Two employees were performing patch work on a chute while cutting a existing piece of metal approx 1'x1' approx 3/8" thick off the piece of metal hinged and swung roughly 4' and fell 4' onto the EEs foot. The sharpe corner of the metal penetrated their boot and cut the top of their foot.

4 man crew tearing down scaffold around preheaters. a scaffold upright was caught in grating of catwalk. employee used a ladder to access catwalk, EE's harness D ring caught on railing causing employee to fall forward onto grating. Employee used arms to break EE's fall causing strain to right arm.

Employee was cutting a piece of belting with an oscillating tool. After completing the task they sat it down on the ground. While walking around to roll up belting they accidentally stepped on the battery portion of the tool and it flipped up and cut the back part of their calf. Supervisor took them to the hospital and they received 7 stitches.

EE had changed attachment on skid steer and noticed loose pin. EE attempted to fix the pin by reaching out of the machine. EE accidentally hit the controls and in turn pinched EE's right hand between the lift arm and the frame of the machine causing multiple fractures to the ring finger and thumb of the right hand as well as soft tissue injury.

EE was unrolling a roll of conveyor belting to splice onto an existing belt. While doing so EE felt something pop in right thumb. MRI showed tear in ligament. EE will have this repaired on 08/16/2024.
Sprained right knee when exiting mini-track loader. While exiting employee stepped off of equipment with left foot and slipped on dust particles on the concrete and twisted right knee which remained on equipment.

EE had been arc gouging during shift. When EE got home and laid down for the night EE noticed right eye was tearing and hurting. EE was unable to identify any specific event that led to the injury. EE went to an ophthalmologist where two foreign bodies were removed, EE had a corneal abrasion and was prescribed an antibiotic.

EE had been working on the dryer kiln and noticed a pain in EE's right wrist at the end of shift. EE was evaluated by a doctor and placed on a temporary 10 pound lifting restriction. EE was unsure of an exact activity that caused the incident.

EE was walking in storeroom and tripped on corner of pallet and fell. EE was released back to work with no restrictions that day. EE was still having pain and saw DR on 12/22 and was diagnosed with muscle tear. Surgery was conducted on 12/23 thus constituting MSHA reportability.

EE was entering CAT long reach when EE's right foot slipped. EE felt pain in right ankle when EE's foot hit the ground. EE was diagnosed with partial Achille's tendon tear and has missed work, thus constituting MSHA reportability.
