The employee was helping to change screens on the crushing plant. The screen slipped and crushed his left finger between the screen and the spring on the vibrator.

EE WAS LOADING A BROKEN SHEAVE INTO A LOADER BUCKET, SUPERVISOR, CAME OVER T HELP PUSH THE SHEAVE INTO THE BUCKET ON ONE SIDE AND IT FELL ONTO HIS HAND.

EE WAS CHANGING THE SCREENS ON OCT 28, HE LIFTED A SCREEN INTO PLACE WITH A SCREEN HOOK. THE SCREEN BEVAME WEDGED AND HE TRIED TO FREE IT BY HAND. THE SCREEN SLID AND CAUGHT HIS HAND BETWEEN THE SCREEN AND THE SIDE RAIL. ON NOV 6TH HE WAS IN THE PROCESS OF TIGHTENING DOWN A SCREEN WITH AN OPEN ENDED WRENCH, IT SLIPPED AND SMASHED HISHAND AGAIN. HE REPORTED IT TO OFFICE.
EE CAME TO MANAGEMENT ON NOV 6TH TO REPORT THAT EITHER ON OCT 19TH OR 26TH WHILE WORKING ON THE PRIMARY CRUSHER HE WAS IN THE PROCESS OF RELINING A STONE CHUTE. HE HAD REMOVED THE OLD CHUTE & WAS PULLING THE NEW LINER UP WITH A CHAIN. HE FELT A PAIN IN HIS BACK. SINCE THAT TIME HE HAS BEEN EXPERIENCING NUMBNESS IN BOTH HIS HANDS.
EE WAS SHOVELING STONE FROM SINGLE SCREW CRUSHER, HE HIT HIS PINKY FINGER ON A PIECE OF METAL ON THE SCREW AND TORE OFF PART OF HIS FINGERNAIL.
EE BENT OVER TO LIFT AN OXYGEN TANK AND LIFTED THE WRONG WAS AND HIS BACK BEGAN TO HURT.
EE WAS UNLOADING OIL DRUMS FROM HIS TRUCK, HE LOST HIS FOOTING AND THE OIL DRUM FELL ON HIS FOOT.
THE PRIMARY CRUSHER HAD JAMMED WITH ROCK, HE WAS CLEANING OUT THE ROCK BY THROWING IT OUT OF THE CRUSHER AND A ROCK CAME BACK AND HIT HIM IN THE ARM AND CUT HIM.
EMPLOYEE WAS IN THE GARAGE AREA CLEANING UP, HE MOVED A SCALLPER COUNTERWEIGHT OUT OF THE WAY. IT HIT A BOLT ON THE FLOOR AND IT CAUSED THE SCALLPER TO FALL OVER AND HIT HIS FOOT. HE WAS WEARI NG STEEL TOED SHOES AT THE TIME BUT IT HIT ABOVETHE STEEL. BROKEN METATARSAL BONE IN HIS LEFT FOOT.
