Injured & EE's partner were removing pan skirts. The injured was installing a swivel eye lifting device while EE's partner partially lifted the liner with a lining bar so EE could install the 3/4" nut underneath. The liner slid down the lining bar, pinching the injured's right middle and pointer fingers between the liner/nut and the pan feeder flights
Employee was finishing up a belt discharge on 72-5. When EE was closing the doors on the crusher EE pinched EE's palm between the door and the lock post causing a 2-inch laceration
Employee was performing a pre-shift on the shovel. The employee was trying to open the door with their right hand. The door was stuck, and the employee felt a pop in their right shoulder.
Injured MTM was installing a drive sheave on a pump with a partner. The MTM's partner was installing the sheave while the injured MTM was pushing on the guard. The injured MTM's right pointer finger was in the pinch point of the guard hinge. When the injured MTM felt the pain from the guard pinching their finger, they pulled it out while it was still pinched.
EE was pushing a sheet of stainless into position to be welded, as they were pushing the sheet they felt a sharp pain in left shoulder. This resulted in a strain/sprain type of injury.
Employee was changing out return rollers on a belt. Employee was lifting return roller on the one side into place on bracket. The roller slipped and dropped to the floor 12 inches smashing the right ring finger fingertip between the floor and edge of roller causing multiple fractures and laceration to fingertip. Employee was sent to VRMC and returned to work with restrictions
Employee was pushing up on a conveyor belt trying to get additional clearance to remove a roller. After releasing the belt employee felt a popping and burning sensation in their hand
Employee felt pain in the left arm when helping move a window a/c unit. THIS INCIDENT BECAME A RECORDABLE ON 3/13/26 WHEN THE EMPLOYEE RECEIVED RESTRICTIONS.
043-06-1 conveyor belt where employees were doing work to the belt and changing rollers. Employee was helping clean up and pulling out a return roller from under the belt at which time EE felt pain in EE's left bicep. Other employees were also present at the time of the incident. Employee said EE is unable to straighten EE's left arm
MTM was changing all-thread to level the thickener rake. When employee was finished, EE felt pain in EE's shoulder. Employee climbed down from the job and reported the incident to supervisor
Employee was moving breaker from waist level off of plant kat onto plastic cart which was at knee height and approximately 2 feet away from plant kat. Breaker weighed approximately 120 pounds; the lift was assisted with another employee. The employee felt pain in their left bicep while moving the breaker
MTMs were changing rollers on a belt. They were dragging a roller with a rope and decided to each carry on one end. The roller weighs about 150#. The injured carried the back and felt a pain go through their left elbow area. The employee reported the incident to their supervisor at the start of shift the next day (9-12-25).
Employee was lifting a box weighing approximately 35LBS putting it into a pickup and felt a pop in left bicep
Employee was changing a belt on a drive motor and pinched EE's finger
EE was removing steel grating from a floor launder for the purpose of cleaning the launder. As they were pulling the grating they experienced a pain in lower back on the right side. This incident resulted in the EE having a sprained lumbar.
Rolling a pin into a skid steer bucket and felt slight pain in EE's lower back.
EE was grinding/welding as part of bull gear repair that was taking place. They stopped grinding to visually check for high spots. When they flipped up their face shield, they felt something enter right eye. it was determined that the EE had a Corneal abrasion as a result of a small piece of grinding debris entering their eye.
At 8:30 am the operator called supervision to report that at approximately 5:40 am they were pulling out the right-side fuel hose and after pulling out 10 feet of hose the reel stopped. At that time the employee stated they felt and heard a pop in their right elbow. A few hours later the employee felt more pain in their right elbow they called for supervision.
Employee was moving a 17" by 17" piece of broken shell liner (discharge liner) to place lifting sling around for removal. The Liners weight came over center, the injured reached for the liner, grabbed a sharp edge and cut EE's right palm area. The liner cut through the TIG welding glove. Cut was approx 2 inches in length
When doing alignment for the fan motor the rotor has to be spun. Employee was moving the rotor manually by pushing on the fan blades and felt pain in the right shoulder. THIS INCIDENT BECAME A RECORDABLE ON 7/24/25 WHEN THE EMPLOYEE RECEIVED RESTRICTIONS UPON SURGERY.
Operator was raising landing gear on a semi-trailer and strained their back
Two employees were lowering the upper handrail of a scissor lift. When one employee pulled the 4th pin holding the handrail, the handrail assembly fell unexpectedly and pinched the injured employee's left pinky and ring finger between the upright and the mid-rail. The first employee then lifted the handrail up to release the injured employee's fingers
Employee was at 071-01 pan feeder pulling a rock to unplug. While pulling the rock the employee felt a pop near their belly button. Hernia.
EE was installing NI-Hard liners inside of a chute. the liner EE was handling slipped from EE's hands, instinctively EE reached out to grab it and it struck the tip of EE's right middle finger pinching it between the chute wall and the liner. This resulted in a fractured fingertip and sutures were required as well.
Employee was pulling a new hydraulic hose out of the back of EE's service truck that still had plastic on it. EE took EE's gloves off before handling the hose so EE could rip the plastic off of it. When EE pulled the hose EE felt a sharp pain in EE's left index finger. When EE looked EE had about a 1 inch laceration on the end of EE's index finger
Employee was assigned to replace the bearing on a return adjustment pulley. As the injured went to pull the bracket out, the bracket became wedged below near a piece of welded flat stock. The bracket then came free and dropped further pinning the employee's left thumb between the bracket and the piece of flat stock.
A Crusher Utility Tech. was in the process of dumping limestone side dump cars. The employee pulled the car dump lever. As the car was dumping, material was frozen in the car, causing the car to lift off the rail on the south side. The employee held onto the dump lever and felt pain in their lower back
Employee was removing a 6'6" chunk of bin rail off of 002-5 conveyor belt after a metal trip. As the employee attempted to remove the rail, their right ring finger was pinched between the rail and a rock. The rail weighed roughly 249 pounds.
Employee was changing out brake calipers/rotors on a haul truck. Employee was removing the outer rotor on #3 and #4 tires. The rotor dropped and pinched employee's right pointer and right ring fingers between rotor and wheel hub. Employee received restrictions on 10/29/24
The employee was repositioning the seat cushion with the driver's door open. When closing the door, the spotlight cord got caught in the door. When moving the cord and shutting the door a second time, the employee's right-hand fingers got smashed. This became a recordable on 11/12/24 when employee had physical therapy.
This incident became a recordable on 10/01/2024 when the employee was placed on restrictions. The employee and a partner were removing the guard on 111-10-4a cobber to repair the sprocket and chain. While removing one side of the guard, the employee felt pain on the inside of left elbow below the Bicep. The weight of the guard is approximately 80 lbs.
Employee was sitting in center office, took off safety glasses and rubbed left eye with left, bare hand. Employees eye became irritated. Employee attempted to flush eye but irritation persisted.
An ironworker was working on a door frame for the pre-heat. EE lifted a 6 foot long section of 5 inch channel iron that weighs 54 pounds. The employee felt a pain in EE's abdomen and was found to have an umbilical hernia.
Employees were adding rebar to sump hole structure when the plywood used to cover the hole, now leaning up against the wall fell on the employee's right hand that was resting on top of the concrete just outside of the sump hole causing it to pinch EE's right thumb between the plywood and concrete causing a contusion to the thumb. Size of the plywood was 3/4 thick by 4x8.
The injured employee was removing the bolts for the discharge pump on the 3P1042 pump. When removing the last bolt, the pipe moved pinching employee's right hand between pipe and the motor base for the pump.
The injured employee and a partner were assigned to change out a troughing roller on the 045 belt. The injured employee was changing out a wing can on a troughing roller and putting in a new wing can. During this process, the wing can dislodged after being stuck, and smashed the tip of the employee's middle right finger.
57 drill had guards removed for steam cleaning. Employee was assigned with partner to reinstall guards. Employee was reinstalling the fan motor guard with partner when EE got finger pinched between the guard and the cooler. Pinched finger handling engine cooler guard. It was their left ring finger. Guard weighs about 50 lbs.
While removing scaffolding from the Line #3 feed cone an employee sustained an injury when a scaffold pan (plank) that was being handed to EE dropped and struck the injured employee on the back of the right hand.
When an employee went to pick up washdown hose, it swung around and struck EE in the right cheek. THIS INCIDENT BECAME A RECORDABLE WHEN THE EMPLOYEE HAD A MEDICAL PROCEDURE TO REMOVE METAL PARTICLES FROM THEIR RIGHT CHEEK ON 8/27/24.
The employee was walking a full 55-gallon drum onto a spill containment pallet from the forklift forks. The forks were positioned level with the pallet for ease of transfer. The employee felt no discomfort immediately following this task but felt discomfort shortly after. Prior to the incident the employee didn't feel any discomfort. The weight of the drum is approximately 400lbs.
4 employees were removing liners inside a rod mill. The employees were using lining bars to remove liners inside the mill. Employees stated that as they were in the process of removing the second liner from the feed end, the liner to the right of the one they were removing tipped over contacting the employees' right leg. The approximate weight of the liner is 688 lbs.
Employee was in the process of loading a filter kit box into the 827-service truck when the employee felt a pop in left shoulder. The box of filters weighs approximately 40lbs. THIS INCIDENT BECAME A RECORDABLE ON 08/11/2023 WHEN THE EMPLOYEE RECEIVED A RESTRICTED LEFT ARM RESTRICTION
Employee had collected pellet sample and was working it up. They were lifting the pan and sample (approx. 27 lbs.) to put into the Gilson for screening when they felt a burning/tearing sensation in their right shoulder. Employee reported to plant supervision when the pain increased.
Employee was working from a mobile platform lifting the left side production truck box pin into position. As the employee was setting the pin in place it slipped in employee's hand. The employee's thumb was pinched between the pin and the #4 production truck tire. Employee slid the pin into position and then came down from the work platform. Estimated pin weight is 80 lbs.
Two employees were replacing the broken railroad sign on the tracks. The signs were slid off the pickup and resting on the ground and tailgate. The sign slipped out of the employee's hands (wearing winter gloves). The weight of the sign brought the sign to the upright position which contacted the employee's right ear. The employee suffered an ear laceration.
Employee was exiting the break room and pushed on the glass portion of the door and the glass broke. Employee received a one-inch laceration on the palm of the left hand.
The employee was operating the loader doing high bottom clean up in the west pit. The right corner shank and tooth broke off the loader. The employee got down to push the shank and tooth into the bucket and haul it out of the pit. While in the process of pushing it into the bucket, it pinched EE's right pointer finger against the other tooth and shank on the bucket.
Employee was assigned to replace a countershaft on 12-18-22. The employee felt something in left eye, flushed the eye and thought that it had been removed. Irritation persisted and the employee reported the injury on 12-19-22. The employee was sent to an ophthalmologist where a rust ring was removed and antibiotic medicine was prescribed.
