Employee was helping to lift a short conveyor frame when ee felt a slight tug in groin. A follow up exam at a medical facility reveled ee has a hernia.

The miner was working on reversing the toggle plates in an Austin-Western jaw crusher. Miner was in the rock box removing a cheek plate when the stationary toggle slid approximately ten inches, and during its travel, it came in contact with the miner's hard hat. As a result, the miner received sprained muscles in back and neck.

He was removing a return roller from the sandstacker. He was using a 9' piece of pipe to push up on the belt in order to take the down pressure off the roller so it could be lifted out of the brackets. he pushed up on the pipe and felt a sharp pain in his right arm between the elbow and shoulder.

This man has a history of diabetes. Anytime he gets a scratch, he gets an infection. He was replacing a bolt in a transmission. He reached down through the floorboard in a D7 Dozer, when he scratched his left elbow. He didn't tell anyone. He got an infection in it and was put in the hospital and on IV's on the 15th of Sept.

Employee was attempting to free a large rock stuck in feed hopper. Had is hand on the conveyor frame when the rock dislodged and fell on his hand before he could remove it.

OPERATOR WAS APPROACHING LOADER, DUCKED UNDER LAND CONVEYOR & RAISED HIS HEAD PREMATURELY & HIT HIS HEAD. SAID NECK WAS SORE. WENT TO HOSPITAL & DID NOT RETURN. HE HAD WEEKS VACATION. WHEN HE RET'D TO WORK HE SAID NECK WAS STILL SORE. WE SENT HIM TO DR & HE WAS PLACED ON LIGHT DUTY 8/17/04.

SHOP DOOR STUCK. TRIED TO PUSH TOP OF DOOR TO OPEN IT AND NAIL STAPLE WENT INTO LEFT WRIST (TRIMWAS MISSING AROUND WINDOW). THE SHOP DOOR HAS BEEN TAKEN OFF.
