Showing all 7Unclear. Employee's statement is that as he was stacking 50-lb. bags at the end of the conveyor belt from No. 3 Bagger he felt a sharp pain in his right wrist. Employee's wrist was not immobilized (cast) with work restrictions until April 10.
Injured employee was stacking 50-lb. bags on a wooden pallet. After laying a bag on a pallet he raised himself back upwards and felt discomfort in his back. The discomfort continued so he was taken for medical examination and diagnosed with a lower back strain. Treating doctor issued work restrictions.
Injured employee was stacking 50-lb. bags on a wooden pallet. He reached down to adjust a bag on the wooden pallet and alleges he felt a burning pain on his Right Side. Treating physician at the Hospital suspected a Hernia; however, Specialist confirmed as Groin Strain on April 26, 2005. Employee has work restrictions and has returned to work on Light Duty.

EMPLOYEE WAS BAGGING TALC, THE BAG SLIPPED AS HE REACHED FOR IT, TWISTING HIS LEFT WRIST. HE DIDN'T TELL US UNTIL 2-24-00. SENT HIM TO THE DOCTOR, THE DOCTOR PUT A WRIST BAND ON IT AND HE RETU RNED TO WORK.
