During a tail screen change at the West Dedicated Plant, an employee was potentially exposed to HCN. After the task, the employee reported lightheadedness and nausea, was evaluated at the onsite clinic, and transported to a hospital. No injury or illness was diagnosed.
An employee was working on an Auto Valve, when the handle was pulled, it came loose and the employee lost their balance causing them to fall. Resulting in a fracture on fibular head.

While an employee's (EE) left arm was supporting a drifter on its side during re-installation on the Bolter Boom, additional mechanics began to push the drifter into place with the EE's arm still holding the drifter. This caused the EE's arm to be pinched in between the drifter and the slide, resulting in swelling and soreness to the EE's arm.
Employee felt a pop in their chest while lifting a pipe to screw in spin locks.

Employee was pulling PVC casing off the truck at the end of shift. While the casing was being removed, it started to fall. The employee reached to catch the falling casing and caught finger between the casing and a pipe wrench. It was at this time EE pinched finger between wrench and casing.

As an employee was moving a bottle jack, they felt pain in their arm. It was determined that a tendon was torn. On 11/26/18, the employee had surgery to repair the tendon.
While removing 9 5/8' bottom hole tooling support wrench the driller started raising wrench while helpers were in position to assist the wrench off of tooling to deck. The wrench tipped and helper two removed EE's hands as wrench started tipping in casing. As wrench tipped helper one's left ring finger was caught between wrench handle and casing.

An employee was carrying and spreading two 25 pound bags of floor dry. The employee drove to the shop and was unable to move. The employee was transported to Nye Regional Medical Center for observation. On 09-03-2014, MRI results showed a spinal injury and the employee was taken off of work.
An employee was cleaning out carbon tanks and his feet got wet. The employee was seen at the RMGC Clinic and was released back to work in a dry environment.
