Employee was closing the rock guard on the front windshield with EE's left hand and had EE's right hand placed on a pinch point. When the guard shut it cut EE's thumb causing a laceration.

Showing all 28Employee went to get a water hose to water the ribs. EE turned the hose on and underestimated the pressure of the hose causing EE to lose grip. The hose whipped around and struck the employee on the head.
Employee was hanging a 480 volt cable with a cab hook. The cable slipped pulling the cab hook off the wire causing the cab hook to come into contact with the employees' right hand causing a laceration.
Employee was checking for a hydraulic leak when the hood came down and struck EE's right hand that was between the hood and the cab.
The employee was plugging in the power cable from the bolter to the power source. There was resistance when EE was trying to plug it in so EE pushed the cable up and felt a pop in the abdomen.
Employee was connecting a 2" drisco line. As he was lifting the line in place to install the clamp, he felt a burning sensation in his left biceps area.
The mechanic was changing flushing head on Bolter 16. While putting the last piece on something caused the inner piece to shoot out. The mechanics finger was caught in between the two pieces. As a result, his finger was cut and so was the tendon. He had to get surgery to fix it.
While driving the pin from a ripper shank, the pin punch splintered resulting in a small laceration to the left thumb.
While unloading supplies he pulled on bundle of rock bolts and felt a pop in his left arm. He had surgery to repair a bicep ligament separation at the elbow.
Removing re-mix lid when it slipped cutting his index and middle finger requiring sutures.

While unplugging a shotcrete hose, cement sprayed in his face. He received medical treatment 7/28/10.
Rock fell while installing vent tube hitting him in the mouth fracturing tooth
Was taking a plate off bottom of haul truck Plate Fell hit his hand on ground cutting finger.
Closed door on loader and cut end of rt index finger off.
While pulling sample tube, tube came to top of rods with overshot intact. Driller stopped pulling tube as soon as ball cage cleared end of rods. Helper grabbed tube with both hands and then driller removed overshot from tube assembly setting it to the side. Helper lifted tube. He attempted to stop tube from slipping down rods, causing his finger to become pinched.
When moving a truck ramp him pulled his arm or shoulder resulting in restricted duty.
While installing a new rod on the Cubex Drill the employee placed his hand on the bottom of the rod. The rod slipped and caught his hand between the two rods. The injury required one suture.
Employee was screwing drill rods together when he felt a pain in his wrist. He was treated and released to restricted duties.
EE was manually re-positioning the forks on a sky trak 8042 fork lift. One fork perched on a frame bracket/divider. The EE dismounted the machine to correctly position the fork, placing his right hand on the fork frame and pushed the hung fork over. The suspened fork impacted his finger on the frame when dislodged
EE was attempting to raise core barrel when he felt a pain in his neck. He visited the doctor and was released to restricted duties.
Attempting to secure the quick couple vic clamp on a 2' pipe with both legs under him for support. Left patella displaced.
Pulling a hose out of the muck pile and strained his back.
While changing a broken fanbelt on the Toro loader, Company #23, the top hose was loosened to relieve the cooling system pressure. When the hose was removed there was enough presure remaining in the system that the coolant sprayed out on employee, burning his arms and chest.
EMPLOYEE WAS TYING IN A ROUND, WHEN HE PULLED ON A NON-ELECTRICAL TUBE TO TIE IN, A SMALL ROCK ROLLED OFF THE FACE AND HIT HIM IN THE TOOTH, CHIPPING IT.
THE BREAKER HAD TRIPPED TO THE HOIST IN THE SHOP. EE LIFTED THE LID ON THE PANEL TO RESET THE BREAKER. HE HAD HIS HAND ON THE SIDE OF THE BOX AND THE DOOR FELL SHUT, CUTTING HIS THUMB.
EE WAS ATTEMPTING TO REMOVE A BROKEN STRIKER BARFROM A ROCK DRILL FLUSHING HEAD BY STRIKING THE FLUSHING HEAD AGAINST THE EDGE OF A SOLID METAL SURFACE. HE WAS HOLDING THE FLUSHING HEAD WITH B OTH HANDS. HE MISJUDGED SWING IT & CAUGHT HIS FINGER UNDER IT. THE FLUSHING IS STEEL & WEIGHTSABOUT 20LBS. RIGHT 5TH FINGER LACERATION/FRACTURE 1ST JOINT. DISLOCATION 2ND JOINT
EMPLOYEE WAS HANGING A POWER CABLE WHILE PLACING THE CABLE HANGER, THE POWER CABLE SLIPPING CAUSING THE CLAMP TO SPRING BACK AND CAUGHT HIS RIGHT INDEX FINGER.
