Recent fatalities in Kansas

Struck against a moving object

Victim was removing sand from the sand wall with a Volvo L120E loader and the wall sloughed off covering the loader and victim with sand.

Struck by rolling or sliding object

An individual was standing in the vicinity of a stockpile when material sloughed off from the stockpile and engulfed them.

Caught in, under or between a moving and a stationary object

Victim was caught between track hoe bucket and haul truck. An excavator was loading material in a haul truck parked in the pit. When the victim exited the truck, he was struck by the bucket of the excavator and pinned against the truck.

Struck by flying object

Individual was hit by fly rock while setting off a shot.

Adjusting and cleaning of a tail pulley on a 110' Masaba Conveyor LOTO was not performed by the EE before entering the tail pulley section of the conveyor. Belt was energized by the operator; EE was in tangled in the pulley from his feet to his mid section of his body. EE was fatally injured.

Caught in, under or between a moving and a stationary object

Haul truck was dumping a load of dirt when the dozer operator approched from blind side to get driver's attention when he was struck by the truck and was fatally injured.

Drowning

Employee somehow fell into the water. There were no witnesses so no details available.

THE EMPLOYEE WAS PINNNED BETWEEN THE FRONT BUMPER OF A PICKUP AND A FILING CABINET. THE PICKUP WAS DRIVEN BY THE PLANT MANAGER WHILE HAVING A SEIZURE. THE EE DIED AS A RESULT OF HER INJURI ES.
