Showing all 17Employee was shoveling frozen material from under a conveyor belt. Employee went home and their back started hurting. Employee had a strained lower back muscle and was prescribed a steroid pack and muscle relaxer.
Dust got in the eye of the employee when they went to retrieve a wrench they dropped. The wrench fell onto a covered belt. Employee hit the cover and dust got into their face. No foreign bodies were found when seen at the doctor. Antibiotic drops were prescribed as precaution.
Deckhand was moving a cable when a burr went through their leather glove and punctured left pinky. The burr had to be removed at the hospital. Antibiotic was prescribed and tetanus shot given. Returned back to work. No other treatment given.
Employee was picking up a 50 gal drum, used as a trash can, and dumping it into a dumpster. When EE went to lift the drum, EE hurt left arm. After medical evaluation EE had a torn left bicep tendon that required surgery to repair.
Day Shift Quarry Manager was inspecting a self retracting life line when they experienced shoulder pain. After a period of diagnostic imaging, it was determined that they had a torn tendon in their shoulder. Surgery was needed to repair the tendon. Surgery was completed on 8/30/19.
Employee was helping install a cutting edge on the motor grader. EE right hand index finger and ring finger was pinched between the cutting edge and the mold board. Stitches were required.
Employee was removing a rock from the walkway of loaded barge. Employee reached down with his left hand to slide the rock off when another rock that was resting on top of the rock he was trying to move released and came down on his left index finger.
Employee was cutting the tabs off of Polydeck screens. Employee went to move a ratchet form that was sitting on the stack of Polydeck panel. The knife slipped out of his right hand and cut his left thumb.
It was a cool sunny morning. Employee was walking up the stairs to operate plant when his knee popped.

EE WAS CHANGING SCREENS ON THE SHAKER. WHEN HE REMOVED SCREEN, IT STARTED TO FALL. HE REACHED OUT TO CATCH IT, CUT HIS FINGER
LIFTED A ROCK TO UNSTOP FEEDER AND TURNED/TWISTED.
