Employee was palletizing and used their left knee to push on the stack of rocks to straighten them causing a bruise reported on 8/9/19. Employee requested medical attention on 8/12/19.
Employee was palletizing landscaping rocks when EE felt a burning sting in the lower right side of the abdomen. The employee had hernia surgery early this year in February. EE was seen in urgent care at the Health Center that same day and had an ultrasound on 7/01/2019. Diagnosis was a strain in the lower right abdomen, no damage to the repaired hernia.
Employee was lifting roll of fencing to wrap pallets and as they sat down the roll they dropped the roll in a weird way and felt their shoulder pop.
Employee was installing new boards to lowboy trailer. They bent over to pull out a new board, when they felt a pop in their back, no pain at the time. Employee did not request help to move the board when other employees were in the area.

Employee held on to the head of a worn plow bolt while another employee tried to take the nut off. Bolt spun in employees fingers and cut them.
Employee was palletizing and as EE removed a stone from the pile to place on the pallet, another stone slid down and caught the employees right hand ring finger, crushing it.
Employee was helping to install a V-Belt on motor sheave, and got ring finger caught between the two crushing the finger.
Employee did not report the incident to his supervisor until 12/16/15 which was diagnosed by a doctor to be tendonitis. Employee incident report stated that he was dismantling the suspension on a 1 ton service truck on 12/8/15. When he awoke on 12/9/15 he was experiencing pain in his left wrist. He worked for one week without telling anyone.
Employee performing a pre-shift start up and while opening inspection doors to check shoes left door bound up. While positioning himself to get a second hand and better grip on the door to apply more force to try and overcome the bind, the door fell back down on his right index finger. Employee went to emergency room and received 11 stitches but is back to work on modified duty.

INCIDENT NOT REPORTED UNTIL OCTOBER 9, 2006. Employee claims original injury occurred 06/02/06. He did not inform the company until 10/09/06. EE claimed, "Nothing out of the ordinary" the he could remember to cause the injry. EE indicated he has injured this body part (lower back strain) before.
WORKER WAS CARRYING A STEEL GRATE. GRATE SLIPPED FROM HIS HAND AND STRUCK HIS LEG CAUSING LEG TO BE CUT & SCRAPED.
WORKER WAS UNLOADING 128TH ANVILS AND SMASHED/CUT FINGER ON LEFT HAND WHEN 2 ANVILS COLLIDED.
EMPLOYEE DROPPED HEAVY OBJECT ON LITTLE FINGER OF LEFT HAND AND CUT IT OPEN.
