Accident type, without injuries

Outside hauler from Brix dropped off material at rap pile and left box up while pulling away, resulting in the box making contact with an overhead 240 powerline pulling it down. Power line fell on top of the hood of the Brix truck. Hauler stayed in the cab for more than 30 minutes until a qualified electrician from PGE removed the power and clear the scene. No injury.

Caught in, under or between a moving and a stationary object

Individual attempted to lift a 5-6 ft. long chunk of I-beam (weighing approximately 150 lbs.) and toss it over the handrail of an elevated screen deck to the ground below. The I-beam shifted and pinched their right thumb between the beam and handrail. Individual complained of extreme pain and was transported to Urgent Care where an X-Ray discovered a broken bone in the right thumb.

On 9/11 individual reported injury of unknown origin. EE later states that on 9/1 EE was attempting to obtain leverage with EE's shoulder on a wrench handle, and injured EE's shoulder trying to break a screen bolt loose. Last date of work was 9/11, on 9/17 was seen by medical provider and released to work with restrictions. No-call, no-showed and terminated 9/21.

Had wrench resting on left shoulder for leverage. Pushed upward with arm and shoulder, felt a pop in shoulder then pain.

Employee positioned their upper body between the belts to knock a rock loose from tail pulley and belt using a hammer. When rock was freed stored up energy caused the belt to move in an unexpected forward motion catching the employee's hands between the belt and the pulley. Employee locked out the breakers but conveyor was not blocked from motion.

Work was being performed on screens. Employee 2 was on an upper level shoveling off material to the ground and employee 1 walked under the area where employee 2 was shoveling material off to. Employee 1 had a small rock make contact with rib cage. No medical was requested at time.

Employee was installing new boards to lowboy trailer. They bent over to pull out a new board, when they felt a pop in their back, no pain at the time. Employee did not request help to move the board when other employees were in the area.

Employee was operating a loader when a Haul truck backed into the loader jostling the driver in the seat and hurt lower back.

Employee held on to the head of a worn plow bolt while another employee tried to take the nut off. Bolt spun in employees fingers and cut them.

Employee was helping to install a V-Belt on motor sheave, and got ring finger caught between the two crushing the finger.

Employee smashed finger against concrete wall behind tail pulley while adjusting the belt using a ratchet. Was on modified duty for two days and then released to full duty.

Struck by powered moving object

Employee was putting tools away in vehicle after repair of water line when loader backed up and pressed employee against vehicle. Employee sought medical attention at approximately 1 PM and was diagnosed with muscle strain. Employee has been working modified duty since the incident.

Employee did not report the incident to his supervisor until 12/16/15 which was diagnosed by a doctor to be tendonitis. Employee incident report stated that he was dismantling the suspension on a 1 ton service truck on 12/8/15. When he awoke on 12/9/15 he was experiencing pain in his left wrist. He worked for one week without telling anyone.

Fall to the walkway or working surface

At approximately 9:20 PM an employee was removing and replacing a section on 9505 and while taking a better stance he slipped and fell on his RT elbow causing a contusion.

Lifting and throwing rock out of haul road. Employee developed lower back pain.

Employee was using a hand grinder with a cutoff wheel to remove a seized up bolt. The grinder grabbed and threw itself into his leg, causing a cut to left knee.

Employee was removing an I-beam when one end of it unexpectedly dropped, raising the other end, pinching his finger between the beam and a chute causing his right middle finger to be cut.

Employee was lifting a piece of screen cloth to the screen deck when he bumped a piece of screen cloth that was staged to be lifted causing it to shift and strike his left hand cutting his fingers. He received stitches to his middle and ring fingers.

Employee was replacing a sheave on a motor overhead. The taper lock came loose and the sheave struck the employee on the back of his hard hat causing a slight strain to the employees neck muscles.

The employee was operating an end dump and was preparing to travel down a haul road to enter the pit. As he began down the haul road he reported the brakes had malfunctioned. There was a front end loader at the bottom of the haul road. The end dump hit the front side of the loader. There was moderate damage to the haul truck. The driver sustained bruised ribs.

The employee was operating a front end loader to clear rocks from a haul road. It was reported that the brakes on an end dump malfunctioned as it traveled down the haul causing the end dump to hit the front side of the loader. The loader sustained moderate damage. The loader operator has soreness in his back.

The employee was disconnecting the road widener machine from the front of the loader boom. The left coupling pin got stuck and he was using a hammer and a large bolt to knock the pin loose. As he struck the bolt, the pin freed up, and the finger he was holding the bolt with was pinched between the couplers cutting his finger. He received stitches from this incident.

The outside contractor was onsite working on the rock drill. A piece of drill steel started to fall out of the carousel. The contractor grabbed it to push it back in place as the operator engaged the grabbers. The carousel turned pinching the contractors little finger.

Employee was replacing a conveyor belt. The new belt was being pulled on by using ropes. While pulling the new conveyor on, one of the ropes broke, striking the employee in the face, causing lacerations to the right side of his face and chin.

INCIDENT NOT REPORTED UNTIL OCTOBER 9, 2006. Employee claims original injury occurred 06/02/06. He did not inform the company until 10/09/06. EE claimed, ""Nothing out of the ordinary"" the he could remember to cause the injry. EE indicated he has injured this body part (lower back strain) before.
