU T THE PAIN HE WAS REASSIGNED TO RUN THE PRIMARY CRUSHER. ON 11/17/00 HE WAS PUT ON MODIFIED WORKDUTY BUT THE CO WASN'T NOTIFIED UNTIL 2/16/01. HIS WORK HAD ALREADY BEEN MODIFIED TO HELP ALLEV
WHILE SERVICING ROCK TRUCK, EE SLIPPED AND FELL BETWEEN THE LEFT FRONT TIRE AND THE MUD FLAP FRAME, INJURING HIS LEFT KNEE DURING FALL.
EE WALKED UP STAIRS TO WATER TANK AND STEPPED ON SOME LOOSE ROCKS WHICH CAUSED HIM TO TWIST HIS KNEE AND FALL.
EMPLOYEE WAS HELPING DRILL CREW CHANGE DRILL STEEL. WHEN STEPPING DOWN FROM DRILL RIG, HE TWISTED HIS KNEE. HE STATED HE WAS WEARING HIS BIFOCALS LOOKING THROUGH THE LOWER PART AND THE GROUND WAS ACTUALLY FARTHER AWAY THAN HE REALIZED. CAUSED INJURY TO HIS LEFT KNEE.
