EE is a Welder Repairman was changing v-belt on conveyor while installing finger became caught between belt and sheave. EE was sent to Occupation Health for a pinch to tip of little finger Rt. Hand. Over the counter meds were given. EE stated that the tip of finger was bent inward. EE was sent to Orthopedics and a splint was placed on finger 7/25/17. Continues to work Full Duty.

Emp was transferring cement from Old Silo 13 through screw conveyor while air lancing angled pant leg cement flushed came out of door and struck him in the face. Emp was wearing eye protection, but did get dust in his eyes. He was sent to emergency room to flush eyes and was given prescription eye drops.

Emp was operating Volvo end loader when he noticed a flash as he dug into pile of rock. He continued to operate when he noticed black smoke. He then realized a hydraulic line broke and was smoking. He retrieved the fire extinguisher mounted on handrail of loader and bumped his Rt. elbow on handrail. He was sent to Occ Health on 6/4/14 he received restrictions of no work Rt. arm.

Emp tried to pull hood open on semi to put washer fluid in container. Pulling on hood handles, pulled hard on hood and it did not move. Emp felt pain in Lt. Shoulder. Emp continued to work until surgery on 3/19/2014.

Emp was attempting to unplug vessel at PreHeat Tower. Emp was at the tipping valve location, valve had become stuck with material. Emp pulled down, valve broke loose causing emp to strike his Rt. hand on metal plate. (10 stitches)

Miner was lifting 12'x8" channel overhead to be welded in place. Miner reported to the employer on May 23, 2013 that he had pain in the lower back. Employer offered medical treatment but miner refused. on 5/24/13 miner requested medical treatment and was placed on restricted duty. The project was 100% complete on 5/24/13 and the miner was laid off at the end of shift.

Emp stated on 03/26/2012 he was installing the door (approx. 50lbs.) on the 710 elevator with three other employees. As they lifted the door he felt a pop in his Rt. shoulder. Emp continued to work until having arthroscopic surgery on 01/14/2013.

Emp was placing door 5" x 14" (10lbs.) back into place at kiln riser duct 5th floor preheat tower. While tightening T-handle screw door slipped and started to drop employee caught door with Rt. hand on bottom of door which then caused the tip of his Rt. hand middle finger to strike edge of door below. X-Ray showed fracture to tip of finger. Emp was wearing heavy gloves

Emp was lifting a chain out of toolbox. Emp stated he strained his Rt. shoulder while lifting. Emp continued to work at his position, when discomfort continued an MRI was performed and showed a tear, emp elected to have surgery on 04/28/2014. He is a lost time as of surgical date.

On 12/06/2011 employee was pulling wire up to a wire tray in motor control room. While lifting the heavier wire with his Rt. hand and untangling smaller wires with his Lt. hand felt a pain in his Rt. thumb. Emp was being treated with anti-inflammatory medication until examination of Rt. thumb trigger finger was determined. A trigger release was performed on 02/01/2012.

On 01/21/2010 emp was installing Finish Mill liners using a com-a-long and guiding liner into position / liner moved suddenly emp reached out to stop movement and felt strain in Lt. shoulder. Emp was examined /possible tear in shoulder emp. remained at job and attended PT. Due to continued discomfort emp had surgery on 05/18/2010

Employee was working in the Finish Mill, saw the bobcat operator moving a 55 gal. barrel from under a dust collector stand. He went to help rotate the barrel around to help the operator, as he grabbed the top of the barrel, the t-bar attached to the bobcat pinched his Rt. hand pinky finger against the barrel and caused a laceration that required 5 stitches.

Employee was changing a conveyor belt in the clinker storage area. The employee was exposed to clinker dust for an extended period causing irritation to eyes, skin and throat. He had a burn from the dust to the ankle and his eyes and throat were irritated causing discomfort.

Employee was splicing #434 belt while securing/pulling the conveyor belt he felt a pain in his Lt. shoulder on 05/29/2007. Employee was sent to the doctor and returned to work with restrictive duty of no heavy use Lt. arm and continued in his maintenance position. Discomfort continued and employee had surgery 09/17/2007 and remains off work.

Employee was examined and continued to work at his regular job with Lt. duty restrictions on his Rt. arm. Employee was scheduled for Ortho surgery on 12/18/2006 for torn ligaments in his Rt. shoulder. Employee has lost work days as of 12/18/2006 and has not returned to work.

Employee was opening the inspection door on the 568 Rotary Feeder door swung down striking Rt. hand finger between the door and the feeder. Employee was sent for examination and stiches were required to the tip of finger. Employee was not a lost time.

Injured Worker was installing an expansion joint into a duct of a preheater. He and a coworker had placed a section of it and then had to slide it into place. The piece weighed approx. 120 lbs. As they were sliding it the IW felt a sharp pain in his lower back.

Employee was picking up a brick off of a stack of bricks while turning with brick in his Rt. hand he struck the forktruck lift bars causing a laceration/contusion on the tip of his thumb. He was taken to hospital and received five (5) stiches in his thumb. He did not miss work.

EE WAS CARRYING A CUP OF COFFEE IN HIS LEFT HAND AS HE ENTERED HIS CONTROL LOADING BOOTH. HE WAS IN THE PROCESS OF SETTING HIS COFFEE CUP DOWN WHEN HE STRUCK THE CUP ON THE CORNER OF A METAL CONTROL BOX, CAUSING THE CUP TO BREAK IN HIS HAND. EE REQUIRED 4 STITCHES IN HIS LEFT HAND.

EE strained back on 12/03/05; was able to continue regular duty. 12/20/05 - doctor ordered light duty restrictions. From these restrictions EE cannot perform all the duties connected with job requirements. EE was attempting to lift a frozen piece of material off a belt when he felt strain in lower back. 1/5/06: new restrictions & meds. - EE will have lost work days.
