Subject was changing the bags in a baghouse on 3/13 and 3/16 which involved a lot of bending and lifting. Subject had discomfort in EE's back and thought resting it would be fine, but still had pain days later. Subject reported it on 3/19/2026. Subject has been going to a chiropractor to help with the discomfort.

Arrived to work when it was still dark to prepare for a bearing change, as the sun started getting brighter in EE's eye, it began to get sensitive to light and started having pain. Eye kept tearing and felt a sensation of foreign material, so EE flushed EE's eye with solution from the eyewash station. Associate was wearing goggles, didn't know what was causing the irritation.

Associate was installing a bearing on a shaft. Felt pop in shoulder area when they had to push, pull and wiggle bearing to get bearing positioned on shaft.

6 months ago right shoulder soreness was reported to coach when associate tried to move a jersey block that was tipped over and stuck in the mud and had water in it. Associate refused to get checked out by medical professional or any medical treatment. 12/8/22 received notice of restrictions from same associate for injury of same shoulder.

Associate was cleaning up around the wash plant, moving cross members to pick up with the telehandler. In the process of picking up the last cross member they heard a knee pop, which caused them to drop the cross member and knee bumped it on the way down. The right knee was fractured below the knee cap which will cause the associate to be on restrictions.

The water truck operator was not able to clear a truck fall protection rack that was left down by an outside truck driver. When trying to raise the metal rack, their hand was pinched between the rack and water truck mirror.

Miner was moving a rock near a culvert when the rock pinched EE's left hand, ring finger, between the rock and culvert causing a cut to the finger resulting in three stitches.

Miner was in a crouching position to clean out under a railcar. When miner came out under the railcar, miner could not straighten knee that felt locked up in a bent position. Miner had surgery on 6/10/2019.

Miner was helping to shovel sand and felt a pain in EE's lower back. The next day EE called in sick and reported having aches, chills and fever that morning that EE associated with EE's back injury.

Miner slammed the right index finger in a truck door resulting in a swollen finger. Miner sought medical treatment on 7/31/2018 for traumatic hematoma. Miner had a procedure done on 8/3/2018 for a cyst removal.

Employee works as a union millwright; they were assembling parts for a conveyor. The employee went to set down a brace (piece of metal-angle) and pinched finger between a rock that was on the ground and part of the brace and it cut finger. The cut required several stitches. The employee returned to normal duties following treatment at the local urgent care.

While changing an elevator bearing, the miner went to assist the other miner on the opposite side of the elevator. EE's body was then out of position and they noticed a shoulder problem when upper range of motion was attempted. Miner had surgery on 06/25/2018 since shoulder did not heal properly over time.

Associate was positioning hoses on top of a railcar to load the car with sand. After the associate got the hoses into the railcar he stood up and noticed a burning and pulling feeling in his groin area.

An associate was working at the Mobile Equipment Shop, changing out the waste oil tote (platic tote in a cage) when he cut his left hand resulting in medical attention (6 sutures). The associate used his left fist to pound an aluminum tag from the tote cage and was not wearing gloves.

EE and co-worker were bolting a beam in place using the wrong size of bolt, they tapped it out and it flew and hit EE in the mouth fracturing his bottom right tooth.

Replacing fasteners on 980G loader bucket cutting edge assembly. One of the fasteners fell down not in contact with blade anymore so associate had to reach through and push the fastener in place to remove and replace it. The worn sharp edge of this fastener cut through the glove and left middle finger.

No specific event. Working at rail loadout car cleaning site installing 136 pound grating sheets and 24 inch belting in between rails. Utilized a material handler to do heavy work and physical work consisted of positioning the grating sheets in place with two associates.

Associate was assisting to move and lift a 32 foot fiberglass ladder from the scrubber building. The ladder was located in a tight area and was entangled in an upper unistrut. While one associate was working to release the upper portion, the associate on the lower side twisted and injured his lower back.

Associate was installing right steering cylinder on 988G loader. When positioning the front end of the cylinder, the rear portion came off the mount and cylinder shifted pinching his left middle finger between the chain and the frame of the loader.

EE BENT OVER TO PICK UP A PIECE OF THREE INCH TUBING IN THE WELD SHOP FOR A MAINTENANCE PROJECT HE WAS WORKING ON AND FELT SOMETHING IN HIS BACK. HE WENT TO THE CHIROPRACTOR AT 11:45 A.M. AFTE R THE APPOINTMENT HE RETURNED TO WORK BUT THEN WENT HOME. THIS IS A REOCCURRING INJURY. LAST 7000-1 REPORTED ON 9/8/03 AND HE WAS RELEASED ON 10/10/03.

THIS INCIDENT WAS REPORTED TO THE MINE OPERATOR ON 11/25/03. WHILE OPENING A BOXCAR DOOR, THE DOOR GOT STUCK SO THE ASSOCITE PUSHED IT WITH HIS LEG. AS HE APPLIED PRESSURE TO THE DOOR, HE FELT A PAIN IN HIS LOWER LEFT ABDOMEN. LITTLE BY LITTLE THE PAIN IS GROWING. HE SOUGHT MEDICAL TREATMENT ON 11/25/03. HE WAS PLACED ON NO PUSH/PULL RESTRICTIONS UNTIL 12/16/03.

ON NOV. 1, 2001, EMPLOYEE REPORTED HE HAD A SORELEFT SHOULDER AND MAY HAVE TORN MUSCLES. HIS IN-JURY OCCURRED ON OCT. 17, 2001, BUT WAS NOT RE- PORTED AT THAT TIME. HE SAW A SPECIALIST ON 11/7 2001 AND WAS DIAGNOSED WITH LEFT SHOULDER PAIN/ ROTATOR CUFF TEAR. HE WAS PLACED ON LIGHT WORK.
