Caught in, under or between a moving and a stationary object

Associate was sitting in the loader and leaned over to close the door. He placed his right hand on door frame for leverage and pulled door closed with left hand. He did not move right hand and severed the flesh from end of middle finger.

Associate was on one side of the anfo truck prepping the primers when another associate on the other side of the anfo truck let down the outriggers onto the first associate's foot breaking 3 bones.

Fall of roof occurred at intersection 235 and 106 in the primary escapeway of the mine. The fall occurred while the scalar operator was in the area scaling when a large amount fell from the back hitting the scalar breaking the window and causing minor scratches on the operator.

Associate was using the scaler to remove loose material from the rib in the 235N/106E intersection. The associate moved the scaler into 106 heading to watch the intersection shortly afterward, a 25X50 section of back came down in the intersection. No one was injured.

Associate was helping out at the washplant, raking fines at the dewatering screen when the rake got caught and yanked out of the associates hand.

Associate was walking to his car and slipped on some ice.

Struck against a moving object

Associate was operating a loader in the wet sand storage tunnels when he hit a bump in the road. He was jostled and felt a pain in his neck. Associate was sent to the hospital for an examination with sprained muscles in his back and neck. Associate returned to work the following day with restrictions. The cause of the accident was a malfunction in the 980K ride control.

Associate was putting a side shield on the back of the loader when he tripped on a rock and the shield bounced off the rear tire striking the associate in the nose lacerating the nose requiring 9 stitches.

Over-exertion (Not Elsewhere Classified)

On February 8th, associate was walking into porthole, tripped on rock and twisted right ankle. Associate refused medical attention and incident was treated as a first aid case. On July 11th, associate went to doctor for a pain that had not gone away in his right ankle. Doctor x-rayed ankle and found fracture in right foot. Associate has no work restrictions and went back to work.

Associate was unloading sheet metal off of a pick-up truck with two other associates. Steel hit ground and cut left hand.

Roof fall in primary 300ft inside porthole #1 on 90N at first intersection. Pile is approximatly 20'x20'x3'to5' deep. MSHA called with in 5 minutes. There were 5 miners and 1 contractor in mine at time of incident. Everyone got out with no injuries.

No injuries. Roof fall on the west side of the mine and the primary escape, at 90 E impeded travel. 10 miners were in the mine, zero contractors.

Struck by rolling or sliding object

The associate was working on a haul truck in section 102 east of the mine when a section of the rib arch sloughed off striking the associate's right leg between the rib material and fender of the haul truck resulting in lower leg injury.

EE was loading trucks in wet sand area noticed smoke, tried to get #7 loader out of mine and made it as far as 90 and #1 portal and loader started on fire. Put out with extinguisher. No injuries, just equipment damage. Mine evacuated, called MSHA, removed loader.

Roof fall in tunnel 86E of primary escapeway impeding travel. No injury and no equipment damage. The roof fall happened under normal mining conditions. The area was immediately barricaded and employees notified of restriction of travel and evacuated. MSHA was notified within 15 minutes (07:36 PM) via Hotline.

Struck by falling object

Employee was struck by loose material falling from roof. He sustained a lumbar strain/sprain and right hand pain and finger abrasion. He returned to work with restrictions and has a follow up appt.

Loose material fell from the back striking employee, knocking him down. Diagnosis is Sprain and Strain of knee and leg, left shoulder.

Struck by falling object

Individual was shoveling under a mine conveyer belt when he was caught by an overhead roof fall. The individual was working in an area with an overhead canopy which protected him from the majority of the roof fall. He sustained bruising and minor lacerations.
