Struck against stationary object

Technician found unconscious and transported to hospital where he died the following day. He had reported bumping his head earlier that same day but never filed an injury report.

Struck by falling object

Employee was retrieving pry bar from storage area on the ground, when personnel above him lost control of new return roller they were changing out. The return roller fell, striking the employee on the ground. While the employee was wearing proper safety equipment at the time, the impact of the roller resulted in his death.

Caught in, under or between a moving and a stationary object

EXCAVATOR WAS POSITIONED ON TOP OF EMBANKMENT WITH TRACKS LONGITUDINALLY WITH BANK. BANK GAVE OU TIPPING EXCAVATOR ON OPERATOR SIDE OF MACHINE.
