A crew of 5 workers were installing an 8'x10' process door when the lifting lug on the door became detached. This resulted in the door free falling approximately 22"" to the ground and ultimately falling over onto one of the crew members.

Employee was crossing over a 36"" pipe in a sitting position and became unbalanced and fell to ground level. Employee experienced pain in left forearm and was treated by Doctor. Treatment include applying a sling to the arm and prescription pain medication.

Contracted service employee (seasonal) was performing landscape maintenance/mowing on site and was stung by bee/yellow jacket. Reporting under our Mine ID as contractor is an independent contractor and does not meet the requirements to have their own mine ID under 30CFR Part 50. However, the reportable should not count against 4100320 as the person is not employed by Alcoa.

Employee says that yesterday, 6/11/2018 around 9am, right leg began to hurt just below the knee and on the outside part of the calf. Employee says doesn't know the exact time the pain started and isn't sure on exactly what cause it. Employee says the pain feels like a constant Charlie horse and is having difficulty walking on the leg.

Employee was operating machinery walking along side via remote when EE noticed something in EE's eye. Eye protection was being worn at the time. Employee reported to security for evaluation and was taken to medical clinic for further evaluation. Employee had an abrasion on the eye and was treated with medication.

Over-exertion in lifting objects

Employee was moving a heavy plate (spacer) used to open a door. The employee felt pain when they lifted the plate with right hand but pain dissipated and completed shift. The following morning they reported pain in the hand/wrist.

Index finger bent backwards while pulling cable off when cable suddenly started going the opposite direction due to cable on tugger was wedged in a spot and started backspooling. Employee went to Doctor on 7/25 and was put on restrictions starting 7/26.

Red line case. On 2/16/2017 employee got in and out of the bucket of the HV bucket truck 5-6 times. Employee did not feel bad at that time but reported the injury on 2/22 after knee got worse.

Employee was scoping a job in a manlift. Employee was reaching out and up to simulate what needed to be done (not lifting anything) when EE felt pain in back. The pain did not continue and EE completed the job. After leaving work and over the weekend the pain progressed and reported pain when arrived to work on Monday 2/13.
