While testing the Stowage of the jib on the boom, it fell off the locking mechanism dropping two feet to the top of the Stabilizer guard. It hit employee on the shoulder bruising it, The sheave mount scratched him on the back, strained his shoulder muscles. The Jib pin locking was not properly installed or double checked. There was no failure of equipment.
While manually loading bricks in the bucket of a bobcat with 3 other employees, a brick being tossed into the bucket hit employees right hand and smashed his index finger against the brick he was tossing. The impact broke his right index finger. Employee was wearing gloves.

A staging platform to open/close truck lids for loading cement powder had a step on the loading dock of the platform which needed resecuring. One step was lifted to expose the repair area of the step needing repair. While working, the lifted step fell and hit the left hand pinky finger of employee.
After removing 7DC8 Dust collector roof hatch using proper lifting techniques, EE stood up and felt a pinch in his lower back which continued to his left side. He refused to go to the doctor and worked to the end of his shift. The next day he had increased pain and then requested to go for medical attention.

On December 8, 2009 EE reported to his supervisor that his Rt knee was bothering him when he was kneeling to change dust collector bags, He refused to go to the doctor at that time (Not Reportable). On 4/8/2010 He requested to go to the doctor to get the knee checked saying it still bothered him and He was put on restricted duty (Now Reportable).

Assisting coworker lifting and sliding sideways a 250 lb pipe onto steel table, pipe coated with nonslip material creating more resistance, Employee felt sharp pain in lower back. Employees should have used mechanical lift but were in "hurry" was the reason for not following their training and company policy on proper lifting.
Employee experienced a sharp pain in his left shoulder on about Jul.2,2008 while installing conduit at the Roll Press and again on Aug. 29 while performing similar work at the Blend Silos. The pain continued to worsen and it started impairing usage of his arm. On Sep.28 he reported to a supervisor his condition and arrangements were made to get medical advice.
