Employee was operating machinery walking along side via remote when EE noticed something in EE's eye. Eye protection was being worn at the time. Employee reported to security for evaluation and was taken to medical clinic for further evaluation. Employee had an abrasion on the eye and was treated with medication.

Employee was moving a heavy plate (spacer) used to open a door. The employee felt pain when they lifted the plate with right hand but pain dissipated and completed shift. The following morning they reported pain in the hand/wrist.

Red line case. On 2/16/2017 employee got in and out of the bucket of the HV bucket truck 5-6 times. Employee did not feel bad at that time but reported the injury on 2/22 after knee got worse.

Changing the end on chisel gun, knelt down to break fitting loose and felt pull in left low abdomen

Was holding a loading spout while coworker unbolted clamps, when last clamp came off the tube fell telescoping into itself crushing 5th digit on left hand

Cleaning a screen box on tray 16, when inserting box finger was smashed between insert and box

EE was squatting to replace a grating, stood to pick up the grating and felt severe pain to low abdomen/right groin area

ee was repacking a pump, the packing gland was stuck under the shaft he applied pressure to pull it out and felt right wrist pop and pain to right wrist

employee was welding on his knees, felt stinging on his knees and discovered that he was kneeling in caustic residue, minimal burn to left knee, 2nd degree burn to right knee

packing was leaking on valve, caustic sprayed out in face and ran down goggles and into left eye causing chemical burn

employee was sprayed in the face by chemical causing chemical burns to cheeks and lips

RED LINE CASE - became recordable on 11/23/15 when restrictions were placed, employee was working in shop welding pipe, he reached down to roll pipe on ground to apply wrap and he felt tightness to right elbow

Employee was picking up a hose and felt sharp pain in mid low back.

ee was opening a valve stem, was turning handle and cut his hand on sharp piece of metal causing laceration to 5th digit right hand

RED LINE CASE unsure of work relatedness, investigation continues employee claims hernia, no specific date of occurrence, states happened in 2014 but did not file injury with Medical department. He did come to medical but it is documented the he wished to speak with union prior to reporting, Employee did not return to file injury report

employee was closing a discharge wheel valve, felt extreme pain in both shoulders

Lifted up a cover for a pump and felt pain in left shoulder.

Employee was caustic cleaning with a pry bay to remove scale, goggles became fogged, took goggles off to wipe them off, was wearing safety glasses and pry bar fell into trench , and caustic splattere under safety glasses causing burn to left eye

We had to move the old valves to the other side where the crane could fly them out. During the process of moving the old valve over the new one, it got caught. Two people let go but the third person didn't. The valve shifted in direction of the heater and pinched his hand.

Employee was hooking up a barge at R52 loading dock, the barge shifted due to wind. He was attempting to board the barge and the barge employee pulled him onto the barge, he felt a pull in his left shoulder.

Employee was pulling a piece of plastic pipe and the pipe slipped and crushed finger causing fracture and laceration requiring sutures.

employee was shoveling hydrate and 2 hours in to job he noticed stinging and burning to both feet, chemical burns to both feet

employee was changing a valve out, he was straddling pipes and when he stepped down he bumped his left knee on pipe and his knee was caught between the pipes causing contusion to the left knee

Report of incident on 4/18/2015 employee was crossing over stair case, the staircase rocked causing employee to be unbalanced, he smashed his right thumb and bruised inside of right har when thumb and arm were contacted by the tools he was carrying

employee was closing a valve on press 8, it was getting hard to close and he felt a pop in right elbow as he was twisting the valve

Employee was rolling a pump and finger caught between shiv and belt causing nail avulsion, laceration and left third finger fracture.

RED LINE CASE injury date was 2/23/2015- did not exceed first aid level of care until 3-2-2015 employee was getting into skid loader, planted right foot and twisted to sit down, felt pop and pain in knee

Re-occurrence of back pain from initial back injury 12-2-2014. States no specific injury but turning wheel valves and hard T valves hurt irritated back condition.

Was hooking air hose up to stinger, opened air valve and hot material shot out of valve hitting hand causing thermal burn to left hand.

The injured employee sustained 3rd degree burns on both knees and lower legs from caustic liquid that seeped into his PPE

The drain had stopped flowing and line was assumed to be empty. #30 Mud Floor valve was not seated properly allowing flow to fill line and exit through plug once scale was knocked loose by contractors during their task. Supervisor was checking on progress and was walking away at the time scale was unplugged. A spray of hot caustic came in contact with supervisor's back.

At approximately 11:50AM on Friday August 29, 2014, a Turner employee was injured while disposing of scrap material in a metal recycle bin. Employee was struck on the right thumb by a troughing roller that unexpectedly shifted while being placed in the bin.

RED LINE CASE was on a ladder, dropped radio, leaned to catch radio, felt pain later in the afternoon and pain worsened the next morning.

Employee was unbolting a flange when he got product on his leg. The employee went to safety shower and rinsed and then went back to work. The employee left shortly after not feeling any discomfort.

Injury occurred 8/5/14 but did not begin reportable until 8/11/14 when restrictions were placed was opening 24 inch flange door, felt pain in right shoulder and neck
