Showing all 7Employee was working on a 10' 11/2" steel pipe on a table with a vise. The pipe rolled and dropped on the edge of the table crushing employee's left pinky finger. Laceration to left 5th finger.
On Feb 1 employee was installing a roller on a dozer, on Feb 2 EE was changing a battery in a semi. At end of day noticed pain and discomfort in abdomen. Umbilical hernia. On 2/21/2022 determined this occurred on an MSHA site.
Two miners were lifting a roll of geo grid into a loader bucket. The roll slipped out of one miner's hands and caused the other miner to lose his grip and drop the roll of geo grid on his foot. The miner broke the big toe on his left foot.
Miner was changing a v-belt on a cone screen. He was rolling the belt on the pulley and pinched his thumb between the belt and pulley. He had placed his thumb in the wrong location. As a result he received a laceration to his thumb. During the investigation it was found that LO/TO was being used as well as necessary PPE.
Miner was helping a mechanic finish repairs on a GenSet. He was installing the raincap on the exhaust stack. He was pushing down and wiggling the cap and the cap slipped on suddenly pinching the miners finger (middle) cutting the tip of it off.
Miner trying to free up rock stuck in machinery. Standing on material, material shifted when rock dislodged, his ankle smashed between couple of rocks. Reason for late filing, conflicting stories reported by miner during investigation. Not hospitalized-observation & time off. Before date of inj EE scheduled day off 6/25, 1 holiday 6/28.
