Over-exertion (Not Elsewhere Classified)

On 03/05/2026 Subject was walking when EE strained EE's calf. Subject first went to the doctor on 03/09/2026 and was put on light duty restrictions. The doctor has indicated that the subject is likely to be on light duty restrictions until May. No further medical information is available.

Fall to the walkway or working surface

Miner went to close hatch on trailer bulk material tank at hatch closing station, stepped in the hole of the tank causing EE to fall. EE's injuries were a sprain of left shoulder, low back sprain and traumatic ecchymosis of right thigh.

Fall from ladders

Contractor climbed up ladder to aid in removing cover to head section. When transferring from ladder to conveyor to tie off, took a wrong step and fell roughly 12 feet backwards landing on the chute and then to the concrete on their back. Contractor broke their tailbone.

Struck by flying object

Arrived to work when it was still dark to prepare for a bearing change, as the sun started getting brighter in EE's eye, it began to get sensitive to light and started having pain. Eye kept tearing and felt a sensation of foreign material, so EE flushed EE's eye with solution from the eyewash station. Associate was wearing goggles, didn't know what was causing the irritation.

Over-exertion in pulling or pushing objects

Associate was installing a bearing on a shaft. Felt pop in shoulder area when they had to push, pull and wiggle bearing to get bearing positioned on shaft.

Over-exertion in lifting objects

6 months ago right shoulder soreness was reported to coach when associate tried to move a jersey block that was tipped over and stuck in the mud and had water in it. Associate refused to get checked out by medical professional or any medical treatment. 12/8/22 received notice of restrictions from same associate for injury of same shoulder.

Walking in deep sand and over rough terrain, bending and twisting. This caused a strain or sprain to the left knee.

Splicing a belt on the McCloskey. Associate was cutting the belt to splice it and the knife slipped and the associate cut the pointer finger on left hand.

While operating the drill press in the wash maintenance shop to make a hole on plate, the drill bit got stuck and the base plate jerked and turned hitting the associates hand and cutting middle finger on left hand. The associate had to get sutures.

Contact with hot objects or substances

The McCloskey S190 was being fed by the loader. The McCloskey suddenly shut off. The operator got out to check what was wrong and there was a code on the machine that said ""cooler fail"". The operator went to check how hot the cap was on the radiator, EE barely touched the cap and the cap flew off and the hot coolant sprayed on EE's forearm causing burns to EE's forearm skin.

Associate was walking towards the stacking conveyor kind of fast next to the clarifier when EE slipped and fell down head first and hit right side of head on the concrete. The concrete was wet and may also have had some algae and floc on it. Possible concussion without loss of consciousness, and sprain of right side ligaments of cervical spine.

Over-exertion in lifting objects

Lifting a chain fall from floor grating, chain got stuck in the grating when pulling up and associate felt pain in their right elbow. Right elbow was sprained or strained.

Struck by falling object

Associate was cleaning up around the wash plant, moving cross members to pick up with the telehandler. In the process of picking up the last cross member they heard a knee pop, which caused them to drop the cross member and knee bumped it on the way down. The right knee was fractured below the knee cap which will cause the associate to be on restrictions.

Struck against stationary object

Changing the head section of the telestacker, associate was tightening a bolt, the wrench slipped off the bolt and caused the associate to hit right elbow against the side of the head section.

Associate was sitting at desk doing computer work in the electrical office, they stood up from chair and turned and felt knee pain.

Contact with heat

Associate was operating a skidsteer on a hot day. The equipment was not equipped with air conditioning and the windows could not be opened because of dusty conditions.

Over-exertion in lifting objects

Employee was lifting 45 degree angle pipes onto pallet when they felt something pop in thier back.

Finger pinched between steel and crescent wrench. Pushing on the wrench and EE was pushing on the wrench. Trying to tighten the belt on the conveyor.

Associate bent over to pick up belt and hurt back from improper lifting. Belt was too heavy for one associate to lift.

Getting screw auger out of baghouse. Arm got caught in ladder cage and smashed by auger.

Fall from machine

Customer Truck Driver was closing the trailer lids at the safety rack when EE lost EE's footing falling off the trailer to the ground below. The employee was transferring from the safety rack platform to the top of the trailer before the incident occurred.

Miner was removing a head pulley shaft when it fell pinching EE's right hand between the shaft and the work table.

The water truck operator was not able to clear a truck fall protection rack that was left down by an outside truck driver. When trying to raise the metal rack, their hand was pinched between the rack and water truck mirror.

Struck by falling object

Miner was checking to see if the feeder gate was shut when a guard fell striking the top of EE's knee.

Fall to the walkway or working surface

Miner was walking in the wash and tripped on a pump cover and fell to the ground. Reported the accident on 02/11/2019. Miner had surgery on 03/18/2019 which resulted in being off work.

Fall from machine

While the miner was stepping off the dredge onto the gang plank, the plank shifted causing the miner to fall into the water. Associate sought medical attention on 2/11/2019 and was placed on restrictions.

Miner was helping to shovel sand and felt a pain in EE's lower back. The next day EE called in sick and reported having aches, chills and fever that morning that EE associated with EE's back injury.

On 9/30/2018 worker had been pulling out wire. An hour later while walking around the plant the worker's back started to become sore. The worker had been feeling better but then while walking on 10/13/2018 felt a pop in back and pain. Worker sought medical treatment on 10/25/2018 which then became restricted duty.

Caught in, under or between a moving and a stationary object

Employee works as a union millwright; they were assembling parts for a conveyor. The employee went to set down a brace (piece of metal-angle) and pinched finger between a rock that was on the ground and part of the brace and it cut finger. The cut required several stitches. The employee returned to normal duties following treatment at the local urgent care.

Struck against stationary object

While walking and carrying an ice cooler the miner stepped on a rock twisting EE's left ankle. Miner stated had a previous ankle injury. The medical service strongly recommended therapy. The miner had received therapy in the past and did agree to try therapy again since it was so strongly recommended by the doctor.
