Employee was removing debris from cutter in cutless area. When removing a large rock the rock shifted, smashing the employee's left middle finger. Employee was wearing gloves. Went to clinic for evaluation and received five stitches. Employee was also placed on restricted duty.

Showing all 14Became reportable on 7/28/25 when employee had hernia surgery. Employee was attempting to put return roller back into bracket by raising the roller with their legs. The roller then came out of the bracket on the right side and fell to the ground. The employee jerked their legs when the roller came out and they felt instant pain in the left groin area.
Employee was pulling a 2" hose, 50' long to wash out sand from under the tail pulley on #14 belt. EE's back began to hurt after. On 12/06/24 it still hurt, and EE reported EE wanted to go to the doctor where EE was diagnosed with a lower back strain & placed on restricted duty.
