Over-exertion in lifting objects

EE was working on housekeeping tasks. They bent down to pick up an old roller from a conveyor belt and felt a pain in their right wrist. Strain/Sprain

Struck by rolling or sliding object

At approximately 11:55am, the Line 3 scrubber drop out box ruptured releasing large volumes of water onto the floor. The employee was likely north of the launder when the water swept them down the inclined walkway wedging them beneath the kiln pier stairs.

When the bit caught a piece of steel, the hand drill twisted in the employee's hand striking the employee in the face with the battery of the cordless drill.

Over-exertion in pulling or pushing objects

The employee was pushing a scrap skip weighing approx. 2200 pounds across the Filter floor on a pallet jack when the employee felt a pop in their right calf.

MTM was lifting Milwaukee battery operated come-along that was in the case from the floor to EE's plant kat

Caught in, under or between a moving and a stationary object

Injured MTM was installing a drive sheave on a pump with a partner. The MTM's partner was installing the sheave while the injured MTM was pushing on the guard. The injured MTM's right pointer finger was in the pinch point of the guard hinge. When the injured MTM felt the pain from the guard pinching their finger, they pulled it out while it was still pinched.

Over-exertion in pulling or pushing objects

EE was pushing a sheet of stainless into position to be welded, as they were pushing the sheet they felt a sharp pain in left shoulder. This resulted in a strain/sprain type of injury.

Accident type, without injuries

Employee became entrapped in the Step 1, 2 Agglomerator basement elevator. The main gate was notified at 852 AM and the employee was able to safely exit the elevator 927 AM.

EE was working in the Kiln. They moved to the side and left knee buckled. this resulted in a strain/sprain type of injury.

Accident type, without injuries

Employee was stuck in the Step 1, 2 Agglomerator basement elevator. The employee became trapped at 902 AM. Security and safety were eventually notified. First responders arrived shortly after 930 and were able to get the employee out at 940 AM

Employee was pushing up on a conveyor belt trying to get additional clearance to remove a roller. After releasing the belt employee felt a popping and burning sensation in their hand

Over-exertion in pulling or pushing objects

043-06-1 conveyor belt where employees were doing work to the belt and changing rollers. Employee was helping clean up and pulling out a return roller from under the belt at which time EE felt pain in EE's left bicep. Other employees were also present at the time of the incident. Employee said EE is unable to straighten EE's left arm

MTM was changing all-thread to level the thickener rake. When employee was finished, EE felt pain in EE's shoulder. Employee climbed down from the job and reported the incident to supervisor

Over-exertion in lifting objects

Employee was moving breaker from waist level off of plant kat onto plastic cart which was at knee height and approximately 2 feet away from plant kat. Breaker weighed approximately 120 pounds; the lift was assisted with another employee. The employee felt pain in their left bicep while moving the breaker

EE was replacing a valve. EE was using a torch to remove flange bolts, once EE was down to the last bolt, water that was trapped in the line started to spray out from around the flange. Not wanting to get wet, EE took a step backwards and tripped over a curb. EE put EE's hands out to catch self and bent EE's right wrist backwards, this resulted in a fracture of EE's wrist.

Employee was traveling to sample pellets from line 4 and about the climb stairway from basement level, when they slipped on the pellets and mud at the base of stairway and twisted their left knee.

Over-exertion (Not Elsewhere Classified)

EE was using a cart to haul boiler tubes. The surface was smooth/flat concrete, slag/ash had fallen off the tubes while loading them on the cart. slag/ash prevented the wheels from freely moving in the intended direction while being pushed. This resulted in the cart tipping to one side. the EE stopped the cart from tipping, in doing so the EE strained the muscles of EE's left arm.

Employee was walking from Line 4 performing routine rounds when they slipped on pellets. The employee caught themselves & jaw came down and locked inflicting pain to lower jaw area. The employee notified the supervisor and EMS was called. The employee was then transported to telemedicine and later returned to work with a sore jaw and will be followed up on next shift per EMS

EE was removing steel grating from a floor launder for the purpose of cleaning the launder. As they were pulling the grating they experienced a pain in lower back on the right side. This incident resulted in the EE having a sprained lumbar.

EE didn't have a belt to hold up EE's FR work pants, so EE used zip ties through EE's belt loops as a makeshift belt. At the end of EE's shift, the EE grabbed a utility knife to cut the zip ties from EE's belt loops, the knife slipped and the EE cut the palm and thumb of EE's left hand. this resulted in stitches.

When doing alignment for the fan motor the rotor has to be spun. Employee was moving the rotor manually by pushing on the fan blades and felt pain in the right shoulder. THIS INCIDENT BECAME A RECORDABLE ON 7/24/25 WHEN THE EMPLOYEE RECEIVED RESTRICTIONS UPON SURGERY.

Fall onto or against objects

The ee was working on the under grate of the line #3 pre-heat. There was a plywood work platform covering the 7-inch holes created by the flipper castings. The ee was pulling another sheet of plywood in to extend the platform, when ee's left foot stepped into the hole ee was planning to cover. This caused ee to drop to left knee, resulting in a laceration.

The incident occurred 30 minutes prior to the start of the employee's work shift. The EE was planning to unload tools from EE's car into the job trailer. When the employee was getting out of the car, EE slipped or tripped but never fell. The employee twisted to the right in an effort to regain balance, this caused a muscle strain.

Struck by flying object

Employees were assigned to change out a bad gearbox on the drive end of the 208-01-2 filter. The gearbox wouldn't move with the hammer. They used a ram hydraulic puller. One of the puller fingers sheared off the gearbox sending the puller fingers flying into the left shin of one of the workers. The employee was lacerated by the puller and was transported to the hospital.

EE was installing NI-Hard liners inside of a chute. the liner EE was handling slipped from EE's hands, instinctively EE reached out to grab it and it struck the tip of EE's right middle finger pinching it between the chute wall and the liner. This resulted in a fractured fingertip and sutures were required as well.

Accident type, without injuries

Employee was trapped in Step 1,2 Aggl Shop elevator at 8:00AM. The employee called the control room and MEI was called to repair the elevator. The main gate and Em. svcs were not notified until 850AM. The employee left the elevator unharmed at approximately 9:06 AM. The elevator failure was caused by a door control issue that appears to have frozen. MSHA was notified at 9:14 am

Employee was using a utility knife to cut loose zip-ties. While cutting the zip-ties the knife blade cut across the left thumb causing approximately a 1-inch laceration that needed to be closed with 2 sutures. Employee was cutting towards their opposite hand and not wearing any protective gloves at the time of the incident.

Conditions in the area were icy and snow covered. The slippery conditions caused the employee to slip and fall while walking to a work area. The incident resulted in a right hip and lower back strain.

Employee was assigned to replace the bearing on a return adjustment pulley. As the injured went to pull the bracket out, the bracket became wedged below near a piece of welded flat stock. The bracket then came free and dropped further pinning the employee's left thumb between the bracket and the piece of flat stock.

Employee was cutting rubber off the inside of a rubber lined pipe when the knife slipped out of the rubber. The blade cut through their glove, lacerating the employee's left forearm.

Accident type, without injuries

Employee was riding the freight elevator down when it broke down leaving the elevator suspended beneath the fourth floor. Communications were made and the trapped employee was able to verbally communicate with the rescue team. The elevator was reset. The doors were manually opened allowing the employee to get out of the elevator at 8:47AM MSHA was promptly notified of the event.

Employee was outside smoking when the wind blew debris into their eye and a foreign object become lodged in their right pupil

Employee descended stairs from flux tank catwalk to slurry tank catwalk, at the bottom of the stairs, while maneuvering around scaffolding, employee tripped and fell, striking the floor grating, causing lacerations to the chin

Over-exertion in lifting objects

This incident became a recordable on 10/01/2024 when the employee was placed on restrictions. The employee and a partner were removing the guard on 111-10-4a cobber to repair the sprocket and chain. While removing one side of the guard, the employee felt pain on the inside of left elbow below the Bicep. The weight of the guard is approximately 80 lbs.

Accident type, without injuries

Two MTMs were stuck in the Step 3 Man Elevator, after others exited on 2nd floor. The elevator dropped 6"" when the door closed & stopped functioning. The 2 were stuck in the elevator from 6:25 am to about 7:14 am. The fire department was called, power to the elevator shut off and the door was manually opened allowing the 2 to exit safely. MSHA was promptly notified of the event.

Employee was jack hammering a plugged surge bin when they felt a piece of hot material hit their eye. The employee contacted their supervisor and EMS was called to the scene.

Struck by falling object

While dumping a rod box a 14 1/8"" long by 1 1/4"" diameter rod weighing 4.4 pounds flipped out of the dump truck box, striking the employee on the left hand. The employee's hand was holding onto the forklift steering wheel at the time of the incident. This incident became a recordable on 7/23/2024 when the employee was placed on restrictions.

EE was working to unclog Biomass Line 3 near the waste bin on east side of Agg. Step 2. EE stepped back on to debris that had fallen over the retaining wall and fell.

Fall onto or against objects

The EE was working from a 5' high scaffold deck, EE was tied off overhead with a self-retracting lifeline or SRL. While working, EE turned and stepped off of the deck, the SRL stopped the EE, and EE's right shoulder struck a steel column that was directly next to the scaffolding. Resulting in a dislocated shoulder. EE returned to work with temporary restrictions.

Struck against stationary object

An Ironworker was using an adjustable wrench to turn castle nuts on the ends of traveling grate rods for the purpose of installing cotter pins. The wrench they were using slipped off one of the nuts and the palm of left hand struck a cotter pin on a nearby grate rod resulting in a laceration of the EE's palm and requiring sutures to close the wound.

The employee had been working from a single high scaffolding platform. EE was coming down off of the scaffolding by way of an attached ladder. As EE stepped off the ladder onto the walkway below, EE rolled left ankle. This resulted in a closed fracture of the employee's left ankle.

An ironworker was working on a door frame for the pre-heat. EE lifted a 6 foot long section of 5 inch channel iron that weighs 54 pounds. The employee felt a pain in EE's abdomen and was found to have an umbilical hernia.

Systems Repair was walking into the 5-7 dust collector room at Step 2 Agglomerator, the door blew shut on the employees left middle finger. Employee was seen at the dispensary and VRMC.

The injured employee and a partner were assigned to change out a troughing roller on the 045 belt. The injured employee was changing out a wing can on a troughing roller and putting in a new wing can. During this process, the wing can dislodged after being stuck, and smashed the tip of the employee's middle right finger.

The employees were installing 4 pieces of packing material using a packing pusher. While attempting to insert the fourth piece, the employee used a pry bar to push the packing in place. The pry bar broke free causing the employee to step back. EE's left ankle became caught in the floor grating causing EE to continue to fall backwards, onto the ground.

While removing scaffolding from the Line #3 feed cone an employee sustained an injury when a scaffold pan (plank) that was being handed to EE dropped and struck the injured employee on the back of the right hand.

When an employee went to pick up washdown hose, it swung around and struck EE in the right cheek. THIS INCIDENT BECAME A RECORDABLE WHEN THE EMPLOYEE HAD A MEDICAL PROCEDURE TO REMOVE METAL PARTICLES FROM THEIR RIGHT CHEEK ON 8/27/24.

Over-exertion (Not Elsewhere Classified)

Employee was coming down the stairs from the line 3 multi-clones. EE was finished checking the dust valves and heading to the main driveway in the basement. EE walked down the west side stairway towards the waste gas fan, then down onto the concrete landing where EE rolled EE's right ankle.

Struck by falling object

4 employees were removing liners inside a rod mill. The employees were using lining bars to remove liners inside the mill. Employees stated that as they were in the process of removing the second liner from the feed end, the liner to the right of the one they were removing tipped over contacting the employees' right leg. The approximate weight of the liner is 688 lbs.

Over-exertion in pulling or pushing objects

MTM was assisting changing out a motor. As the motor was being lowered to the pallet, from a lifting jig, the MTM was using a no touch tool to tip the jig and motor on its side to remove the jig. As the MTM was pushing with no touch tool, the MTM felt a burning sensation and discomfort in the groin area.

Fall from ladders

Employee was descending a step ladder when EE's left foot slipped from the second rung from the bottom of the ladder. Injured employee slid down the ladder, catching self upright on left foot at ground level, with all the bodyweight on left leg and EE's knee locked. As a result the injured employee sustained an injury to the left knee.

THIS INCIDENT BECAME A RECORDABLE ON 7/6/2023 WHEN THE EMPLOYEE RECEIVED RESTRICTIONS Employee was assisting a co-worker with the removal of a sump 4-inch discharge pipe. Employee kneeled down to operate a pulzall cable winch, as the employee was exerting pressure to stand up they felt discomfort in the left knee.

Over-exertion in pulling or pushing objects

Employee was working on the 3A Fan tightening bearing cap bolts with a 3/4"" drive ratchet and was using a pulling motion. Later in the shift the employee felt pain in their right shoulder. ** Additional information - A 1/2 inch adaptor was used with the ratchet to tighten bolts. This adaptor failed while the injured was tightening bolts. This occurred twice.**.

THIS INCIDENT BECAME A RECORDABLE ON 06-02-2023 WHEN THE EMPLOYEE STARTED PHYSICAL THERAPY. Employee slipped on snow covered ice and fell while between 2 cars in the parking lot. The employee hit the ground on backside and experienced pain the right shoulder.

At approximately 7:45 pm, an employee was in an elevator that came to a quick stop above the balling floor. The elevator repair person was called and was enroute. There were several unsuccessful attempts to reset the system. The power supply was replaced and the attendant exited the elevator at 8:57 pm. Safety Dept. was notified at 8:36 pm and MSHA was notified at 9:11 pm.

While replacing liners in a pump, the employee was using a 5' pry bar. The pry bar slipped causing the employee's right thumb to be pinch between the lining bar and the pump. This caused a laceration.

Over-exertion (Not Elsewhere Classified)

The EE was carrying a 4' x 4' piece of plywood to cover beams that had been painted when EE tripped and in EE's words ""rolled ankle"". EE fractured the 5th metatarsal bone of right foot. The injury did not involve mine equipment or system, job skills, proficiency, training, attitude or any protective items related to clothing or protective devices on equipment.

Struck by flying object

Employee was changing out a can on a conveyor belt. The employee was using a chain jack to lift the belt up to get access. The chain jack was hooked to an I-beam on each side. When lifting the belt the jack slipped off and struck the employee just above the right eye.

Employee was notified that water was backing up at a sump. The employee opened the dump valves to stop the water. When walking away, the employee's right leg stepped into an open launder that was covered with water. The employee attempted to grab a wall pipe with their left arm which caused a dislocation of the shoulder.

MTMs were changing a liner with the cover plate on the floor for the sump. Injured MTM had their left hand on the case while using the the right hand to pry the liner and throat bushing into place. The case cover shifted pinching the employee's left ring finger between the sharp edge of the case and curb of the sump causing a break on the left ring finger and sutures.

The employee was assisting a fellow utility tech splitting pellets when EE heard a pop in the right knee, accompanied by pain. The employee iced their knee about 1 hour with no relief.

Closing a clam guard on a pump the guard caught the employee's left thumb as it came down. The employee's left thumb was caught in/between causing a fracture.

Over-exertion in lifting objects

Employee was picking up metal floor grating to install in a floor launder. Employee removed BO grating from launder and had the new grating standing up against the sump pump. As they picked up the new floor grating and started walking (2-3 steps) to the area to set into place, the employee felt a pull in their groin area.

At 2:45pm on 04/26/2022 operator noticed smoke emanating from sawdust trailer. Trailer was picked up at supply site 04/26/2022 and delivered to landing yard at mine site at approx. 9:30am. Trailer cut to access fire point. Damage to trailer and total loss of material. Total damage approx. $4500.

Employee was chipping ice/material buildup in the dribble hopper with a pry bar. While chipping the employee's hand made contact on top of dribble hopper causing a laceration to left index finger.

Employee was adjusting cutter bar teeth on a balling drum. While working the the balling drum, the employee stepped into the opening of the scroll and fell, twisting their left knee.

Accident type, without injuries

Two employees were trapped in the Agglomerator 1-2 elevator. The elevator company was notified and responded. MSHA was called at 12:15pm. The employs were removed from the elevator at 12:42pm.

Over-exertion in pulling or pushing objects

MTM was opening the main housing door on Line 6 Waste Gas Fan with another MTM. While opening the door, the door got stuck once it opened about 3 inches. They were applying pressure to the door and the door swung down and MTM tried catching the door. After this happened, MTM felt pain in lower back.

MTM was working on the lime slurry mixing tank agitator. When descending the ladder, EE's foot slipped and EE put most of weight on EE's left arm and felt a pop in the bicep.

A small fire occurred at the Reclaim Pumphouse. The fire occurred outside the building when insulation started on fire near some electrical connections between the building and a transformer. The electrical department was able to shut off power allowing the fire department to extinguish the fire. MSHA was notified at 6:02 am

Two Lakehead employees were trapped in the west, Step 1, 2 Agglomerator elevator. It was first noted that the contractors were in the elevator at 10:20am. After an unsuccessful attempt to reset the elevator, the elevator repair company was contacted and the two contract employees were freed at 11:03am

An employee was trapped in the Step 2 AGG man elevator. Employee was removed from the elevator at 11:59am. Employee was in the elevator approx. 32 mins.

Tractor/Trailer Extreme cold led to landing gear sticking. EE attempted to use pry bar to turn landing gear handle. Pry bar slipped and handle struck EE in left orbital, causing cut/swollen eye, and broken bone.

Employee was attempting to clean in the line 8 basement south of the 147 pumps by the 23-03-1 belt. There was a piece of classifier skirting employee was trying to move about 10 feet. When the employee was pulling the skirting, they slipped and fell up against the 120-08-1 feed end wall injuring left shoulder.

2020 · 11 incidents

Struck by flying object

Employee was taking a pellet sample in the step II Agglom from line 4. While taking the sample a piece of slag hit the scoop knocking it out of employee's hand with the handle striking employee in the chin. Employee was sent to telemedicine and returned to work without restrictions but does state that employee has 2 broken teeth on the upper left side in the back.

Struck against stationary object

Injured EE was attaching a lifting device to a forklift mast. As they were threading the safety chain around the mast, right index finger was lacerated on a burr on the mast. EE had removed leather glove to facilitate access to thread the chain around the mast.

Over-exertion in lifting objects

Filter attendant was blowing down the filter chutes with a 6' air lance. As EE was finishing with the 208-7-5, felt a sharp pain in left upper bicep. EE reported the incident approx. an hour after EE felt the pain.

Employee was adjusting guard on the 026-02-1 when they felt a pop in the right shoulder. This incident became a recordable when they had surgery on January 26, 2021.

Caught in, under or between a moving and a stationary object

EE was calling for plant operator to lower overhead crane hook so EE could attach rigging to remove temporary steel support on the Concentrator Section 7 launderer. The operator did not see the signal to stop and as the hook lowered the EE's left index finger was caught between the hook and adjacent structural steel resulting in a fracture to the distal end of the finger.

Over-exertion in lifting objects

While attempting to relocate a 10-inch 90-degree elbow, the employee felt pain in EE's back. The employee bent EE's knees and put EE's arm through the elbow and started to lift the pipe. The pain started when the pipe was about 3 inches off the ground. Following the incident, the elbow was weighed and found to be approximately 114 lbs

Over-exertion in lifting objects

Employee was working to replace a pipe elbow. While positioning the coupling, employee experienced pain the shoulder while making adjustments to the coupling to be installed. It was not specified at the time of this report as to which shoulder.

A shipment of dry sawdust used for fuel was delivered to the site on 7-11-2020 at 4 PM. The load is delivered in a 53 foot semi-van container and is left in the container for distribution to the process at a different time. Later that evening at 11:00 PM a Minntac employee noticed smoke coming out of the van. Minntac employees put out the fire quickly and put people on watch.

Employee became trapped in the Step 2 Aggl. man elevator at 1:30 pm. The control room was contacted and safety was notified. The elevator repair company responded and got the employee out around 2:40 pm. Due to the entrapment being over 30 minutes, MSHA was immediately contacted by the on-call safety person. The elevator was put back in service.

Employee was installing a guard on the west side of a classifier when EE's right ring finger was pinched between the guard and the lip of the classifier tank.

Struck by falling object

Employee was beginning to move a green two wheeled recycling container. When they tipped the container back to move it, the left wheel fell off and the container fell on top of the employee. Another employee assisted getting the container upright. This became a recordable when prescription medication was prescribed on 1/14/20.

Accident type, without injuries

Employee was trapped in the Step 3 freight elevator at the concentrator around 9:45pm. MSHA was notified at 10:11pm. The elevator company responded and the employee was freed at approximately 11:20pm. The elevator was put back in service.

Accident type, without injuries

Safety Dept. on-call person received a call @ 1:50 pm reporting that a manager was trapped in the Conc. Step 3 elevator. MEI was contacted & they stated they would send someone out to repair the elevator. Safety Dept. notified MSHA @ 2pm to report the entrapment and again at 4:30 when employee was rescued.

Caught in, under or between a moving and a stationary object

EE was working with crew raising section of cooler water trough from floor to work stands. As the section was being lifted the injured EE was holding the end of the trough when the load shifted resulting in left hand being pinched between the trough and the cooler wall. Laceration to right palm.

Contact with hot objects or substances

EE was lighting a oxy/acetylene torch when the gas hose split resulting in a flash fire that resulted in second degree burn to EE's right wrist and lower abdomen. Anti-flashback valves were in place on the torch handle.

Caught in, under or between running or meshing objects

2 EEs were setting up to move pipe off the deck of a Broderson to a pipe stand. MTM 1(operator) was positioning the outriggers on the left side of the equipment. MTM 2(the injured)was on the front side to rig the pipe from the deck. MTM 1 heard MTM 2 yell as the left front outrigger came in contact with their left foot. MTM1 retracted the outrigger and notified the supervisor.

The injured employee exited a service truck and was walking across the parking lot to the building. Employee slipped on compressed snow and ice landing on the right shoulder. This became a recordable case on 2/22/19 when the doctor put EE on restrictions.

Over-exertion in pulling or pushing objects

3 MTMs were helping Door Service Inc. employee prep garage door for installation of a tracking bar. The crew attempted to unroll the overhead door material but found the door was too heavy to be unrolled. The injured employee heard three pops in their right arm while unrolling the material. This became a recordable case on 2/21/19 when we were notified employee needed surgery.

Safety Dept received a call @ 12:32am from manager that an employee was stuck in elevator. Several attempts failed to reset the elevator. MEI was contacted & they stated they would be there in 1 hour. Manager notified Safety who called MSHA at 12:39 reporting the incident. MSHA operator was told that the manager was in contact with the employee by radio and phone.

Struck by flying object

The employee and 2 others were replacing a 1.5"" ball valve. Once completed, the valve was energized & the employee tried to open the valve. The entire pipe assembly started rotating. The valve was isolated & the employee tried to tighten the assembly by hand. The valve was re-energized & the employee was struck by the assembly rotating off the header pipe extension.

A Lab Tech, was exiting truck at the Step 2 Agglomerator at line 5 and slipped on ice striking left elbow and felt pain in left shoulder. After telemedicine exam, ee returned to work without restrictions. Incident became a recordable on 2/20/19 when we were notified the employee needed Surgery

Over-exertion in lifting objects

The injured employee and 2 other MTMs were working on a belt change out. The injured employee was moving the 28"" x 56"" 150lb platen into place. When lifting one end of the platen, the employee felt a burning sensation in lower back. Employee reported the incident to the supervisor, and was sent to the dispensary for evaluation. This incident became restricted on 12/11/18.

Over-exertion in lifting objects

The injured employee was working with 2 other employees to remove the lower 72"" platen from the 003-01-1 belt. While removing the platen, the employee felt pain in upper right shoulder. The platen weighs approx. 200 lbs. Employee became restricted on December 6, 2018

Injured was using 12lb sledgehammer to install concrete anchor bolts in mill sole plate. While swinging hammer ee struck an adjacent coupling extending into the swing path pinching the tip of right index finger between the coupling and the hammer handle resulting in laceration.

Employee was given restrictions on 11/5/18 making this an MSHA reportable. Employee felt discomfort in the left knee while turning body and lost balance. When falling, employee struck the lower part of the left thumb on a grinding ball. Employee reported both knee and thumb pain.

Employee was in the Step 3 elevator when it stopped unexpectedly, trapping person in the elevator. Employee used the phone to call for help. MEI Elevator was called. MSHA was notified of the incident in a timely manner. MSHA returned a call asking to be notified when person was out of elevator. Notified MSHA that person was released at 12:12pm. MSHA returned second call @ 12:32 pm

MTMS were threading impeller onto the barrel shaft of a pump. Injured was turning the driven sheave while the other MTM lined the impeller. The impeller started to thread onto the shaft. The injured employee tried to assist by pulling on the v-belts at the drive (motor) sheave resulting in right hand getting caught in the pinch point between the v-belts and drive sheave.

Employee was installing the motor for the 206-07-1 pump. As employee was driving in the key for the sheave the hammer missed the key and hit the left thumb. Employee's thumb was pinched between the hammer and motor shaft. Employee was using another piece of square stock to drive the key in.

Employee crawled into the duct of Line 3 scrubber to remove a baffle plate. EE gloves got muddy while crawling into the duct. EE removed right glove to get a wrench and flashlight from pants pocket. While repositioning self ee hand struck one of the baffle plates causing a laceration on one finger.

Employee was pulling with a 36"" pipe wrench on a pump shaft. The wrench slipped off and employee fell backward. Employee put out right hand and caught self from hitting the floor. After that, employee felt pain in right tricep.

While adjusting a belt being installed, employee's right middle finger was injured as coworkers spun the sheave. Finger was caught between the belt and sheave.

Aggl 3 CR Op. experienced shortness of breath, chest pains, sweating & heart palpitations at 5:45pm. Employee called emergency assistance & was sent immediately to local hospital by ambulance. Examination by medical staff ruled out heart related event and employee was sent for additional tests. Medical professionals determined the event was not work related & employee is doing ok.

Struck by falling object

After completing several tasks, employee noticed a foreign body in eye when washing up. Employee tried to remove the object with a tissue. An hour later, employee attempted to flush the material out. When that didn't work, employee reported the condition and sought medical attention.

The employee was working on the 134-17-1 hydrant. While changing out components the employee manually lifted and positioned the hydrant assembly. After completion of the job the employee noticed minor pain in the right groin area.

Employee was in the process of repairing the 027-07-2 belt. Employee was transporting the cooker platen from the belt to the cart. While employee was lowering the cooker platen to the cart, felt a pop in right shoulder. The employee was given restrictions on 7-10-17.

The employee was dressing the edge of 1/4"" thick diamond plate on a stationary grinder. Employee was grinding the 3 1/2"" wide end holding the piece with both hands toward the top of the material. While grinding the piece of material employee's hand was pulled into the grinding wheel causing a laceration on the top of the left hand.

Employee was completing a lube route and noticed a BO lube line on the Line 5 Ball Mill in the Step II Agglomerator. Employee climbed the ladder to remove the BO lube line. Employee then proceeded to fabricate a new line and climbed the ladder again to change it out. While climbing the ladder, employee felt a pain in lower left back.

The employee was repairing a guard and was in the process of cutting the metal to its final dimensions. While using a grinder to make the cut, the employee held the expanded metal down with right hand while operating the grinder with the left. As EE was doing this, the grinder contacted gloved right hand causing a laceration on the base of right index finger.

Employee was hauling material from line 5 chunk box to group 13 dump site using a dump truck. After crossing the railroad tracks, the loaded truck was heading east when the employee lost control of the truck. As the employee attempted to regain control of the truck, the truck turned over onto its side injuring the employee inside.

The employee was lifting side castings up and handing them to another employee. During one lift, employee turned to the right, while holding the side castings, and felt pain in right knee.

Employee was performing hotwork on the preheat damper blade. Employee noticed irritation to EE's right eye on 4/13/16 and reported the irritation on 4/14/16. Foreign body was removed from right eye on 4/14/16.

Injured was using a flexible lance to clean a 3"" pipe in the agglomerator. As EE was retracting the lance from the pipe, the hose uncoiled, causing the nozzle to slip out of the pipe quickly. A high pressure stream of water contacted the operator's leg, resulting in a 5"" long laceration which required cleaning & sutures. EE returned to duty the next day with no restrictions.

Employee moved a 96lb portable welding machine 6'. Employee had a strain to his abdomen while setting portable welder down.

EE was between the vertical pallet and adjacent horizontal pallet. The push bar dislodged and fell approx 4 feet striking EE in the groin. The end of his penis was pinched between the push bar and the edge of the pallet.

Life-saving efforts were immediately started by responding employees and sustained by plant and local EMS upon arrival but were unsuccessful.

Employee was performing structural inspection when dust entered left eye through gap between face and side shield of safety glasses. Physician at clinic irrigated left eye to remove particles and then applied antibiotic corticosteroid ointment (TobraDex) as a precaution. No damage to eye was noted. Employee was released to return to work.

Elevator Entrapment In 10:38 a.m. -- Out 12:24 a.m. - Employee was stuck in the Concentrator Step 3 freight elevator @ 10:38 a.m. for 106 minutes. MSHA was called at 10:58 a.m. Electronic voltage controller lost power - did not see voltage so the motor timed out, most likely due to dirty contacts. Vendor keeps contacts clean.

Concentrator freight elevator - the plantat fit tightly between the manual inside doors so the south door had to be pushed out slightly to close. When the elevator was going past the 4th floor level, the south inside door caught on the outside door causing the inside door to be knocked out of the track.

At 7:30 ee started balancing 298-04-3 grate discharge dust collector near kiln. The ee began to experience a headache, nausea and some disorientation around 8:00am. The temperature in the area was around 140F. They took a break at 8:30am and ee went to the office to cool off. After 1/2 hr the ee decided to seek medical treatment and received an IV treatment at Med Ctr

Injured was using a fire hose to wash down deck in the Fines Crusher Turn Bin 5-03 conveyor gallery, while pulling on hose he lost his footing and fell backward. He extended his arm to break his fall and the arm took the force of the fall resulting in a simple fracture to his right forearm (radius bone). Concrete floor was sloped and wet but clean and EE was wearing rubber boots.

Employee was pushing a piece of ductwork on a 4-wheeled cart and pinched his right index finger between the flange of the ductwork and a steel guard protecting a start/stop switch.

While rotating a stripper rod, the rod dropped 3"", resulting in a crushing amputation of the tip of the employee's right pinky finger.

Employee lost his balance while burning bolts on a grate pressure bar and became impaled on a grate bed depth finger. The incident was reported at 8:09 a.m. to USS Management, at 8:35 a.m. to Dave Wallin with MSHA, and at 8:54 a.m. to the MSHA Emergency number by Laurie Potter. The employee was removed from the grate at 8:40 a.m. and from the property at 8:50 a.m.

Struck by falling object

Injured EE was installing nihard liner panels in pan feeder. He had placed a 9""x2""x18"" liner & left it unsecured as he attempted to remove material from under the liner below to provide add'l room for the unsecured liner to fit properly. While doing this the unsecured liner became displaced, fell striking his left hand. Resulted in a fracture to L index finger.

Struck by falling object

EE was struck by 5 ft steel prybar while walking down stairway from cooler to basement. The pry bar was dislodged when kicked by another employee who lost footing while using another prybar to position a section of moveable cooler wall. This resulted in the bar leaving the cooler, clearing the external walkway & striking the injured EE on left shoulder as he descended the stairway.

Vacuum hose was plugged and ee was feeling hose for vibration so he knew where the plug was. He couldn't tell without taking off his gloves. When he found the plug he cut the hose and also cut the tip of his finger.
