Showing all 19EE was throwing rocks into the hopper and felt a pop in his left arm. X-rays taken and MRI scheduled but suspect left shoulder strain.
Employee was moving a steel plate to close a set of doors and smashed is left hand between the corners of the steel. He has a small puncture wound that required cleaning our and a butterfly closure. No stitches required.
While setting safety latch on hydraulic lift, the release lever got bumped and smashed his middle finger on his left hand between the latch and hydraulic cylinder on a Case 420 skidsteer, causing an avulsion to the soft tissue.
EE was trying to clear the shoot on the scalper and a rock bounced and smashed his little finger on his left hand, bruising it.
Employee was unscrewing a drill bit from the drill and it unscrewed the entire 18' drill steel from the drill and it fell down on her left hand. Several stitches were required to the lacerations of the last three fingers on her left hand. There were no broken bones but there is still concern that amputation of part of her ring finger may be required.
EMPLOYEE WAS LIFTING AND TWISTING AROUND TO WELD TAB ON CHAIN AND HAS A RIGHT INGUINAL HERNIA. HE DID NOT REPORT THIS TO ANYONE UNTIL 6/21/05. HE WILL UNDERGO SURGERY FOR REPAIR ON 6/23/05.
EE was removing the loader bucket on a 216 cat skidsteer when it fell against his right shin and foot, causing contusion and a chip in the metatarsal and abrasions to his shin.
Employees were re-blocking the cone crusher and the patient caught his fingers between a rairoad tie and the crusher frame. X-rays show no fracture but put on restricted duty until 11-8-04.
EMPLOYEE WAS CLIMBING BACK INTO HIS BULDOZER AND HIS FOOT SLIPPED. HE GRABBED THE DOOR CAUSING IT TO SLAM SHUT ON HIS RIGHT RING FINGER. HE HAD A LACERATION REQUIRING NINE STITCHES AND IT BROKE THE BONE IN HIS FINGER.
EE WAS REPLACING THE 3RD MEMBER ON A CAT 966C LOADER AND STRAINED HIS BACK. HE DID NOT REQUEST TREATMENT UNTIL 6/10/03.
EMPLOYEE WAS REMOVING THE BUCKET AND INSTALLING CLEAN-UP STINGER ON CASE UNILOADER WHEN THE STINGER FELL AND SMASHED THE TOP OF HIS LEFT FOOT. IT FRACTURED THE 3RD METATARSAL ON HIS LEFT FOOT.
EE ALLEGES SHE WAS LIFTING A 50 GALLON DRUM AND INJURED HERSELF. WE WERE NOTIFIED OF THIS INJURY 5/21/03. SHE CLAIMS INJURY TO LOW BACK AND BODY AS A WHOLE. SHE CANNOT STATE AN EXACT DATE OF I NJURY AND NOTHING WAS REPORTED LAST YEAR AT THAT TIME. WE HAVE VERY LITTLE INFORMATION ON THIS CLAIM AND SHE HAS BEEN TERMINATED SINCE 4/3/02.
EE WAS PULLING ON A CONVEYOR BELT AND FELT A PAIN IN HIS GROIN AREA. REFUSED MEDICAL SURGERY IN FUTURE. EE IS PRESENTLY WORKING UDER RESTRICTIONS.
EE WAS CHANGING WIRE SCREEN CLOTH ON THE PLANT AND IT FELL BACK AGAINST HISS KNEE. IT BEGAN HURTING. HE WENT TO THE EMERGENCY ROOM AND WAS DIAGNOSED WITH PRE-PATELLAR BURSAL INFECTION IN LEFT KNEE WITH CELLUTITIS. HE WAS PUT ON ANTIBIOTICS AND TOLD TO STAY HOME FOR ONE WEEK. RELEASED TO RETURN TO WORK ON 7-30-01.

HIS LIP WAS SWOLLEN & LACERATED BUT HE REFUSED TREATMENT ON 6/19/00 HE REPORTED THIS TO THE OFFICE, HIS TEETH HAD NEVER TIGHE ENED UP & THEY ARE TURNING BLACK. HIS DENTIST H AS DETERMINED THAT HIS FRT TEETH WILL NEED TO BEPULLED & A PARTIAL PLATE FITTED
EE WAS CHANGING HAMMERS IN THE IMPACTOR MILL. HETHINKS THAT WHILE WIPING HIS EYE, HE GOT A PIECEOF METAL IN HIS RIGHT EYE
EE WAS ARRANGING BELTING TO PUNCH SLICE HOLES WHEN A ROLL OF BELTING FELL HITTING HIM ON THE BACK. HE SUFFERED MULTIPLE BRUISES.
