Operator was emptying a hydrated lime bin. When EE dumped the bin, the material covered EE's feet and up EE's legs. The operator turned to get out of the material and fell face first in a pile of hydrated lime. The hydrated lime mashed around and under EE's safety glasses going into EE's eyes.

Maintenance employee was welding an inspection port in mill room 1 on feed chute when a piece of metal or slag came off the weld and down into EE's welding hood and underneath EE's glasses.

Employee was cleaning the bottom of a chute for solid fuel. Employee was using a 6' long hand held jack hammer bit to break the material free. When the material came loose it drug the jack hammer bit down contacting a belt conveyor. The bit was jerked out of the employees hands and made contact with the side of EE's head, causing a small cut on the left middle finger.

Employee was training with the oiler moving barrels when EE mashed hand between two barrels. EE was taken to the doctor for diagnostics. X-rays were negative. EE had a contusion on left hand. EE was placed on light duty until 9/28/2017. Employee worked through 9/22/17 and then never returned.

Employee was finishing up last shift. EE was passing bricks to another crew that was at the same job location. They were working along with another crew to finish the rim on the kiln. The other crew was not attaching the bricks correctly and the bricks fell from the rim and some bricks landed on Employee's left finger and it broke it.

Checking cooler grates for #3 kiln by opening access door at the same moment a puffing phenomenon related to fuel mix occurred, blowing fine dust under low pressure out this opening some of the dust got around glasses and irritated eyes ee reported this at about 11:00 AM no lost time or restrictions returned to work same day with sulfacetamide 10% Ophthalmic solution.

Cleaning out screw conveyor by pushing material down an opening when pushing too hard resulted in sharp shooter shovel penetrating too deep and employee jerked back on the handle causing the right little finger to strike the flight resulting in a bruise relieved by drilling a tiny hole in the nail to relieve pressure and three days light duty no lost time.

saddle swung off pallet smashing hand, ring finger swelled...reported to superintendent who ask if he was injured and EE replied not now...there was no further discussion regarding finger until Monday...x-rays taken Monday 07/28 revealed no fracture but osteoarthritis...the nurse practitioner placed index and ring finger in a splint and place EE on restricted duty until 08/06

flat steel on forklift needed to be moved outward from the vertical portion of the forks...another worker moved his side first...this motion included a slight counter clockwise rotation...opposite side also moved with the rear corner sandwiching right index finger against the vertical portion of the fork...laceration to the right upper index finger...(4) stitches

Employee was bagging 10 lb bags of hydrate when bag burst. Employee was wearing safety glasses, but some hydrate entered area behind glasses. Result was irritation to both eyes with the right side the worst. On site washing using water and eye wash plus several daily trips to eye doctor resolved the situation ending with two days restricted duty, but no lost time.

EE did not report this to co. officials until he returned from vacation, approx. 1 week after possible occur.It is still unclear if this occurred during perf. of the EE's duties while on co. property. No Witness, W.C. will issue letter of denial.

#3 kiln burner fan blade inspection.. raised safety glasses to see.. blew(by mouth) dust from one blade looking for cracks.. most of the dust particles dispersed, but a few particles made their way under the eye lid.. .we were unable to completely wash out.. irritation and scratching resulted in doctor cleaning twice and a anti-biotic. .no lost time or restrictions.. already healed

Employee called out to splice broken conveyor belt...belt broken on return leg...belt must be pulled through narrow confines of frame near ground level negating use of regular clamping equipment...large C-clamp used to pull belt became caught in conveyor frame and employee dislodged by hand...freed clamp struck web of left hand resulting in fracture to left thumb.

EE was on his back looking up while performing maintenance on a maintenance truck. Debris fell onto his face and some got behind his safety glasses. He immediately washed his eyes out. A spot warranted a trip to the eye doctor - revealing a small piece of metal.

Two employees were setting a newly fabricated lid for unit II mill feed screw...this lid was fabricated from .5 inch stock...One employee dropprd his end first and the impact caused the bottom piece to bounce upward and strike the other employee's left hand area resulting in a hairline fracture to the tip of the left middle finger...no lost time or light duty.

Scale attendent loading railcars...on top of car to open access doors(hatches)...properly working seals resulted in some resistance to opening...attendent (admitted being aggrevated) jerked up on one door and felt a pull in his lower back later diagnosed as a pulled muscle...attendent missed Friday, so a lost time accident...returned Monday on light duty.

Employee was bagging Hydrated lime in 50 pound bags using a machine manufactured for this purpose...he was removing a filled bag when it split spewing hydrate onto his face...some got under his left eyelid irritating it...today, he made his third trip to the Company Doctor before the doctor pronounced him totally cured...he lost NO time and his left eye was NEVER affected.

Employee traveled from the crusher to the lunchroom area as he does every day. After getting out of the pickup he closed the door. His foot slipped/slid or something and he lost his balance. He had his right hand on the top of the cab near the door as it shut. His right ring finger was mashed between the cab and the closing door. A trip to the ER resulted in 5 stitches.

Ee ended day with small metal fragments in left eye. They didn't bother him & he didn't report this information at work. Irritation from the fragments started causing concern later that night & he went to the Calico Rock Hospital ER. This was going to be a 1 day lost time accident except he had a reaction to the medicine requiring a medicine change.

INJURED AND CO-WORKER WERE REMOVING DRIVE GUARD FROM SECONDARY CRUSHER IN OLD QUICKLIME SYSTEM. AS THEY WERE RAISING GUARD IT CAUGHT ON SOMETHING AND THE JARRING ACTION RESULTED IN MUSCLE SPASMS IN LOWER BACK, A TRIP TO THE DOCTOR, ANTI-INFLAMATORY AND PAIN MEDS AND A THREE DAY LOST TIME ACCIDENT. EMPLOYEE RETURNED TO WORK 7/19/04 WITH NO RESTRICTIONS.

EE RECENTLY BID INTO A JOB INVOLVING CLEANUP. HE WAS CLEANING SPILLAGE OFF DECK OF CONE CRUSHER USING A SHOVEL. AFTER THE TASK HE REPORTED THAT HIS LEFT ELBOW HURT, THE SAME ELBOW INJURED SEPT 2002. DOCTOR TOOK HIM OFF WORK TO LET DRUGS ELIMINATE INFLAMATION. NO SKELETAL INJURY. EMPLOYEE SAID HE COULD CONTINUE HIS JOB EXCEPT CLEANUP. (LOST TIME UNTIL DOCTOR VISIT ON 3/10)

EE HAD COUPLED RR CAR AIR HOSES TOGETHER IN PREPARATION TO SWITCHING OUT CARS. HE THEN TURNED OPEN THE AIR VALVE TO CHARGE AIR LINES FOR BRAKING. ONE OF THE COUPLINGS FAILED CAUSING THE AIR HO SES TO SEPARATE IN VIOLENT MANNER. ONE HOSE STRUCK HIM ON THE LEFT ELBOW AND HIS BEEN DIAGNOSED AS A SPRAIN. EMPLOYEE WAS HELPING MAINTENANCE MAN REMOVE THE DRIVE GUARD FROM THE QUICKLIME SECONDARY CRUSHER AT OUR OLD PLANT SITE. THIS IS A LARGE AND HEAVY GUARD. THE UNBOLTED GUARD SLIPPED FROM ITS MOU NTING BRACKET FALLING ONTO EMPLOYEE'S FINGER. THIS RESULTED IN A FRACTURE OF THE TIP OF THE FINGER AND A LACERATION REQUIRING 5 STITCHES. EMPLOYEE RETURNED TO WORK AFTER LEAVING HOSPITAL.

HE HASN'T LOST ANY TIME. THIS EE ALSO HAS EXTREME PROBLEMS WITH DIABETES. AN MRI HAS REVELED A POSSIBLE BULGE FROM A DISC. THE PLAN IS TO ATTACH THIS PROBLEM WITH STERIODS & SURGERY IS POSSIBLE

THIS CAUSED SOME PAIN AND NUMBNESS BETWEEN SHO ULDERS AND RIGHT ARM. DOCTORS INCLUDING SPECIALISTS HAVE FOUND NOTHING TO INDICATE NERVE OR SKELETAL DAMAGE.
