Absorption of radiations, caustics, toxic and noxious substances

Employee was removing PPE, EE realized EE had some lime dust on wrists. EE removed the dust from skin. The next day EE noticed that the wrists had swollen and looked infected.

Accident type, without injuries

At 8:20am a fire was detected on #5 conveyor belt in the cooler pit. FD was contacted immediately and fire was extinguished at approximately 9:15am. No injuries resulted from the fire.

Fall from ladders

Injured was applying a temporary patch to a stone feed pipe using a ladder for access. Injured was working from the 5th rung while a second employee was holding the ladder. Ladder slid and fell causing injured to sprain EE's ankle.

Two employees worked on gratings. When assembling the gratings on the stack, grating moved and cut right middle finger of the employee. It was considered as first aid and it was provided immediately. Wound was cleared and bandaged, bleeding was stopped. Next day the finger still hurt, so employee was taken to infirmary.

Caught in, under or between a moving and a stationary object

Employee was moving the piece of bracing (5 ft, 46 lb). When EE put it down, it slipped and pinched EE's right hand middle finger against another bracing resulting in bleeding abrasion. Accident was witnessed by co-worker. Employee was transported to ER for treatment.

Employee was climbing into the man door on the side of the cooler to place a ladder to inspect the cooler. After the ladder was in place the employee attempted to enter the door and in the process twisted knee causing a sprain.

Employee was using the bobcat to clear the plug up in #1 dust chamber. A rush of dust came down around employee. The dust burned EE's cheeks on the outer edges where safety glasses and respirator fit.

Employee was working on the #2 Penthouse Conveyor speed switch when an employee working above on the screen level knocked lime fines down onto of the injured employee. Dust entered eyes causing irritation.

Over-exertion in lifting objects

Employee was rebuilding the Industrial Gun in the Maintenance shop, and while lifting the gun into place felt a strain in the shoulder. Employee requested medical treatment when condition did not improve and was given work restrictions on 3/29/2017.

Employee was walking on the dock / stone pile area placing signs for the boat delivery when EE fell on top of the stone which caused a contusion to shoulder. Injured requested medical treatment on 5/09/2016 when condition didn't improve.

Employee was replacing screen decks in the Penthouse. The combination of pushing, pulling, and overexertion caused back pain. Employee felt tightness in his back and reported to direct supervisor. Employee requested medical attention on 11/23 and was given work restrictions.

While using a come along to pull section of new tunnel together, an eyebolt broke causing the come along to strike employee in the left arm

Fall onto or against objects

Injured was checking oil levels in kiln trunion after attending man lift training for most of the day. Injured became dizzy and blacked out causing him to fall and strike his head causing laceration. Injured was taken to the ER and diagnosed with dehydration contributed to by a medication he was taking, and low potassium levels.

Struck by falling object

Injured was investigating a non-functioning heater in the pump room when he bumped a pipe, causing dust to fall and enter his eyes.

Injured was using a hammer and chipping hammer to remove a corroded bushing on a cooling tower fan shaft. A small piece of metal (1/16"" x 1/16"") from the bushing became lodged into his right forefinger during this process.

Injured was lifting a piece of steel plate to install on a tunnel feeder when he felt pain in his groin. Injured reported this incident on 4/1 but did not request medical treatment until 5/13.

Employee was cutting out a collapsed section of 4"" hose that he was using on the vacuum system in the baghouse. He was holding the hose with one hand and cutting around the diameter with a utility knife when the knife slipped out and cut his wrist. He was sent to the clinic and received two sutures.

Employee was inspecting a feeder at the bottom of a hopper for a baghouse. Equipment was locked and tagged. Inspection hatch was opened to allow material to drain out. After material stopped flowing, the hatch was removed. Material inside the hopper came loose and flowed out and the employee was exposed to hot dust.

8/3/10 EE stated he had seen the doctor about continuing discomfort from the accident. The doctor ordered a MRI on his neck and shoulder to be performed 8/6/10. On 5/12/10 EE was working between dust collectors when free standing pipe was dislodged from the debris he was removing, striking him across the back of the head, neck and shoulder.

Kiln was cut back due to decreased demand for product. There is less draft through ID fan & bottom flap of dust catcher does not stay closed. A bar was placed to keep flap closed; water sprays are used to cool air & knock dust down into dust chamber. Employee removed bar to open flap; material in chamber flushed out. Mixture of water & hot lime dust created steam.

Shooting elevations from between pier 1 on kiln #1 & #2, high heat and prolonged exposure caused pant leg to become extremely hot.

Employee reported that he was climbing out of skid steer when his right leg slipped and he hyper extended his left leg. Amendment: Claim was denied by Workmans Comp & is being disputed by company.

EE used Hi-Vac to clean dust on #2 baghouse bottom floor.End of 4"" hose was outside baghouse;EE picked up hose under his right arm, left hand holding end of hose proceeded through door.Hose hung up on railing throwing EE off balance.hose partly pulled under his arm;suction caused his left hand to be pulled into vacuum.

EE was cutting bolts on a screen deck using a hand grinder. The sparks created by the grinder where hitting his leg & one of them burned thru his jeans & burned his shin. The burn was small and had a small blister. The blister broke and he got an infection in it so he went to the clinic and was given an antibiotic.

He was adjusting a flap on the dust chamber. His finger got caught between the flap and the chamber and pinched his finger. He was sent to the clinic. His finger needed four stitches to close the wound.

EE entered the door and went down the stairs to the cement platform at the head of #3 belt. He was carrying a length of prefab railing section and the one end caught on the stairs as he stepped onto the cement landing. There was a thin layer of mud on the cement and he slipped, but caught himself and felt a twinge in his lower back.

Walking in kiln, worker did not see a piece of clinker or brick, stepped on brick/clinker, tripped and fell. The worker's face hit a pile of lime dust getting dust into his eyes. Worker was wearing safety glasses.

Slipped in mud while pushing a wheel barrow of coal and mud in #2 coal mill area. Twisted right knee that has previously been injured. NOTE: Received an injection to the knee. Mistakenly thought this incident was not reportable because the employee sought treatment as a past injury. Has come to our attention that this is a reinjury and is reportable.

While pulling a hose and walking, employee tripped over vac hose laying across catwalk and fell. Not working- lost time.
