EE was hoisting piece of metal onto saw horses with 1840 Skid steer loader & the metal slipped & struck EE in right knee while sitting in the operator's seat. Informed me at 3:00 pm that day but refused treatment. The next morning, Friday, informed me it was still sore & was sent to doctor & placed on restricted duty with follow up visit scheduled.

Annual audiogram indicated a reportable hearing loss in right ear when compared to baseline audiogram from January 2006. Sent to specialist for retest and evaluation on 4/2/18

Miner was positioning a bearing housing in the hydraulic press. The bearing housing slipped on the frame, causing the housing to fall partially off the frame, pinning EE's fingers between the housing and the frame. EE went to urgent care clinic, was referred to the ER where EE received sutures. Returned to work the same day on light duty, and released to full duty on 5/6/16
Not an accident. An employee has had an audiogram when compared to baseline audiogram falls under the category of Reportable Hearing Loss. This is the purpose for this report.
Miner was hooking a lifting strap to lift a drill steel out of the drill. Steel stuck in the bit, causing the eye bolt in the drill head to fail. Miner was struck by falling shackle. Miner refused treatment, wanted to see how it felt the next morning. On 3/8, miner sought treatment, diagnosed with a fracture of the right radius. Miner returned 3/8 on light duty.

Employee (Ee) was greasing the loader at the start of shift. Ee reported he ducked under the boom arms and felt a pop in his lower back. Ee said he'd like to see how it felt in a few days before he went to clinic. Ee went to clinic 11/05/15, diagnosed with lower back strain. Physical therapy ordered w/ no restrictions. Released from care 11/17/15
Ee slipped while walking on an elevated walkway checking conveyors. Ee grabbed handrail, straining right shoulder. Ee reported injury to foreman on 7/9/15, but did not request treatment/evaluation. 7/14/15 Ee received evaluation of injury. Dr. diagnosed sprain of right shoulder, returned to full duty same day (7/14/15), & ordered physical therapy.

Employee was using sledge hammer to move slabs of rock that had jammed a conveyor. The employee hit himself in the shin with the sledge hammer. It was not a direct hit but slid back from rock being hit into shin. The accident resulted in bruising and a cut that required 6 stitches.

Employee was cleaning frozen mud from the pulley on main belt conveyor using a round bar 5' to 6' long. Right small finger pulled in between bar and conveyor belt which pulled part of the skin off the finger as he pulled out his finger. He received 8 stitches from a specialist and returned to work.

EE STATED HE WAS ON TOP OF THE PLATFORM HOSING DOWN OIL ON HIS MACHINE. HE STARTED BACK DOWN THE STAIRS, & SLIPPED. THERE WAS EITHER OIL ON HIS BOOT OR HE SLIPPED FROM THE WATER. HE PUSHED HI MSELF AWAY TO PREVENT HITTING HIS BACK OR HEAD AS HE FELL AND LANDED ON HIS LEG.
