Employee was assisting with the movement of machinery, with hook, clevis and chain attached to a track hoe. Clevis did not fit, so they were changing it out with another hook and clevis located in the track hoe. Track hoe operator tossed the hook from the excavator to the ground. EE tossed the clevis moments later, but employee reached for hook, and then clevis hit EE's hand.

Employee were trying to lift up a slab of marble and a piece broke free. The employee tried to move out of the way but the material hit him in the leg causing a deep laceration that required stitches.
Continuous twisting while lifting marble on the tile line. EE complained of lower back pain. Diagnosis-bilateral muscle stain of lower back.
