A temporary EE stated that sharp pain was felt while working with a second EE to install new bags onto cages for a dust collector. The injury was reported at the end of the shift and the EE declined treatment. The next day, the EE stated that the pain was bad. The EE was seen at a doctor for lumbar pain, which resulted in work restrictions.

On October 21, 2025, Schad personnel were performing demolition work on Kiln #4 at Carmeuse Lime. While moving a debris buggy into the kiln, the buggy became lodged on the ramp at the kiln entrance. A Schad employee attempted to free the buggy manually. When the buggy became dislodged, it shifted unexpectedly and struck the employee's right knee, resulting in a contusion.

Injured reports that EE was dragging a 4" corrugated vacuum hose when EE felt pain in EE's back. Injured did not report injury until 7/10/2024 when EE requested medical attention. Injured was given work restrictions on 7/10 which made the injury reportable.

Employee was tarping a trailer at Carmeuse-Grand River when they slipped and fell, striking their head on the ground. Employee sustained a small laceration above the right eye, a concussion and vertigo. Employee was transported to health center via ambulance.

EE was moving the hood rake to sweep the floor upon moving the rake, loose dust from above fell down and got in between the safety glasses effecting only the right eye. EE was treated for the injury by a Dr. on 2/1/18 and received a Rx for eye drops to treat the eye.

EE strained his back while removing debris from the kiln. Pieces of quad foil that had broken off from the main structure were lifted into wheel barrows and transported to the kiln entrance. It is believed the injury occurred tossing pieces of this material over the discharge dam. Medical Treatment Started on 1/11/13 - Rx Med / Restricted Duty

While working on a chain connector, dirt or lime dropped from above and got between the miners face and safety glasses and got in his eye. Repeated flushing did not completely relieve the pain, so miner when to care facility for evaluation. Care facility prescribed antibiotics for cornea abrasion.

Employee was cleaning a over flow from #2 packer bin on the floor at the base of the bin. Employee claims lime dust may have entered his boot at a separation in the laces and tongue. The lime material caused chemical burn to the employees 3rd and 4th toes on the left foot.

Employee removed a lid from a 55 gallon drum. The internal lip from the barrel had a sharp edge sticking out. The employee was moving the barrel and his hand slipped and his thumb was cut on the sharp edge.

Moving glycol barrels to separate out a single barrel to move it with the fork lift. Trying to spin barrel off skid pinching left index finger between the other barrels. Felt pain on left hand finger, removed his glove found left index finger bleeding. Require stitches at hospital.

EE had to reposition himself to complete his job and as he did the lime dust was disturbed. The dust collector was running @ this time. The drafting of the fan inside the wizard and EE disturbing the lime caused the lime particles to be sucked up around and underneath EE's safety glasses and into his right eye.

Employee was manually adjusting the turn heads to back fill the stone bins. The employee failed to open the dampers to air the environment out. It became heavily dusty. The employee was required to climb some stairs and stated that he couldn't breathe with the dust mask on so he removed it a few times. Employee went to doctor and was diagnosed with bronchial spasms.

EE was moving a piece of plywood, approx 25 pounds, he sat the plywood down and stated that he had received an electrical shock. The area was looked over and it was found that there was no cords in the area, he stated that he wanted it recorded that he thought he had received a shock.

On 3/16/2010, the employee reported continued pain, after believing to have tweaked it while lifting a sample bucket over the weekend. Contractor's driver finished unloading glycerol into mine's storage tank. Proceeded to unhook hose from storage tank and due to pressure build up, hose blew off of connection causing glycerol liquid to splash into driver's right eye forcing safety goggles off driver's head. Grit was in the liquid and scratched right eye cornea. Right eye is healable.

Kiln was cut back due to decreased demand for product. There is less draft through ID fan & bottom flap of dust catcher does not stay closed. A bar was placed to keep flap closed; water sprays are used to cool air & knock dust down into dust chamber. Employee removed bar to open flap; material in chamber flushed out. Mixture of water & hot lime dust created steam.

EE suffered lime burn to both eyes.The EE & his co-worker were raking refractory bricks out of the kiln hood.Was wearing the required face shield/safety glasses, he felt a sharp burning pain in both of eyes.He immediately stopped & flushed eyes out.EE then went to ER & was given Rx Eye Drops & saw a specilist that Monday.

EE was opening a 3 ft by 8 ft fiberglass top railcar lid. He raised the lid and it became stuck. The employee went around hitting the door with a hammer to break the ice free. He then went back to the lid & gave a hard jerk; the lid became stuck again. At that time the employee felt a twinge and pain in his lower back. Rx Pain Drugs and Modified Duty.

Employee had minor laceration to lip the required 2 sutures. minor bleeding, no ambulance intervention needed and No loss time event.

The maintanence crew was replacing an electric motor. The motor was lined up with two of the bolts and the motor was suspended approximately 2 inches from the base. The EE proceeded to reach between the motor and the base to line up the support bolts. As his hand was under the motor, the motor fell crushing three of his fingers. Rx Drugs, Stiches.
