While repairing the safety Latch that holds trailers in place at the dock bay the shock came off unexpectedly causing it to hit EE's right index finger and thumb, resulting in a cut that needed stitches.

Employee informed EE's Supervisor that EE would be moving slowly while working. The Supervisor asked why EE would be moving slowly and EE told EE's Supervisor EE bent over to lift a box from the floor and felt a pop in EE's lower back and that EE's right leg went numb. When asked if EE needed medical attention the employee stated no, EE would be fine.

The EE was working on a case packer unit. Cardboard boxes had become jammed together. The EE locked out the machine and released the stored energy, reached in with hands to take out jammed boxes while pulling on a box his left hand slipped off of the box flap and struck a jug guide flap. This caused a laceration to middle finger on left hand.

The employee was placing a roll of stretch film on a stretch wrap machine. The stretch wrapper arm was not in the correct loading position and he reached out while holding the roll of film (38.4) lbs). He said that he twisted his upper body while placing the film. THE EMPLOYEE AND ANOTHER EMPLOYEE WERE ADDING A 4' X 10' SECTION OF THE CONVEYOR TO AN EXISTING CONVEYOR. EMPLOYEE WAS TRYING TO LINE UP THE BOLT HOLES. HE GRABBED THE CONVEYOR AND ATTEMPTED TO MOVE IT. HE STRAINED HIS LOWER BACK WHILE PULLING ON THE CONVEYOR.
