Employee was putting a piece of metal on the back of a truck when his finger was caught between the metal and the bed of the truck. Employees finger was smashed and two blood blisters formed.

On July 28, EE was changing screens in a shaker and scratched his elbow and finger. On Aug. 8 EE complained that his finger was beginning to bother him and went to the Doctor. The Doctor requested he return on Aug. 10. On Aug. 10 the Doctor determined that his finger was infected and had his finger lanced.

LIFTINGS PROCEDURES USED TO LIFT SCREENS, EE WAS LEANING OVER A SPRAY BAR USING HIS CHEST AS LEVERAGE TOLIFT UP SCREENS WITH BOTH HANDS ON A SET OF HOOKS ATTACHED TO THE SCREENS TO PULL IT UP WHEN EE HEARD A POP AND FELT PAIN IN HIS CHEST.
