Employee was operating a haul truck and hadn't notice a loader had stop to let him go by (both haul truck and loader were going in same direction). The haul truck's catwalk, on the driver's side, came in contact with the side of the loader bucket which pushed the hand rail of the catwalk through the driver side (open) window, which hit the employee on his nose and broke it.

Employee was operating a 50ton haul truck and had started dumping a load of sand when the haul truck leaned to one side at which time the operator retracted the bed of the truck which caused the truck to jerk. The operator complained of soreness to his back and shoulder.
Crusher liner fell on employee's left leg causing pain in left knee. This accident happened on 5/23/06 but employee did not go out on injury until 5/27/06.

WHILE STEPPING OFF OF TRUCK ONTO A PLATFORM TO CHECK OUT SPARKS COMING FROM A CONVEYOR, EE SLIPPED AND FELL 8 FEET AND HIT BOTH KNEES.
EMPLOYEE WAS STEPPING OFF HIS TRUCK ONTO A PLATFORM WHICH HAD CONTROLS USED TO FILL TRUCK FROM A STORAGE BIN INTO THE TRUCK TRAILER. THERE WAS A GAP AND EMPLOYEE FELL TO THE GROUND HURTING HIS SHOULDER.
