Miner was positioning a bearing housing in the hydraulic press. The bearing housing slipped on the frame, causing the housing to fall partially off the frame, pinning EE's fingers between the housing and the frame. EE went to urgent care clinic, was referred to the ER where EE received sutures. Returned to work the same day on light duty, and released to full duty on 5/6/16

Showing all 3Miner was hooking a lifting strap to lift a drill steel out of the drill. Steel stuck in the bit, causing the eye bolt in the drill head to fail. Miner was struck by falling shackle. Miner refused treatment, wanted to see how it felt the next morning. On 3/8, miner sought treatment, diagnosed with a fracture of the right radius. Miner returned 3/8 on light duty.
Employee (Ee) was greasing the loader at the start of shift. Ee reported he ducked under the boom arms and felt a pop in his lower back. Ee said he'd like to see how it felt in a few days before he went to clinic. Ee went to clinic 11/05/15, diagnosed with lower back strain. Physical therapy ordered w/ no restrictions. Released from care 11/17/15
