EE was unrolling a roll of conveyor belting to splice onto an existing belt. While doing so EE felt something pop in right thumb. MRI showed tear in ligament. EE will have this repaired on 08/16/2024.
EE was pulling on a slide under a silo when it broke free and caused EE to hit thumb on a piece of angle iron.

EE had been working on the dryer kiln and noticed a pain in EE's right wrist at the end of shift. EE was evaluated by a doctor and placed on a temporary 10 pound lifting restriction. EE was unsure of an exact activity that caused the incident.

EE was removing piece of jammed metal from elevator. As the piece of metal was jarred loose, the tip of EE's thumb was pinched between the piece of metal and the elevator casing. EE required stitches thus constituting MSHA reportability.

A mechanic cut finger while changing belt splice. EE was holding the back of the clip with the other hand to apply pressure to the back of the clip, the bolt came free and contacted EE's right middle finger resulting in a laceration, requiring 4 stitches.

8/3/17 Electrician pulling new wires for a baghouse and had left shoulder muscle strain. 8/4/17 employee received post urinalysis testing. 8/7/17 employee was seen by the doctor and released. 8/9/17 Urinalysis results came back and employee return to work. 9/25/17 received physical therapy which resulted in the reportable injury.

A mill worker received dust/debris in both eyes when overloads were tripped on a circuit breaker inside the MS11 conveyor panel. The mine believes this incident did not and does not require reporting pursuant to 30 CFR 50.20.

The employee was installing a metal plate on the jaw crusher. While attempting to install a bolt, the plate shifted pinching the employee's finger between the plate and bolt head. The resulting wound required sutures. There is no lost time associated with this injury.

Employee was cleaning out a chute during a system clean down, the employee reached into a chute to remove material inside the chute and his glove caught the rotating equipment pulling it to a pinch point. The distance of the employee's reach was 12-18 inches. This is not a standard procedure for the clean down.

Employee was setting a new chute section on a screen tower. The chute was rigged to the crane line with a single choker hitch. As the chute was being lowered through a floor opening it got hung up on a flange. Employee tried to straighten the piece and it swung and crushed his left bicep up against the structure.

Employee was holding a piece of steel up to be bolted. When the connection could not be made, employee dopped the piece on the ground. A piece of channel attached to the dropped beam struck employee in the back of the leg. The leg was bruised and scraped. X-rays showed a hairline fracture of the tibia, left leg.
