Employee had placed a 5'x10' sheet of metal on the north edge of table while vertical. Employee then went to lower sheet metal to a flat position when the west end of the sheet metal slipped and cut the lower left forearm of the employee.

Employee was climbing into the Grove Crane cab and pulled something in right shoulder. Employee continued to work, only receiving physical therapy. EE will be going in to repair tendon tears in the right shoulder on 1-24-2024. Off 8-12 weeks.

EE was lifting pan to top of 332 silo when the pan EE was guiding caught EE's pinky finger between the pan and piping. Laceration to pinky finger, received 3 stitches and returned to work.

Employee was lifting elevator K019 chain and buckets. Cable on hoist twisted and slipped off of head pulley. Cable caught employees left thumb when it became untwisted.

Employee was emptying out a railcar loaded with Gypsum rock. EE was removing an air powered vibrator (weighing approx. 45 lbs.) from the railcar. EE removed the vibrator and turned to lay it down on the ground, approximately 36 inches. When EE turned, EE felt a sharp pain in back.

Employee was throwing rocks off walkway when a rock smashed EE's finger against handrail. Employee smashed finger, tore nail off finger and out EE's finger. Received 6 sutures to EE's left hand ring finger.

EE and another worker were removing an analyzer from the kiln feed shelf. As the analyzer was pulled out ee dropped end due to sudden pain in right shoulder. The weight of end was no more than 40 lbs. Results of MRI indicate possible rotator cuff tear.

Employee received a first degree burn to right cheek and chemical burn to eyes from kiln feed during an inspection. EE was not wearing proper PPE. EE was treated at the hospital and then later by an eye Dr and by another Dr. ee will be released to go back to work the evening of 8/7/17.

Replacing a motor on K660 coal drag. While lowering the motor to the ground it got caught on a mounting bracket causing the motor to roll, resulting in side-loading the gate on the hook of the Tuggit. Failure of the gate caused the motor to fall. In the fall the motor struck the removed guard causing it to slide into the employee's left shin and right ankle.

Employee was sandblasting and was climbing down to platform when EE's foot slipped allowing EE's right hand, that was holding onto a piece of angle iron, to slip off cutting the inside part of EE's right hand below EE's thumb. Employee received 4 stitches to EE's right hand. Employee did not miss any work.

Employee was packaging cement into 70lb. Masonry bag when bag exploded blowing cement underneath his safety glasses into his right eye. Employee was taken to local eye specialist. The eye specialist required the employee to be off work for 3 days to allow the eye to heal. Eye received a chemical burn and abrasion around the cornea.

EE was helping lift a rolled up piece of rubber belting measuring 36"x25' when he felt pain in his back on 5/16. EE was involved in shoveling & sweeping on 5/29 around the plant when he felt more pain in his back, more of a strain. EE was seen by a doctor on 5/28 & sent home on 5/29 to rest, ice & take medications for strained back after more pain. He will see specialist on 6/11.

EE was shoveling small rock and fine material onto cleanout belt when he felt a pop in his right bicep. EE cont. to work on the day of the injury. EE sent to local Dr. 4/9 then on 4/11 EE could not work due to pain in right bicep and shoulder areas. An MRI was done 4/12 when determined there was a tear in the rt. bicep tendon. EE will be off 8-12 weeks.

EE climbed up on tip of railcar to do preparatory work for loading. As ee started to stand up on the roof of the car his right knee popped our of place and locked up where he was unable to bear weight on his leg. EE was seen by a local doc and released to return to work. On 9/18 ee complained about his knee locking up and sent to another doc, on 9/21 ee was not release

Employee was welding on K670 drag, crawling around inside of the drag on his knees. Went to get out of drag and his left knee was sore and hard to bend. Employee never missed any days from work due to this injury. Went to see a Dr and had an MRI on 11/8/2011. Will have surgery on his left knee around December 9, 2011 and will be off 1-4 weeks

EE was lifting coal buckets full of refractory material on the 2nd flr. of the #5 side of the preheater. No medical treatment was sought at the time of the injury. EE was sent to a local Dr. on 3/28 then an MRI and X-rays on 3/29 and was seen by a back specialist on 3/31/11. Possible herniated disk of L4-5. EE will be off at least 6 weeks doing P.T.

EE was helping the painter move a cart loaded with angle iron where it could be picked up by a forklift. The angle iron was resting on 2X4's. The cart hit a rock causing the cart to suddenly stop causing the 2X4's to roll forward onto their sides. When the 2X4's rolled one rolled onto EE left hand, Middle finger causing a fracture on the last digit of the finger.

EE was loading obsolete pumps onto a pallet. While lifting a pump the EE turned to place the pump onto a pallet and felt pain in his left knee. No medical treatment sought at the time of the accident. EE was still having knee problems and was sent to a doctor on 12/10/10. EE rec. an MRI and saw a Ortho. Dr. on 12/22/10. EE is still working until his knee surgery on 2/15/11.

Employee was remving bin pulley drum from the c024 tripper and strained his middle/lower back. He continued to work through July 13,2009. Company sent employee to the dr on July 14,2009 because he was complaining of having back spasms and pain in legs and feet. Dr wants to evaluate in one week after resting back.

Employee was lifting door on the roller mill fan with his right hand, slamming door onto the left hand of the employee. Left hand swelled up and took employee to the emergency room to x-ray hand & wrist. Employee received a small distal radius fracture to his left arm. Employee is estimated to be off work around 3 weeks.

EE was helping to install the gas pipe on the kiln, the gas pipe spun pinning his right index finger between the gas pipe and forklift rack. The right index finger-tip was broken and required surgery.

Employee was lifting a large container of glycerin in the lab onto the table countertop, felt pain in right arm, hand & shoulder. Employee did not receive any medical treatment at the time of the injury. The employee went to a medical specialist on July 23, 2008 & will require surgery to his elbow & wrist on August 19, 2008 & will be off 4-6 weeks. Lost time begins 8/19/08.

EE was stepping between dust collector compartments when his pant leg caught on the hold down for the door. Twisted his right knee. No medical treatment accident. He was still having knee problems and the company sent him to received a MRI on 10/17/07. He will be seen by a specialist at a date not yet known. He is still able to work at the present time.

Pain is now going into the right shoulder. The accident was not reported to the co. until sept 12. pain increase sept 19-send to dr.

Employee was installing a spacer between a kiln gas pipe and the flex pipe. He was removing a 7/8" bolt from the flange because the bolt hole did not line up to the bolt. Employee was pulling the bolt and the bolt let go coming out hitting his right front tooth, breaking the tooth.

Employee standing directly under 2x2x1/4" angle while millwrights fitting side walls of a duct collector screws housing when the angle fell 1' onto EE's hard hat. He was taken to the hospital for observation. He was released to full duty. During the night he went back to hospital & got restricted duty release. He resigned 2/24/06.

EE WAS REBUILDING A CARDOX TUBE THAT HAD BEEN INSIDE AN ACTIVE PREHEATER. ONE END OF THE TUBE WAS NORMAL TEMPERATURE, BUT WHEN HE TOUCHED THEOTHER END WITH HIS BARE HAND HE RECEIVED A SECOND DEGREE BURN.
