Initially this was a first aid injury until surgery was required on 3/29/2018. Employee was pushing a camera through pipe. EE was kneeled down pushing hard when ee felt a pain. This incident resulted in surgery to repair hernia.

Showing all 27Liquid spilled off the lid of a 5 gallon bucket of oil while contractor was lifting it out of the containment area. The spilled liquid contacted EE's shirt and pants. As the day progressed EE said it was irritating on the skin.
Employee was in Hopper room shoveling out water into pails in order to remove the water from the location. At one point, the employee picked up a full pail and turned at his waist. He felt pain in his left hip.
Associate was sitting in the loader and leaned over to close the door. He placed his right hand on door frame for leverage and pulled door closed with left hand. He did not move right hand and severed the flesh from end of middle finger.
Moving a section of sand chute with a forklift and when one was lifted another chute shifted and fell on ee left foot. He was wearing steel toe boots but is still cut his little toe. Taken to hospital where he received stitches.
Associate was putting a side shield on the back of the loader when he tripped on a rock and the shield bounced off the rear tire striking the associate in the nose lacerating the nose requiring 9 stitches.
EE was hooking up chain to fuel barrel, was getting off of barrel and jammed shoulder
The associates were moving a retention pond boat by themselves and lost control. The boat slid down a water pipe and struck the associate in the face and arm, resulting in lacerations, requiring stitches.
Operator was opening a rail car door, when wrecking bar slipped out of door clamp and pinched finger on right hand between the bar and the railcar.
Dumping small lab hopper and associate hit the frozen sand with hand to free material. The frozen sand came out and the hopper weight shifted and hopper lid stuck right hand index finger, peeling skin back. Taken to urgent care and received 5 stitches.
Two associates were rotating a steel frame. The frame was pulled to the edge of the work bench and one of the associates reached to grab the frame when it fell to the ground. His small left finger was pinched between steel frame and table top resulting in a fracture.
EE was removing fuel tank from sweeper in an area with low lighting, and not sure how, but wire cut EE's middle finger on his right hand.
Associate was unloading sheet metal off of a pick-up truck with two other associates. Steel hit ground and cut left hand.
Got out of backhoe took 3 steps in loose sand, knee felt different, started to get weak that night and swell. Did not report injury until 7/17/09 to supervisor
Employee tried to kick a frozen chuck of sand loose. Kicked the ice and felt a pain in his back.
Lifting Return Rollers at Screen Tower
While lifting winch cable to unhook from railcar, cable twisted and injured lower back.
EE was replacing take-up pulleys on conveyor in a crouched/bent over position. When he stood up he felt pain in rt knee. He was in position for 1 hr+ & did NOT slip or fall, he twisted knee wrong. He had surgery on rt knee 1/26/05. Lt. duty until 2/7/05.
REPAIRING PUMP HE CUT HAND ON FRAME HOUSING.
APPROXIMATELY 100 # SAND FELL FROM EDGE OF RIB ANEAR ROOF STRIKING EMPLOYEE ALONG HIS BACK.
EE WAS HELPING TO REPLACE A PIN ON LOADER AND CUT LEFT INDEX FINGER ON SHARP METAL.
EE WAS PRYING ON PIPE TO LIFT PIPE SO LINE COULD BE CLEARED WHEN HE FELT A TWINGE IN HIS BACK.
NORMAL CLEANING OPERATION CAME IN CONTACT WITH UNKNOWN OBJECT WHICH PUNCTURED LEG REQUIRING 2 STITCHES - RAILCAR CLEANING.
TRIANGLE PIECE OF STEEL FELL AND HIT ARM CAUSING 2 PUNCTURE WOUNDS AND CHIPPING UPPER ARM BONE NEAR ELBOW.
HE WAS REMOVING A GUARD FROM A MOTOR COUPLING WHEN IT KICKED OUT AND HIT HIM IN THE ELBOW AND FOREARM.
WAS SHOVELING SAND CLEANING CONVEYORS TAIL PULLEY, TURNED TO THROW SAND AND SOMETHING POPPED IN BACK.
PULLED A MUSCLE WHILE LIFTING A PART WEIGHING APPROX. 50 LBS. CAUSED SEVERE BACK SPASM.
